This popular sponsorship opportunity provides attendees a chance to win the car and gives YOU additional networking opportunities. How it Works All attendees receive a Networking Passport upon on-site registration. Attendees must receive a stamp from each Participating Company and Priority Club Sponsor listed on the Networking Passport. Once the Networking Passport is completed, an ACMA Staff member will validate their passport and the attendee will be entered into the car drawing. There are 2 ways your company can participate: Participating Company - $500 - includes the following: Your booth will be listed in the Networking Passport You will have personalized stickers to be used in Networking Passport Booth signage designating your booth as a "Win the Wheels" participating company Your company listed on conference website and in printed materials Priority Club Car Sponsor - $1,500 - includes the following: All benefits available to Participating Companies Booth signage designating your booth as a "Win the Wheels" priority club car sponsor 100 personalized entry tickets to be given to conference attendees at company s discretion Sponsor Logo will be included in all pre-conference car giveaway emails and communications Should the winning ticket be your company s ticket, your company will receive a photo with the winner, a press release in an ACMA publication, inclusion on the conference web site and a complimentary post conference e- mail blast to conference attendees Priority Club Car Sponsors have the option of purchasing additional entry tickets to give away. $500/100 tickets, limit 3 per company.

HeadShotLOUNGE EXCLUSIVE SPONSORSHIP The ACMA HeadShotLOUNGE is an executive photo experience where conference attendees can sit for a professional makeup session and then get expertly photographed by the best portrait photographers in the country. Perfect for updating employer badges or social media pages, an updated and professional headshot is a must! Access to the lounge and the executive photo will be made available to attendees and exhibiting company representatives who make a nominal donation to ACMA s fundraising efforts. Tentative Dates & Times for Headshot Lounge April 14: 2pm 7pm April 15: 10am 7pm April 16: 7am 4pm The ACMA HeadShotLOUNGE Sponsorship Package includes the following: Sponsor provided logo will be included on the lounge access ticket given to attendees and exhibiting companies who donate to ACMA Fundraising Additional sponsor logo recognition will be provided in the following locations: in pre-event marketing emails on the conference website on the conference app on conference signage located near the lounge in post-event emails reminding attendees to log in to access their digital photos ACMA will verbally recognize sponsor from the stage during the conference Sponsor staff can assist in the operation or simply remain in the lounge area to network with attendees. Headshots of Sponsor staff are included at no additional cost Sponsor can provide snacks & beverages for the lounge (additional fees will apply) Cost: $20,000.00 Lanyards $1,500 Company provided lanyards will be placed by registration for conference attendees to choose. Provided lanyards must have one of the following closures to be compatible with conference badges: O-ring, Key ring, D-ring or Double J-hook. Company is responsible for any fees associated with producing/purchasing/shipping of lanyards and for contacting ACMA prior to the event to obtain quantity and shipping instructions.

WIFI Partner $7,500 Help attendees interactively participate and stay connected. Single or co-sponsor availability. Sponsor(s) Logo will be displayed on signage in key locations and by session room entrances. Wi-Fi will be available in all General Sessions, all concurrent sessions and foyers. Umbrella Sponsorship $10,000 ACMA provided umbrella will include sponsor logo and will be distributed from the sponsor s booth. Attendees will be given a ticket when they arrive at registration that they can then bring by your booth to redeem. Massage Break Lounge $5,000 Give Annual Conference attendees a much-needed break by sponsoring a special Massage Lounge. This break area will be in an adjoining 10x10 located next to your location and include 2 massage therapists and 2 massage chairs. This will generate ongoing traffic to your booth throughout the duration of the conference. Your company name and logo will also be displayed in the Massage Lounge area that will also be recognized on the floor plan. Pre or Post Conference Email Blast $750 (limited quantity available) Company provided email copy will be distributed via email to all registered conference participants (email submitted to ACMA as html code) DIRECTIONAL FOOT PATH DECALS (Two sponsorships available) Your logo will be featured on Ten directional foot path decals that will guide attendees throughout the Exhibit Hall to your booth. Sponsor Logo must be provided to ACMA as.eps file by March 26 th, 2019. Cost: $2,500.00

