Overview Mariers Hospital coducted a commuity health eeds assessmet i 2016 to better uderstad the healthcare eeds of the commuity it serves i the Upper Keys of Moroe Couty. As a result, the followig six priority areas were idetified: Availability of Primary ad Prevetive Care Chroic Disease Maagemet Barriers to Accessig Care Access to Care (uisured) Metal Health Socioecoomic Challeges This writte implemetatio pla addresses each of these six priority areas, with strategies orgaized aroud each priority area. 1

Availability of Primary ad Prevetive Care Icrease Primary Care Providers The Uited States has a recogized shortage of primary care physicias ad Moroe Couty has fewer primary care physicias per 1,000 residets tha the atioal average. Makig these key healthcare providers available to deliver routie care ad maage chroic disease coditios before they flare up ito acute episodes is critical to a well-fuctioig local healthcare system. Recogizig this, Baptist Health plas to ivest heavily i icreasig the supply of primary care providers withi our commuity. Eight primary care cliics, each followig the best-practice patiet-cetered medical home model have recetly opeed, icludig two i souther Dade, the area i the study exhibitig the greatest eed. Success i this edeavor will be judged by reduced emergecy departmet use ad a reductio i prevetable admissios. Support of Primary Care Providers Through Cotiuig Medical Educatio Activities Baptist Health provides cotiuig medical educatio i support of its primary care providers ad allied health professioals through its accredited medical educatio program. I the past year, more tha 251 educatioal courses were held, amoutig to more tha 1,770 CME/CE credit hours. This beefits the commuity greatly by keepig physicias up to date o approaches to prevetio, diagosis ad treatmet i the primary care settig. Icreasig Primary Care i the Commuity Through Academic Affiliatio With Florida Iteratioal Uiversity Baptist Health is committed to teachig ad educatig future physicias. West Kedall Baptist ad Baptist Health has etered ito a affiliatio agreemet with Herbert Wertheim College of Medicie to have our hospitals ad outpatiet ceters serve as traiig sites for Florida Iteratioal Uiversity medical studets. These studets begi rotatig durig their first year i our emergecy departmets, ad rotate throughout their traiig i a variety of other specialties. 2

Chroic Disease Maagemet Commuity Offerigs: Exercise, Nutritio ad Weight Maagemet Mariers Hospital cafeteria provides discouted Welless Advatage meals durig luch to promote affordable healthy diig choices. These meals are specially desiged by Baptist Health dietitias to be lower i calories ad saturated fat. Complete with a free bottle of water, these are set at a price poit that makes it the lowest cost meal optio, thereby ecouragig healthier eatig habits. Welless Advatage meals are available to ayoe, icludig visitors ad employees alike. Our Welless Ceter is ope to the public. The Welless Ceter offers complete health ad fitess assessmets, icludig: restig blood pressure ad heart rate, body compositio, blood cholesterol ad glucose, flexibility test, ad stregth ad edurace tests. Based o the assessmet results, aswers to a detailed health questioaire ad specific fitess goals, a exercise physiologist will develop a idividualized exercise program ad guide members through workouts. I additio, services iclude persoal traiig by exercise physiologists ad group exercise classes. Welless o Wheels is a eed-based commuity service, facilitated by exercise physiologists that is geared toward educatig ad implemetig healthy lifestyles ad habits. The service is free to busiesses, schools ad other orgaizatios located i the Upper Keys. The program cosists of educatioal semiars o utritio ad exercise, alog with exercise classes desiged to meet the specific eeds of the participatig orgaizatio. Mariers Hospital will cotiue to make this service available to the commuityas requested. Commuity Screeigs ad Health Fairs Baptist Health s Commuity Health departmet orgaizes ad staffs free health fairs that iclude screeigs for cholesterol, blood pressure, glucose, body compositio ad osteoporosis. The screeigs occur both o the hospital grouds ad at commuity-based public evets. Durig the evets, screeig participats are couseled about their results ad give istructios ad educatioal iformatio o how to prevet or maage their risk for chroic disease. Durig the past year, Commuity Health coducted more tha 272 health-screeig evets for Baptist Health. Diabetes Educatio Program Diabetes is a chroic disease of icreasig prevalece i the U.S., ad is associated with icreased morbidity ad mortality for patiets admitted to acute care settigs. Baptist Health plas to provide i-depth diabetes educatio o this ipatiet populatio i a effort to reach a critical mass of diabetic patiets withi the commuity. It will offer a multifaceted approach, begiig with teachig survival skills (i.e., isuli admiistratio techique, blood glucose testig, basic utritio ad whe to call the doctor). High-risk patiets will be idetified ad referred to outpatiet educatioal classes ad olie semiars, ad will receive follow-up phoe calls from diabetes educators. I additio, Baptist Health will offer frequet iteractive classes at o charge to discharged patiets ad the geeral commuity orgaized through Commuity Health. Baptist Health also will dedicate additioal resources to its Commuity Health departmet to provide diabetes educatioal programmig. I 2017, Baptist Health will lauch the diabetes prevetio program, aimed at idividuals with pre-diabetes i a effort to educate ad ecourage behavioral chages ad ultimately prevet diabetes. Support Groups A importat psychosocial compoet of maagig chroic disease is to have support groups i place where people with similar diseases ca get together ad share the challeges they face livig with their disease. These groups also ca become a source of hope ad stregth to participats as they realize they are ot aloe i their sufferig ad ca share stories o how to successfully cope with their disease. It also ca play a importat role i educatig them o commuity resources that may be of beefit to them. Baptist Health ecourages ad promotes support groups through advertisig ad the use of our facilities to host groups. Baptist Health hosts 55 support groups regularly to address chroic diseases such as diabetes, heart disease ad cacer. The groups are coordiated at the corporate level by the Commuity Health departmet. Cacer Care Ope a state-of-the-art Miami Cacer Istitute, icludig cuttig edge techologies such as proto therapy. Successfully icorporate supportive care ad prevetative services to meet the eeds of our commuity. Mariers Hospital is a satellite locatio for the Istitute, ad treatmet will cotiue to be provided for Upper Keys residets. 3

