However, these studies are neither exhaustive nor consistent. The need for studying nurse staffing is critical due to factors such as the nursing shortage, hospital restructuring, the growing concern for patient safety, and recent policies mandating nurse to patient ratios. The purpose of this study was to examine the effects of nurse staffing on patient satisfaction, patient adverse events, patient length of stay, and nurse job satisfaction. The study used a retrospective, correlational, longitudinal, and nonexperimental design. Data were collected using a secondary analysis method of existing databases of study hospitals for the years of 2002 through 2006. The sample included 39 units of five hospitals in one midwest state. Different unit types were investigated including medical, surgical, medical surgical, step-down, ICU, and ED units. The Kane et al. (2007) framework of nurse staffing was used to guide the current study. Variables controlled to isolate their effects on the outcomes of care included hospital, unit type, RN experience, and time of measurements. A multilevel model procedure was used to examine the relationships between the study variables. Increasing total nursing care hours per patient day was associated with decreasing levels of patient satisfaction with nursing care, patient satisfaction with overall care, patient satisfaction with pain, and patient pressure ulcer rates. However, an increase in RN hours per patient day was associated with an increase in patient satisfaction with nursing care, patient satisfaction with pain, and patient satisfaction with educational information; and a decrease in patient fall rates. A higher unlicensed staff mix was associated with a greater patient satisfaction with educational information and also a longer patient length of stay. The study reveals that the number of RNs is an important factor in improving quality of nursing care. In addition, replacing RN staff with other providers appears to decrease patient satisfaction and increase adverse events. Replication of this study with a larger sample of nursing units from a wider geographic area is recommended. 2
