We ask that museums receiving the kit report back about your experiences through this online report. This report contains questions to help us understand the kinds of events your museum hosted and how the kit materials were used. As part of your application to receive a kit, you committed to this reporting requirement. The reporting deadline is January 31, 2017. Important Information About Filling Out the Report: The report takes approximately 15 minutes to complete.