Barriers to Accessig Care Trasportatio Mariers Hospital ackowledges the difficulty Upper Keys residets face if they do ot have access to trasportatio, ad Mariers will help patiets access the local services available. Moroe Couty offers Paratrasit services to elderly, disabled ad trasportatio-disadvataged residets. Mobile Iitiatives Baptist Outpatiet Services has lauched Care o Demad, a telehealth service offerig 24/7 access to healthcare providers. Exteded Hours To better meet the eeds of a diverse commuity, Baptist Health Medical Group primary care ceters will offer exteded hours, icludig early morig, eveig ad weeked hours, as will Baptist Health urget care ceters. 4

Access to Care Commuity Cliics for the Uderisured ad Uisured Baptist Health directly fuds the operatios of local commuity cliics that provide healthcare to uisured ad uderisured members of the South Florida commuity. Baptist Health fuds five cliics: the Good Health Cliic, Good News Care Ceter, Ope Door Health Ceter ad South Miami Childre s Cliic, ad Carig for Miami Project Smiles. I some istaces, Baptist Health s cotributio amouts to more tha half of the operatioal budget for these cliics. I 2015, Baptist Health s combied cotributio exceeded $2.3 millio, eablig these cliics to care for patiets i more tha 18,000 visits. These existig commuity cliics have greater eeds beyod fudig their cotiuig operatios. They eed parters to doate medical supplies ad pharmaceuticals ad to provide follow-up services, such as advaced diagostic testig ad surgery, to treat patiets coditios ad prevet them from worseig. Baptist Health has doated over $250,000 worth of pharmaceuticals aually to the commuity cliics. I 2015, cliic patiets sought ad received follow-up ad diagostic care at Baptist Health hospitals more tha 7,300 times. The cost of this free care amouted to over $4.7 millio. Support Providers i Carig for the Uisured Baptist Health directly supports the care of the uisured i two ways: free hospital care to idividuals qualifyig uder the Baptist Health charity care policy, ad direct paymets to commuity physicias to treat these patiets at the hospitals. Baptist Health s charity care program has bee a itegral part of fulfillig our missio to care for those less fortuate throughout our history. Baptist Health has icreased its effort to provide free care to those who eed it ad raised public awareess of this assistace through commuity outreach ad educatioal iformatio give to patiets. The program provides free care to idividuals ad families with icomes up to 300 percet of the federal poverty level. I 2015, more tha 10,100 Baptist Health hospital patiets received charity care at a cost of more tha $60 millio. I additio, Baptist Health ecourages our affiliated physicias to care for uisured patiets i the hospital ad i their offices o a timely basis by providig direct subsidies to them. I 2015, Baptist Health hospital paymets to affiliated physicias for ucompesated care ademergecy departmet coverage totaled more tha $8 millio. Erollmet Assistace Healthcare Coverage Educatio ad Assistace Baptist Health believes all members of the commuity should have a medical homewith a secure relatioship with a trusted primary care physicia. A log-stadig barrier to this objective has bee the rates of uderisured ad uisured residets i South Florida. Baptist Health plas to cotiue our efforts to educate patiets about the importace of primary ad prevetive care, ad health isurace coverage. Baptist Health ca help patiets uderstad their eligibility for govermetal programs. Yet, Baptist Health uderstads that educatio ad applicatio assistace aloe are ot eough to esure access to timely care. Isurace premiums ot covered by govermet subsidies, high deductibles, co-paymets ad coisurace are ofte fiacial barriers to timely care. To prevet mior health issues from escalatig ito acute episodes that require expesive emergecy ad/or ipatiet care, the orgaizatio is committed to idetifyig iovative ways to remove these barriers for patiets to access timely primary care. 5

Metal Health ad Substace Abuse Support Groups Recogizig the substace abuse takig place i the commuity, Baptist Health hosts regular behavioral health support groups. Bipolar disorder support groups, alcoholics aoymous, arcotics aoymous ad depressio support groups are examples of regularly held meetigs at Baptist Health facilities. Behavioral Health Screeig All Baptist Health Medical Group sites offer depressio screeig ad referral services at primary care locatios. I additio, all Baptist Health facilities that provide labor ad delivery services offer post-partum depressio screeig, educatio ad referral for follow-up care. Baptist Health recogizes the icreasig strai metal health issues ca add to healthcare i geeral, ad all etities are committed to stregtheig existig parterships with metal health providers i the commuity. Care ad Couselig Services Baptist Health s Pastoral Care departmet provides free depressio screeig ad axiety resiliece educatio to those i eed. 6

Socioecoomic Challeges Due to the complex socioecoomic eeds of our patiets, we kow that we have to step outside of our four walls ad look at ways to egage commuity stakeholders to help keep our populatio healthy. We have leared that while we are meetig our patiets healthcare eeds, their basic eeds are ot met. May of the idividuals i our commuity are challeged with several barriers that impact their health status. Our challege is to egage ad stregthe commuity support systems after patiets leave the hospital. Eligibility Specialists We offer a professioal service to evaluate our ufuded patiets applicatios for govermet assistace (i.e., Medicare ad Medicaid) ad our charity program. Baptist Health plas to expad its erollmet assistace resources to iclude healthcare marketplace applicatio assistace.
