Background The demographic development challenges healthcare globally The population s care needs become more demanding and complex One way to respond to the increased need for expertise is the introduction of AKS AKS is a specialized nurse with master's degree. They have greater expertise in pharmacology, pathophysiology and clinical research methods with a specialization in geriatrics, which is a better starting point in addressing the aging patients and their health. In meeting the challenges of the future, the healthcare starts to recognize that practice has to transform. A healthcare able to adapt where patients are treated with a holistic approach. A core quality is to give elderly patients the right to geriatric competence.

Nordic model The Nordic definition of AKS is a nurse with advanced clinical skills who independently will assess, diagnose, and treat common acute health needs and health status and be responsible for and manage follow-up and care of long-term health needs in an advanced manner. She / he can systematically perform a comprehensive clinical examination of the patient and investigate the patient's health history and health needs in depth. Based on this clinical assessment, he / she has the preparedness and ability to determine the patient's health needs and perform the nursing care and treatment required. He / she may also prescribe examinations and medication, referrals and enroll or discharge patients, ie, provide broad-ranging care and treatment in a particular area of specialization. Lead and coordinate health promotion and disease prevention work. Other key areas of expertise include ethical decision making, cooperation, consultation, case management, leadership, research and development (Schober & Affara, 2006). An advanced clinical nurse has an education as a nurse, enough work experience and an education equivalent master's degree in advanced clinical nursing (Fagerstrom 2011).

AKS in primary care An Advanced Clinical Nursing clinic is conceived as part of primary health clinic. Activities are for patients with both acute and chronic conditions. Goal 1. Care of patients who are seeking for acute problems 2. Care of patients with chronic conditions for follow-up and treatment according to clinical guidelines 3. Planning and development of nursing at the health clinic. 4. Teaching and counseling staff at the health clinic and in home care.
