LONG TERM CARE We offer an array of specialized long term services aimed at the needs of our residents. With our Medical Director on-site, we offer physician services seven days per week through an in-house physician covering service. Our physicians are expert in the needs of our Long Term Care residents and collaborate with the interdisciplinary team. We offer on-site consultative services in: Audiology Dermatology Dysphagia (Swallowing) Care Medication Management Optometry Pain Management Podiatry Psychiatry Psychological Counseling Speech & Language Wound Care WanderGuard We have a Roam Alert System that allows for mobility within the organization with its goal of securing the safety of our residents. Care Team Meetings Every family is invited to meet regularly with our team of highly trained professionals to review and participate in the resident s treatment and plan of care. The interdisciplinary team consists of a physician, psychiatrist/ psychologist, nurses, certified nursing assistants, social workers, recreation leaders, and dietitians. Members of this team are specialty trained and dedicated to the care of the long term population. Somers Manor is recognized for its resident centered care. With day rooms and cozy corners on every long term care unit, a homelike atmosphere is fostered. Hospice/Palliative Care Program We offer end-of-life care by coordinating with Hospice and Palliative Care for residents and families in need of comfort care. Our program focuses on managing pain, while providing emotional, social, and psychological support to the resident and family members.

REHABILITATION & POST-ACUTE CARE We have a dedicated Rehabilitation Services Center, totaling over 5,000 square feet of space, with technologically advanced equipment. Our Rehabilitation Center is conveniently located adjacent to our two Post-Acute Rehabilitation Units with 67 private rooms. The Rehabilitation Center is expertly divided between Physical and Occupational Therapy so as to better serve the plan of care of our residents. Rehabilitation Services are offered 7 days a week. Our Speech and Language Pathology space, at 400 square feet, is located in a discrete area so as to optimize the care and assessment of speech and swallowing therapy needs. Additionally, we have a common area between the center and our Rehabilitation Units for ambulation and mobility training, which is almost 1000 square feet. Our residents receive therapy in an optimal location and spacious area, with the best equipment and expertise to achieve their rehabilitation and discharge goals. Continuity of Care Our Electronic Medical Record allows us to communicate with the physicians in the community, as well as the hospitals in which we serve. Discharge Planning Home Safety/Consultation Medical Equipment Therapeutic Appliances Medication Modalities Pharmacy Services Our on-site Pharmacy automated dispensing unit (ADU) allows for accurate and timely medication management. Hospitality Room We understand that family and friends contribute to the healing process. We welcome all families and visitors to our inviting Hospitality Room on Main Street where coffee and snacks are available. Our Hospitality Room has a television and restroom for your comfort while visiting.

ALZHEIMER S & DEMENTIA CARE We have a specialized Memory Care unit that allows residents an enhanced quality of life that specifically caters to patients with Alzheimer s Disease, Dementia, and other types of memory problems. Our dementia program is structured to provide a unique, nurturing environment for the residents that we serve. Through an enhanced recreation program, thoughtful physical design, and specialty trained staff, the dementia residents are afforded opportunities to maintain their highest possible level of physical, social, and psychosocial functioning. Behavior Health We offer specialized behavioral support through psychiatric and psychological counseling services. Our services utilize the appropriate interventions to minimize risk, ensure safety, and improve quality of life. Alzheimer s & Dementia Support Group The Somers Manor Support Group is designed to provide emotional, educational, and social support for caregivers through regularly scheduled meetings. The support group encourages caregivers to maintain their own personal, physical, and emotional health, as well as optimally care for their loved one with Alzheimer s/dementia. The group meets once a month and hosts guest speakers every other month. Our own team of social service professionals expertly guides the group s process. *The Somers Manor Support Group is open to all caregivers in the community.

SOMERS MANOR SERVICES Our Recreation Department offers therapeutic programs and activities from 9:00am until 8:00pm, 7 days a week. We have skilled recreation leaders dedicated to the residents socialization. The Recreation Department provides a full calendar of social events designed to fuel the mind, nurture the body, and kindle the spirit. Our outdoor, heated courtyard allows for recreating and leisure activities for our residents and families. During the summer months we host a weekly outdoor barbecue. Banking: For personal banking needs, we offer on-site business services. Beauty Parlor Services: Our beauticians are available every Tuesday through Friday, starting at 9 am, for all hair and styling needs. The Salon is located on Main Street in a separate area for your comfort, privacy, and enhanced beautifying experience. Family Council: Family Council Meetings are held quarterly at the discretion of the Family Council to afford families a venue to collaborate and bring forward issues to the organization. General Store: Our General Store offers assorted pastries, snacks, and drinks for residents and families. We encourage you to visit us on Main Street and chat with our recreation staff and community. Proceeds from the store are deposited into a Residents fund, whose use is at the discretion of the Resident Council. Religious Services: Several religious denominations are represented on a weekly basis to nurture the spirit. Resident Council: Resident Meetings are held monthly to discuss agenda items raised by the residents and addressed with input from leadership staff. The Resident Council meetings discuss the use of Resident funds for special events, new programs, recreation items, and facility enhancement. Shopping Services: Shopping services are offered twice per month for jewelry, make-up, and beauty needs. Technology: There is free Wi-Fi throughout the facility. Free telephone and television services are included in the stay with us. We have a spacious Activities Center with an area designated for computer use.

Somers Manor Rehabilitation and Nursing Center, a family owned Rehabilitation and Skilled Nursing Facility for over 55 years, is nestled on more than 100 acres in Northern Westchester County, New York.
