AccessOne provides complimentary services to make access to your infusion therapy simple and convenient. It s all about making access as easy as possible for you with just one call, one point of contact. One-to-one support If you and your doctor decide that REMICADE (infliximab) is right for you, your doctor can request that AccessOne determine if your treatment is covered by your health plan. A personally assigned Care Coordinator will quickly assess your insurance coverage, then call you and your doctor to explain: Your health insurance benefit. Potential out-of-pocket costs including your deductible, co-payment, and co-insurance. Your Care Coordinator can answer any questions about your health plan benefits and help you choose the services you d like to receive: A health plan overview to help you understand available health coverage options. Disease-specific education materials appropriate to your needs, questions, and concerns. Location of infusion sites, near your home or office. An informative newsletter to help keep you up-to-date. We can help you find ways to afford If you have commercial insurance RemiStart Patient Rebate Program You pay $5 per infusion * If you are eligible, RemiStart can provide a rebate for your out-of-pocket medication costs after you pay your full co-pay amount to your healthcare provider when you receive your treatment. RemiStart rebates are determined by medication cost only and not the cost associated with IV infusion. For additional details including complete eligibility and restrictions, please visit www.remistart.com. * Program exhausts after 12 months, or a $10,000 maximum annual benefit, whichever comes first. Once you exhaust your 12 months in the program, you may be able to re-enroll to continue receiving a benefit. Frequently asked questions Q: How does this explanation of benefits work? A: At your doctor s request, AccessOne will contact your health insurance company to determine your benefits eligibility. One of our Care Coordinators will then call both you and your doctor to explain your benefits and potential out-of-pocket expenses. Q: How often will my Care Coordinator contact me? A: Your Care Coordinator will only call you as often as needed for the services you have chosen. Q: Will my doctor coordinate my infusion care? A: Yes. Your AccessOne Care Coordinator will only provide the support services surrounding your infusion care that you have chosen. Q: Can you help me find a location for my treatment? A: If your doctor doesn t provide infusion service in his or her office, your Care Coordinator can work with your doctor to help locate an infusion site that s convenient to your home or office. Q: What if I don t want AccessOne to call me anymore? A: Just let us know, and we won t contact you any further. Q: What if I have other questions about REMICADE (infliximab)? A: If your Care Coordinator doesn t have the information you need, you will be referred to someone who does. However, if you have questions about your condition or treatment, you should always consult your doctor first.
