APPLICATION INSTRUCTIONS Accelerated BSN Program All applicants must submit a new completed application for each application cycle. The School of Nursing DOES NOT keep old applications from previous cycles. All applications must be postmarked by the application deadline. Late or incomplete applications will NOT be considered. Summer Admission Postmarked by January 15 th BY THE APPLICATION DEADLINE, APPLICANTS MUST: 1. Apply to Northern Arizona University at: Northern Arizona University Undergraduate Admissions P.O. Box 4084 Flagstaff, AZ 86011 http://home.nau.edu/admissions/undergrad.asp Students will send OFFICIAL transcripts to NAU Undergraduate Admissions with application. 2. Submit a Completed School of Nursing Application (pages 8-10): Application for the Bachelor of Science in Nursing Accelerated BSN Program (Application can be submitted prior to taking the Kaplan Admissions Exam but exam needs to be completed by deadline) Be sure to include: 1. Unofficial Transcripts from each college or university attended (NAU transcripts do not need to be submitted with your application) 2. Course descriptions for prerequisites taken outside the state of Arizona 3. Contact your nursing advisor to get verification that you are eligible to take the Kaplan exam, then submit your request to the School of Nursing to register for exam. 4. Schedule and complete the KAPLAN Admissions Exam NEEDS TO BE COMPLETED BY APPLICATION DEADLINE (refer to page 7 for directions) (Results will be downloaded by the School of Nursing after the application deadline) *Incomplete applications will not be considered for admission* Please NOTE: Students are required to bear the expenses for travel and housing related to clinical experiences which may be held in outlying communities in Arizona. In addition, there is a $2400 program fee that all BSN students must pay. The $2400 fee will be evenly divided among your three semesters with your tuition fees. ADMISSION REQUIREMENTS & CRITERIA FOR SELECTION Accelerated BSN Program 1. Proof of BA or BS degree from an accredited college by program start date. 2. A minimum grade point average of 3.0 (on a 4.0 scale) in prerequisite courses. Please note, the minimum GPA of 3.0 does not guarantee admission to the program. Most successful applicants have a GPA well above the minimum 3.0. 3. Proctored KAPLAN Admissions Exam results with a minimum overall score of 70 and 65 on Science. Please note, the minimum scores of 70 Overall and 65 Science does not guarantee admission to the program. The exam results will be used as part of the process of screening and ranking applicants. (Results will be downloaded by the School of Nursing after the application deadline.) 4. Completion of all prerequisite courses with an average GPA of 3.0 or better. INSTRUCTIONS FOR KAPLAN ADMISSION EXAM Accelerated BSN Program NOTE: The KAPLAN Admissions Exam score is used for screening and ranking of applicants. KAPLAN EXAM NEEDS TO BE COMPLETED BY APPLICATION DEADLINE Kaplan Exam Students are required to get verification from their nursing advisor that they are eligible to take the Kaplan Admissions Exam. Registration for the Kaplan Admissions Exam will not be completed without advisor verification. Students may take the exam only once per application cycle. Kaplan results can be used from previous cycles, however, applicant must provide date Kaplan was taken, location Kaplan was taken and a printed copy of the results. Test taking disability accommodation is allowed for qualified students through the NAU Disability Resource Office. If this type of accommodation on the Kaplan Entrance Exam is warranted, please contact NAU Disability Resource Office at 928-523-8773 or e- mail DR@nau.edu. Please submit Disability Resource accommodation requests in a timely manner so approvals can be sent to the School of Nursing. This is n ecessary to ensure testing can occur before the application cycle deadline of March 15th and September 15th each year. The test consists of 91 questions for a total of 165 minutes. The breakdown is as follows: Reading 22 questions; 45 minutes Math 28 questions; 45 minutes Writing 21 questions; 45 minutes Science 20 questions; 30 minutes It is important to schedule the exam in advance, and email your scheduled exam information to the School of Nursing at least 5-7 business days before the exam to ensure a Kaplan username and password is generated in time for your scheduled exam date. Testing sessions are by appointment only. There is a $50.00 proctor fee that is due at the time of scheduling. 2. Send an email to nursing@nau.edu with ADMISSION EXAM in the SUBJECT line. Please indicate your registered NAME (no nicknames), the EXAM DATE, ADMISSIONS TERM applying for, and that you are taking the exam at the NAU TESTING CENTER or you may send your testing center scheduled receipt. Please send this 5-7 business days before your exam date. 3. You will receive a confirmation email from nursing@nau.edu; print and keep a copy of this confirmation to take with you to the testing center. You will be required to bring two forms of ID that match your registered name, including at least one photo ID in order to take the exam. For Students not in the Flagstaff Area: 1. Call a testing center in your local area, find out if they have proctoring services and is willing to proctor the exam. Most community colleges and universities have a testing center but do not list that they have proctoring services. You will need to contact them to find out if they have this service. Be sure to get a valid email address and contact information for the testing center. 2. Schedule an exam date. Make sure that the testing center has internet access. 3. Send an email to nursing@nau.edu with ADMISSION EXAM in the SUBJECT line. BACKGROUND INFORMATION The Arizona State Board of Nursing uses a fingerprint background checks to investigate previous felonies, misdemeanors, and arrests. If a felony is less than five years since absolute discharge, the applicant is ineligible to apply for licensure. Although having a misdemeanor or arrest is not cause to deny licensure, the behavior that caused the event may indicate unprofessional conduct and may be grounds to deny or restrict nursing licensure. The following two questions provide necessary information related to future licensure. 1. Are you currently under investigation or is disciplinary action pending against a nursing license, CNA certificate, or any other license or certification you hold in any state, or territory of the United States?
