What is BPCI-A? Bundle Payment for Care Improvement Advanced is a Medicare initiative that seeks to improve the quality of care by utilizing financial and performance-based accountability The bundle encompasses all services (acute and post-acute) a patient receives from the date of admission through 90 days Applicable to Kaleida Health FFS inpatients with Dx of AMI, cardiac arrhythmia, cellulitis, hip and femur procedures (except major joint), percutaneous coronary intervention (inpatient and outpatient), renal failure, sepsis, simple pneumonia and UTI Hospitals, post-acute rehab facilities and physicians must execute efficient care coordination strategies to ensure patients receive the highest quality of care to avoid adverse events The key to SUCCESS is to increase awareness, communication and coordination of CARE for our patients

Who benefits from the bundled care initiative? Patients with Medicare part A and B Patients with a diagnosis from one of the Kaleida Health participating bundles: Acute Myocardial Infarction (MS-DRG 280 thru 282) Cardiac Arrhythmia (MS-DRG 308 thru 310) Cellulitis (MS-DRG 602 and 603) Hip and Femur Procedures except Major Joint (MS-DRG 480-482) Percutaneous Coronary Intervention Inpatient and Outpatient (MS-DRG 246-251 & 273-4) Renal Failure (MS-DRG 682-684) Sepsis (MS-DRG 870-872) Simple Pneumonia and Respiratory Infections (MS-DRG 177-179 & 193-195) Urinary Tract Infections (MS-DRG 689-90)

How do we manage our bundle patients effectively? Care managers identify potential BPCI-A patients daily to alert care providers in an effort to improve coordination and quality of care throughout the care continuum Care managers work with patients to help them manage their condition through education and follow-up coordination In an effort to provide a higher level of care, we work closely with our patients and their transition of care to post-acute care, preferred SAR providers and VNA homecare The care managers closely follow bundle patients after discharge and through the 90-day episode in an effort to avoid readmissions, decrease LOS and improve outcomes
