Code #: 4 The Crown Point High School Alumni Association will award two $500 scholarships to the members of the Class of 2015 who best exemplify the Portrait of a Graduate through the Six C s. Please read each of the Six C s carefully and write a clean paragraph to explain how you have in some way exemplified each. It is not necessary that you fill all space provided. The questions provided are intended to provoke your thinking and guide your essay; however, the goal is a clear paragraph, and any guiding questions that do not apply to you can be entirely disregarded without penalty. You may neatly handwrite on this packet in pen OR you may type your paragraphs on separate paper with the required signatures. Then, choose a supervisor or Crown Point High School staff member who will verify with his or her signature that the short essay is accurate. It is likely this will require six different signatures. (If you cannot comfortably state that you exemplify all six of the C s, it is still worth applying as long as you can defend that you exemplify at least four of them. It is possible in any given year that there will not be a candidate who meets all six strongly.) College: Value, pursue, and obtain post secondary education and training. What are your future academic plans? Where have you applied/been accepted to? How have you challenged yourself through your high school coursework to achieve this goal? Describe how you have been successful in your academic coursework.

5 Career: Experience, develop, and practice professional ethics, attitudes, and skills. Have you worked outside of school or been in a position of leadership in an organization or extracurricular activity? Have you taken courses to develop particular skills? How do you reflect professional ethics, attitudes, and skills? Citizenship: Advocate, initiate, and engage in self and community improvement. What have you done for your community service credits? How have you gone above and beyond this minimum expectation for our graduates?

6 Creativity: Imagine, construct, and communicate ideas, emotions, and solutions. Are you involved in the arts in any way, whether it be industrial arts, performing arts, or fine arts? Do you enjoy writing? Designing? How do you use creative problem solving abilities? Courage: Demonstrate character and leadership through words, decisions, and actions. How have you faced life s obstacles? When have you stepped out of the crowd to make decisions and/or improve situations?

7 Culture: Identify, appreciate, and respect the perspectives and expressions of diverse people, places, and times. How have you come to appreciate people from other cultures? How have you come to appreciate history? Do you have an awareness of the arts that other cultures have offered to the world? Optional additional statement: Use this space if you would like to, in any way, present a fuller picture of yourself or tie any of the content above together.
