Hw t apply fr Accreditatin Requests fr accreditatin must be submitted t the Accreditatin Subcmmittee via the Site Accreditatin Unit. All requests must include a cver letter and a cmpleted Adult Medicine Accreditatin Assessment Frm (Accreditatin Survey Frm). The prcess fr accreditatin and reaccreditatin f Secndment Sites require the submissin f a cmpleted Accreditatin Assessment Frm and a letter f recmmendatin frm the Directr f Physician Educatin at the parent facility t the Accreditatin Subcmmittee fr review. The Cllege des nt rutinely undertake accreditatin assessments t Secndment Sites unless determined necessary by the Accreditatin Subcmmittee r the site is applying fr an upgrade in accreditatin status. Fr sites requesting an accreditatin upgrade, submissin f the fllwing dcuments are required t accmpany the cmpleted Accreditatin Assessment Frm and cver letter: trainee rsters, academic appintments in subspecialties, and an educatin schedule. Hspitals seeking Cllege accreditatin as a training site fr the first time must apply t the Accreditatin Subcmmittee in the calendar year prir t the year they wuld like accreditatin t cmmence. Level 3 Teaching Hspital A Level 3 Teaching Hspital shall have: At least tw departments within the Divisin f Medicine (r equivalent) that are cnsidered academic departments n the fllwing criteria: 1. The department is headed by a prfessrial r assciate prfessrial appintee based full-time at the hspital 2. The department is staffed by tw r mre senir academic appintees An established undergraduate and pstgraduate teaching prgram as well as demnstrated significant activity in clinical and basic research n the basis f grants and published papers At least nine medical specialty departments, each headed by a physician with a substantial appintment available t supervise trainees. T have a term in these departments cunted twards basic training subspecialty requirements, a trainee will need t spend at least 50% f their time in that subspecialty in at least tw f the fllwing three areas: inpatients, cnsults, ambulatry care Expsure t General Medicine must be available at the hspital, r thrugh rtatin t anther hspital At least nine psitins within the Divisin f Medicine available t basic trainees A Directr f Physician Educatin (DPE) wh shall be assisted by a senir registrar nminated by the Divisin f Medicine, and an established training prgram aimed at Basic Physician Trainees An Intensive Care Unit with a directr having a gegraphical full time appintment. Basic Physician Trainees shall have a rle in the lngitudinal care f their patients wh are admitted t the unit An Emergency Department with a directr hlding a substantial (at least half-time) appintment. Basic Physician Trainees shall have a rle in the initial management f patients admitted under their care thrugh the department Expsure t patient management in ambulatry settings. Ambulatry care is any cntact with a cnsultant physician which ccurs while the patient is nt an in-patient f a hspital. Ambulatry care wuld include cntact with patients at the fllwing lcatins: physician's ffice, clinics r cmmunity centres, patient's hme r nursing hme r hstel. Basic trainees shuld attend at least ne ambulatry clinic fr ne sessin a week Facilities and preparedness t hst the FRACP Clinical Examinatin Cmprehensive diagnstic labratry and imaging facilities including MRI and Nuclear Medicine A cmprehensive medical library apprpriately equipped fr physician training; A range f plicies and prcedures dealing with: Lifting plicy Infectin cntrl There will be a range f ther plicies dealing with ther health and safety issues Hspitals shuld als ensure that physician trainees are expsed t an envirnment that fsters and supprts quality assurance and clinical practice imprvement. These cncepts are seen as being integral t mdern physician practice with lifelng learning and Cntinuus Prfessinal Develpment. In any teaching hspital expsure t the cncepts and their peratin must ccur at an early stage f training. Hspitals are required t ensure all Basic Physician Trainees cmplete a cmprehensive Advanced Life Supprt (ALS) training curse f minimum three hurs duratin (as part f a cntinuus blck) by the end f their first term as a basic trainee, if they have nt undertaken an ALS curse within the past 12 mnths. Hspitals shuld be prviding supprt fr their basic trainees undertaking such a curse. Trainees shuld nt be placed in a psitin where they are expected t manage a medical emergency r respnse team withut adequate training. Physician Trainees shuld be adequately supervised ut f hurs and a frmal clinical handver fllwing night duty shuld ccur t prvide educatin and supprt as well as ensure cntinuity f patient care. This shuld ccur every day f the week and wuld ideally be cnsultant led invlving thse units wh are mst actively n take vernight. Accreditatin as a Level 3 Teaching Hspital allws up t 33 mnths f Basic Physician Training. At least 12 f the 36 mnths f Basic Training must be spent at a Level 3 Teaching Hspital. At least three mnths f the 36 mnths f basic training must be satisfactrily cmpleted utside Level 3 Teaching Hspital. If nly 12 mnths f the 36 mnths f basic training are spent at a Level 3 Teaching Hspital, nly up t three mnths in a secndment term will be cnsidered as part f the 12 mnths f the Level 3 Teaching Hspital experience. 2

Hwever, this can increase t 18 mnths if the trainee is, in additin t nrmal 12 mnths, secnded t Level 1 Teaching Hspital frm a Level 2 r Level 3 Teaching Hspital fr 6 mnths. Level 1 Teaching Hspital (as part f a netwrk) A hspital may be suitable fr recgnitin as a Level 1 Teaching Hspital (L1TH) if it substantially fulfils the abve criteria required fr a L1TH and the remaining criteria are fulfilled by virtue f it being part f a recgnised netwrk. In particular, adequate frmal training must be available, but nt necessarily n site. Such hspitals wuld be called L1TH (as part f a netwrk). Hspitals are required t ensure all Basic Physician Trainees cmplete a cmprehensive Advanced Life Supprt (ALS) training curse f minimum three hurs duratin (as part f a cntinuus blck) by the end f their first term as a basic trainee, if they have nt undertaken an ALS curse within the past 12 mnths. Hspitals shuld be prviding supprt fr their basic trainees undertaking such a curse. Trainees shuld nt be placed in a psitin where they are expected t manage a medical emergency r respnse team withut adequate training. 4
Secndment Site A Secndment Site shall have: At least 50 medical beds At least tw physicians n staff An emergency department Adequate investigative and educatinal resurces (Fr Secndment Hspitals within NSW training netwrks nly) a Directr f Physician Educatin (DPE) and an established training prgram aimed at Basic Physician Trainees Accreditatin f a hspital as a Secndment Site depends n the recmmendatin f the DPE at a Level 3 r a Level 2 Teaching Hspital. The DPE may decide t waive ne f the criteria if it is felt that the terms prvide sufficient and gd quality teaching relevant t internal medicine. Training in these hspitals will be n rtatin frm a Level 3 r Level 2 Teaching Hspital. A ttal f six mnths f the 36 mnths f basic training may be spent at Secndment Sites, r hspitals which frm part f the rtatin netwrk. 5
