What if there were steps you could take to protect yourself from serious diabetes complications? Getting regular check-ups can help keep diabetes under control and avoid other health problems. Talk with your primary medical provider (PMP) about how often you should get these tests: Dilated eye exam. Regular dilated eye exams can help detect eye problems caused by diabetes. An eye doctor can check your eyes for any problems. Hemoglobin A1C. This blood test is done at the lab to measure your average blood sugar levels over a few months. This gives your PMP more information about your blood sugar and how well your treatment plan is working for you. Foot exam. Diabetes can hurt the nerves in your feet. A regular foot exam by your PMP can check for signs of nerve problems. Kidney tests. Your PMP will use blood and urine samples to check if your kidneys are healthy. How would it help you to have a nurse who can work with you by phone whenever you need help? That s what our Disease Management Centralized Care Unit (DMCCU) can do for you. DMCCU works closely with you to help manage your health care and get the care you need from your health care providers. We can talk with you and your health care team, mail you information to learn how to manage your health conditions and help you get the equipment and services you need. Our programs include case management for: Lung conditions like asthma and chronic obstructive pulmonary disease (COPD) Diabetes HIV/AIDS Heart conditions like coronary artery disease (CAD), congestive heart failure (CHF) and hypertension (high blood pressure) Behavioral health conditions such as schizophrenia and major depressive, bipolar and substance use disorders We also offer help with weight management and tobacco cessation We can help you find local resources, get referrals, improve your overall health and get past barriers to achieve your goals. Frequently asked questions about HPV What is HPV? HPV stands for human papillomavirus. This is a group of viruses that can cause genital warts, cervical cancer or other cancers. How does HPV spread? HPV is spread through sexual contact. HPV infections are very common. Many people don t know they have an HPV infection because many kinds of HPV don t cause any symptoms. This makes it very easy to spread HPV infections to others. How can I prevent HPV? The best way to prevent HPV infection is with a series of HPV vaccines. The best time for girls and boys to get the vaccines is as a preteen, at age 11 or 12, but they can be given up to age 26. Side effects are very mild, and studies have shown that the vaccines are safe. Ask your PMP if the HPV vaccine series is right for you or your child. We have case managers to help you understand and care for your health conditions. While your doctor helps you with your care, it s important you learn to care for yourself. During our welcome call, we ll find out if you or your child needs case management services and if you do, we will refer you to a case manager. Your case manager will work with you and your primary care provider (PMP) to set up a plan of care. Access to case management Our case managers may also call you if: Your PMP thinks you would benefit from the program. You re let out of the hospital and need some follow-up coordination of care. Referrals and prior authorizations. You call our 24/7 NurseLine and need more follow up for ongoing care. If you would benefit from Disease Management services. Reviewing your plan of care as needed. Here are some common vaccines for adults: How can you take pain medicine safely? Prescription pain medicines can reduce pain and improve your quality of life. But they need to be taken correctly to work and be safe. These tips can help: Vaccine Tetanus, diphtheria, and pertussis (Tdap) Influenza (Flu) Pneumococcal Zoster (Shingles) Recommendation Every 10 years for adults Each year for everyone over 6 months of age Two doses for adults ages 65 and older One dose for adults ages 60 and older Take medicines as prescribed. Tell your health care provider about your health history. Don t leave anything out. Tell your provider about any history of addiction. If you have more than one provider, pick one to be in charge of all your pain medicines. What options are there besides pain medicines?
