Clinical Audit and Assessment Provectus offers a range of unique services to providers of health and social care both in the NHS and private care industry. Advanced Nurse Practitioners and Specialist Medicines Management Technicians provide honest and balanced opinions of your business needs. This ensures provider compliance with the Care Quality Commission (CQC) Essential Standards of Quality and Safety. Our audits cover the following main areas: Clinical delivery and operations; clinical audit and assessment; person- centred care planning and nursing delivery Operational management to include the health and safety processes in place, as well as assessment of ancillary services Operational management including workforce planning; budgeting; and resourcing. Once the audit has been completed our report to the provider includes full discussion and planning to ensure the business gains maximum benefit from the findings. Ongoing support can be made available to ensure the necessary required changes are implemented. Medicines Audits Medicines audits can be provided individually where required to give the provider peace of mind in knowing they comply with both CQC and NHS standards. A Specialist Pharmacy Technician will conduct a full audit of all aspects of medicines management, and where appropriate, guidance and support will be provided to the client to ensure full compliance with CQC Essential Standards Outcome 9. 3
Advanced Quality Framework This new educational care model enables the integration of health and social care to ensure high quality care delivery for vulnerable people either in care homes or home care settings. Our Advanced Quality Framework promotes partnership working with all relevant health and social care professionals to facilitate early recognition and reporting of new and declining health conditions. This has been evidenced decrease in unnecessary contacts and admissions to Accident & Emergency Departments thus resulting in cost savings to the NHS as a public body. It has also evidenced the need for additional care home staff to provide escort, but more importantly it reduces unnecessary stress and trauma to the individual resident. As NHS commissioners constantly monitor the numbers of admissions and attendances from care homes to Accident & Emergency, any reductions reflect positively on the provider. Provectus Advanced Nurse Practitioners work alongside the key staff of providers coaching them how to use and implement the Advanced Quality Framework thus helping them to develop the necessary skills to promote positive health outcomes for vulnerable people. The Advanced Quality Framework is a new model that is designed to provide sustainable results that enable people to make choices about where and how they are cared for. This ensures that they receive the best possible care in a dignified way. The system is designed to fit in with any existing care home documentation thus reducing the need to change whole systems. It also provides additional evidence of care delivery as is unfortunately required in this litigious society. Provectus provides a full coaching and support programme that provides both theoretical and practical application of the Advanced Quality Framework. This aspect of our service is primarily aimed at care homes and home care however funding establishments such as Banks and Venture Capitalists, Local Authorities, CCGs and GPs would find this of benefit ensuring effective high standards of care delivery in the care home sector. Enabling Safe Discharge Programme Part of the Advanced Quality Framework, the Enabling Safe Discharge Programme has been designed specifically to promote best practice when patients are discharge from hospital to care home / home care. The current drive to relieve pressure on acute hospital beds is proving a challenge to commissioners, and equally care homes that are expected to accept their residents back from hospital often during the night or when staffing levels are much lower. This programme fits alongside other elements from the Advanced Quality Framework to allow medical and nursing staff in both the hospitals and care homes to have confidence that the risks, including those to the patients have been minimised and that discharge from hospital is not fraught with additional difficulties. There are few programmes that allow positive support mechanisms by health and or social care partners to care homes, and to ensure care provision for patients is met commissioners often turn to using contractual levers to promote compliance. The Enabling Safe Discharge Programme however can demonstrate how this is done using positive rather than negative motivators by facilitating true partnerships that are sustainable. The Enabling Safe Discharge Programme uses a deep understanding of how care homes operate and manage their resources, with the specialist nurses in Provectus having many years nursing experience in both acute care and in care homes. 4

She holds an advanced degree in Strategic Leadership (Learning and Development) and a BSc (Hons) in Advanced Professional Practice. In addition to these qualifications she also holds specialist Diplomas in Nursing including the ENB Higher Award. As well as being the author of many articles published in professional peer review journals, Sandra s work has also been cited world- wide by other authors. Sandra worked at a senior level in the private care home sector having controlled budgets exceeding 20 million and as a Centre Manager in specialist care set up new services for those requiring care. In this role Sandra realised annualised pre- tax profits of 1.2 million. She now uses this knowledge to assist new and existing care home providers to set up and enhance their services. Sandra has lectured at the University of Nottingham on behalf of the School of Nursing. As part of this support to the University, she also managed several student placement programmes, assisting nurses from both the UK and Overseas as part of their training towards NMC registration. Sandra has over 30 years specialist nursing experience ranging from ITU to Brain Injury Rehabilitation. This is combined with over 10 years Senior Management (Operational and Clinical) experience in the private health and social care sector. Sandra works with providers to get the best out of their business whilst ensuring high quality nursing and care service delivery. Sandra has more recently worked with the NHS providing innovative approaches to Quality Monitoring: being the primary designer for i- Care an electronic quality assessment tool that is accepted by the DoH for use in the East Midlands. Sandra Jackson BSc (Hons) Advanced Professional Practice, Cert HE, PG Dip, SPGP, RGN Sandra Jackson is a Registered Nurse RGN holding a BSc (Hons) and a Specialist Practitioner Degree in Primary Care and Post Graduate Diploma in Advanced Nursing Practice and non- medical prescribing. Sandra used her expertise to develop services for older people in Primary Care. This role enabled Sandra to develop new models for nursing assessment for the elderly, and along with Sandra Horton, this has resulted in the progression of Provectus programmes and tools. Sandra demonstrates innovation by continuously designing and developing clinical tools to promote improvements in care. Sandra is passionate to develop SMART objectives using new concepts for improved care delivery. Alongside gaining valuable experience working with young adults in a variety of settings Sandra has worked closely with all age groups of vulnerable people in a variety of settings. Her latter focus has been on care of the elderly providing clinical assessment, care planning, risk management and evidence based best practice. Sandra is an Advanced Nurse Practitioner with many years experience and holds the Non- Medical Prescribing Qualification being an autonomous practitioner in her own right. She undertook project work in a consultancy role for Trent Strategic Health Authority (SHA) to advise and standardise practice in Offender Health and has previously worked in Primary Care Trust (PCT) / Prison Healthcare for nine years, during which time she has been acknowledged by the National and Regional Offender Health Teams as being visionary and has attained Gold Standard Services within prison health.
