S.S.N.: This is the client s social security number. 6. BIRTH DATE: This is the client s date of birth (MMDDYYYY) 7. SEX: This would be M or F for Male or Female. It is not always available, as client did not specify or was not completed when client was added to system. 8. CLIENT ALERT: There is different coding that can appear next to this. This coding comes from the PRAP screen, which carries over to the CLPM.