WELCOME RECEPTION PARTNERS April 14 from 4:00 pm 7:00 pm Join over 1,600 case management, transitions of care and industry professionals for networking and education during ACMA s 2019 National Conference Welcome Reception. There are several options for participation as noted below. PLEASE NOTE: partnership opportunities are available on a first come, first served basis. OPTION 1: Beverage Partner - $2,000.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide beer, wine, soda, water & coffee during the reception ACMA will include partner recognition on the conference app OPTION 2: Hors d'oeuvres- $2,500.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide hors d'oeuvres during the reception OPTION 3: Sweet Treats - $1,000.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide sweet treats during the reception

FOOD & BEVERAGE PARTNERS: MAIN CONFERENCE April 15-16 OPTION 1: All Day Beverage Partner - $7,500.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide an all-day beverage station during the conference on April 15 and 16 OPTION 2: Lunch Partner - $5,000.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide lunch for attendees and exhibiting companies in the exhibit hall on April 15 and 16 OPTION 3: Networking Reception Partner - $10,000.00 (multiple partnerships available) Partnership fee will be used by ACMA to provide food & beverage during the networking reception on April 15 EDUCATIONAL WEBINAR IN ACMA S MEMBER ONLY CE PORTAL Your educational webinar* will be placed in ACMA s CE portal which offers free on-demand CEs for our 8000+ members and will remain active in the portal for 1 year. This is great exposure for your company as the majority of our members are required to obtain CEs for their professional licensure. PLEASE NOTE: because we offer CEs for these webinars, they must be educational in nature, not marketing focused. If you wish to provide handouts for the educational content and/or marketing materials promoting your company as supplemental materials, those will be made available for download to anyone who views the webinar. OPTION 1: ACMA Member-Only CE Portal (pre-recorded) $1,000.00 OPTION 2: ACMA Member-Only CE Portal (recorded by ACMA) $1,500.00 *If you have a pre-recorded educational webinar, you can send the file to ACMA and we ll upload in the portal. If you do not have something pre-recorded, ACMA will work with your speaker to record and upload.

CHAPTER PRESIDENT S MEETING SPONSORSHIP April 13 from 1:00pm 5:00pm EXCLUSIVE SPONSORSHIP This exclusive sponsorship will afford a sponsoring company the opportunity to distribute materials (ie marketing collateral, sponsor provided amenity, etc.) to ACMA Chapter leaders in attendance, recognition on signage during the meeting and the opportunity to speak to the Chapter leaders in attendance for 3-5 minutes during the meeting. Please note that photography is limited to ACMA delegated staff. Event photos of sponsor will be sent electronically post meeting and the publication of photography by sponsor must be prior approved by ACMA. Sponsor is responsible for having a rep present to speak, providing designated quantity of materials for distribution and for any associated shipping fees. Contact ACMA for exact time of speaking opportunity, quantity and shipping instructions. Cost: $3,500.00 Sheraton Hotel Sponsorship Opportunities Sheraton Room-Drop $4,500 - (Sunday, April 14 th or Monday, April 15 th ) Showcase your brand by presenting attendees with a special gift that will be delivered to the guest rooms by hotel staff members and placed in a high visibility location. Room drop can be a single page, packet, or small gift item we just ask that it is limited to (1) item per person rather than multiple loose pages or items. Sponsoring company is responsible for any fees associated with producing/purchasing/shipping of collateral and for contacting ACMA prior to the event to obtain quantity and shipping instructions. Sheraton Hotel Room Key Cards $5,000 This is a simple and effective way to put your company logo in attendees hands multiple times throughout the event ensuring repeat brand awareness. Hotel room key cards are the first thing attendees will see when they check in. The company logo is to be provided by the Sponsor and ACMA will be responsible for production of the key cards. Sheraton Elevator Door Clings $1,000 Per Door 1 Day $1,600 Per Door 2 Days Capture the attention of conference attendees with your company s graphic on an elevator door. This is a unique opportunity to advertise with every open door. Sponsorship includes printing, setup & tear down.
