Radiology - Kudos Award Program The KUDOS! Award Program is a Radiology Department only recognition program and it recognizes staff employees who have gone the "extra mile" and performed "above and beyond his/her daily job responsibilities. Radiology campus employees & other departments units that help make Radiology process & operations run smoothly are eligible for this award Email nominations to rad-kudos@ucsf.edu Award of Excellence certificate, and $25 Target gift card (up to 3) Gift card limited to 3/year, but can continue to receive certificates There is NO limit to the number of award nominations which include an Award Certificate! 3
Cathy Garzio Award for Outstanding Campus Staff Employee of the Year Named after former Chief of Administrative Director, Cathy Garzio, for exceptional administrative campus staff. Awarded 1/year at the annual Holiday Party Recognition plaque and $2,000 award May nominate more than one Radiology staff career employee or past winners Criteria: 1. Career campus staff 2. Substantial contributions 3. Attributes: Integrity, Collaborative Spirit & Continuous Improvement. 4
UCSF Campus Employee STAR Program The Staff Appreciation and Recognition Program (STAR) is a cash recognition program designed to recognize and reward excellence in University service, significant achievements and contributions, and/ or outstanding individual and team performance. Eligible Campus Staff Career Employees: Two types of STAR Program awards: 1. Spot Awards (contributions in short period - $1K/10% of salary) 2. STAR Program: Spot Award Recognizes significant employee achievements/contributions, for a specific project or task over a relatively short period of time e.g. On the spot Eligible Radiology Campus Employees $1,000 or up to 10% of base salary (whichever is lower) Email nominations to rad-spot@ucsf.edu 6
STAR Program: Achievement Award Recognizes sustained, exceptional performance and/or significant contributions from an employee(s) over an extended period of time. The contribution that is being recognized at the Achievement Award level must reflect at least one of the Chancellor s Priorities and/or the UCSF PRIDE Values. Chancellor Awards Award Description Each year these awards recognize and honor three individuals at UCSF one faculty, one staff, one student/resident/postdoctoral scholar who have demonstrated outstanding commitment and service to the ideals of diversity, equity, and inclusion beyond the scope of their job, area of research, or training. 13
Chancellor s Awards All four Award Categories include the following criteria! Each year this award recognizes 3 UCSF individuals 1 Faculty; 1 Staff; and 1 Student/Resident/Postdoctoral scholar Award Criteria Demonstrated outstanding commitment and service to the ideals of diversity, equity, and inclusion beyond the scope of their job, area of research, or training. Maintain at least a 50% appointment in 1 of the 3 appointments listed above. Have not previously received the award. 14
Chancellor Awards Disability Service Award Description Each year this award recognizes and honors three individuals at UCSF one faculty, one staff, one student/resident/postdoctoral scholar who have demonstrated outstanding commitment and service in furthering the goal of enhancing accessibility of the accommodation and full inclusion of people with disabilities within the UCSF community, beyond the scope of their job, area of research, or training. 15

Have been nominated by their supervisor and endorsed by their department head. Have not previously received the award. Have exhibited and sustained outstanding service to the University. 17
Chancellor s Awards Exceptional University Management This award recognizes exceptional departmental and employee management that goes above and beyond the scope of a manager s role and is inspirational to future managers and supervisors. 18

Holly Smith Awards Exceptional Service to the School of Medicine Award Description: The award was established in 2000 to recognize exceptional career service that has resulted in broad and long-lasting benefit to the School of Medicine. The award is named for Lloyd H. (Holly) Smith, Jr. whose 35-year career as Chair of Medicine and Associate Dean exemplified such an outstanding career effort. 23

Holly Smith Awards Continued. Awards will be presented at the Annual Salute to Excellence reception held in May 2017. Recipients of the awards will receive a prize of $5,000 and their names will be added to the Holly Smith Award plaque outside of the Dean s Office. 25
