Internship Position at the Richard H. Fine People s Clinic, Hospital Application Deadline: Friday, April 27, 2018 The Richard H. Fine People s Clinic at the Hospital is offering year-long internship positions during the 2018-2019 academic year for Pre-Medical or Post-Baccalaureate students to gain valuable experience working in a vibrant academic program for underserved primary care in the San Francisco safety net. The mission of the Richard H. Fine People s Clinic (RFPC) at the Hospital is to provide comprehensive, high quality primary care services for underserved patients. San Francisco is known for its cultural and ethnic diversity. Patients and staff at Zuckerberg SFGH and RFPC reflect that diversity, with each major racial/ethnic group representing roughly a quarter of our patient population. RFPC provides primary care to more than 8,000 patients, the majority of whom are low-income individuals with complex medical conditions. RFPC believes in service learning and is an ideal site for someone interested in a future career delivering primary care for the underserved. Our RFPC interns are integral members of the clinic and work closely with our management team and the clinic medical directors to coordinate quality improvement efforts for our underserved patient population. Internship activities would include direct patient contact, quality improvement opportunities, program development, health coaching, and opportunities to shadow providers in clinic. Interns will be involved in mentored projects that can result in scientific presentations and journal articles. This coming year we are excited to offer several internship positions within the clinic. Each position would have a focused project, outlined below, and interns would also be able to tailor their experiences to their interests through other projects and shadowing. We are requesting a minimum commitment of 2 days per week (12-15 hrs/wk), for a minimum of 10-12 months, starting in the summer (June-July) 2018. The days of the week can be flexible between the hours of 8am-5pm. We understand the demands of applying to medical school and offer flexibility for interview travel. We view this internship as an educational opportunity in the delivery of high quality primary care

services for underserved and vulnerable populations. We also are happy to assist with mentoring for the medical school application process. Please send application materials to Ghezal Saffi at ghezal.saffi@ucsf.edu Description of Positions: Medical Scribe Interns (7-8 positions) Interns will work side-by-side along with a faculty physician during their primary care sessions at the (RFPC) to support documentation in the electronic medical record through scribing and provide health coaching services to patients. These positions provide interns with experience working directly with faculty and patients in a vibrant primary care clinic and training in medical scribing. Quality Improvement/Preventive Health Intern (2 positions) The Preventive Health Intern will work on the RFPC initiative to keep GMC patients healthy by offering a robust outreach and in-reach panel management for cancer screening, smoking cessation, and diabetic eye care. In this role, the intern will manage patient registry data, make outreach calls to patients, coordinate warm handoffs with SFGH Ophthalmology clinic, and help evaluate the quality improvement initiatives. Patient coaching will be a

key part of this intern's skill set and training in coaching skills will be offered at the outset of the internship. Hypertension Quality Improvement Interns (2 positions)- RFPC launched the Bring It Down GMC! hypertension control initiative in 2014 to improve care of patients with high blood pressure. The Hypertension QI intern will work closely with the inter-professional Bring It Down team to implement our blood pressure control initiatives, which focus on outreach to patients and reducing racial/ethnic disparities in blood pressure control. The hypertension intern will focus on outreach and coaching for patients with poorly-controlled hypertension. In this role, the intern will work directly with patients to educate them about their blood pressure and refer them to nurse visits. They may also set action goals and refer patients to free physical activity and wellness resources. We would prefer for our hypertension intern to have a specific interest in improving health equity. Diabetes Quality Improvement Intern (1 position) The Diabetes QI intern will focus on closing care gaps for diabetes patients. They will outreach directly to patients to help control their blood sugar, obtain recommended screening to protect their kidneys, eyes and feet, and refer patients to free physical activity and wellness resources. The diabetes intern may also assist in implementing a therapeutic food pharmacy, which will provide fruits and vegetables in clinic to patients with diabetes. Women s Health Quality Improvement Intern (1 position) The Women s Health Quality Improvement Intern will work on the RFPC initiative to improve pap smear and mammogram screening rates for our patients. He or she will work closely with our RFPC physician lead for women s health initiatives and quality improvement team to develop a strategy for improving pap smear and mammogram screening rates including direct outreach to patients, managing the patient registry data, and providing health education and guidance for our patients. In this role, the intern will have the opportunity to interact directly with primary care providers to identify strategies to assist individuals in helping to reach our clinic-wide goals for screening rates. Depending on interest, there will also be opportunities to shadow in our RFPC general medicine and women s health clinic. Intimate Partner/Interpersonal Violence (IPV) Quality Improvement Intern (1 position) The IPV Quality Improvement Intern will work in conjunction with the RFPC physician lead for women s health initiatives, the Aspire to Realize Improved Safety and Empowerment (ARISE) team, and the quality improvement team to assist on various clinic-wide strategies focused on universal education in the area of IPV. The intern will have the unique opportunity to work with an interdisciplinary team that has a range of projects including universal health education, patient-provider conversations around relationships, and quality improvement initiatives. He or she will also be responsible for managing the patient registry data and

supporting the current team members who are carrying out our initiatives in clinic and in the San Francisco Health Network. Depression Care Quality Improvement Intern (1 position) - The Depression Care Quality Improvement intern will work on efforts to improve depression screening, diagnosis and treatment efforts in RFPC. Depression is one of the most common chronic medical conditions seen in primary care and if undiagnosed or untreated, can result in poor patient outcomes, poor adherence to care and decreased quality of life. The intern will gather and analyze data for the depression registry, help coordinate teambased depression care and help RFPC meet our goals on depression screening and remission metrics. The intern will be supervised by the Associate Medical Director overseeing behavioral health integration efforts and initiatives in RFPC. Resident Project Intern (1 position) RFPC is a teaching clinic for internal medicine resident physicians, medical students, nurse practitioner students and others. RFPC's primary care resident physicians do a variety of innovative quality improvement projects on areas like HIV prevention, hepatitis C treatment and strengthening relationships between patients and members of their care team. The intern will interact directly with residents and patients to help residents gather data for their projects and implement changes. This intern will also have the opportunity to attend components of the resident quality improvement curriculum. Chronic Pain Management Intern (1 position) The Chronic Pain Management Intern will work on the RFPC initiative to improve chronic pain management. In this role, the intern will manage patient registry data, make outreach calls to patients, and help evaluate the pain management program. He or she will also collaborate with redesigning our clinic refill system and assist in the process of multi-disciplinary review of challenging cases. The intern will have the opportunity to work directly with patients in our opioid refill clinic if interested. Anticoagulation Quality Improvement Intern (1 position)- The anticoagulation intern will work on the RFPC initiative to improve care for patients on chronic anticoagulation (blood thinner medications used for different cardiac conditions). In this role, the intern will manage the patient registry data, outreach to patients, and help design interventions to improve care of patients on anticoagulation. The intern will work closely with a team of pharmacists, primary care physicians, and cardiologists. Patient Advisory Board Intern (1 position) RFPC values patient engagement and launched a Patient Advisory Board in January 2013 that brings patients together to provide feedback on the clinic and help improve services. The intern will interact directly with patients, and will be involved in planning,

implementing, and leading advisory board meetings. This intern will also work on chronic disease group visits, which are currently held for chronic kidney disease, diabetes, and hypertension. Practice Transformation (1 position) RFPC is a patient-centered medical home, using a team-based care model to provide improved healthcare services to patients. The Practice Transformation Intern will be involved with implementing and evaluating the patient-centered medical home model. In this role, the intern will work within the clinic to analyze data from the clinic registry system in order to assist with quality improvement efforts and interdisciplinary team-based care. Medical Education Intern (1 position)- RFPC is a training site for the UCSF Internal Medicine Residency Program. Through their time in clinic, medicine residents learn how to provide primary care for vulnerable populations. The medical education intern will help track resident data, assist with the development of curriculum, and schedule conferences and other educational opportunities. Patient Safety Innovation Intern (1 position)- The RFPC is participating in a quality improvement project to design and develop technical and workflow interventions that prevent medical errors and improve the safety of care. The intern will interact directly with providers, assist in data analysis, and help document patient monitoring practices. Specifically, he or she will help facilitate primary care provider focus groups on test result notification practices.
