Boschert) Pages: 5 Air Force Manual (AFMAN) 23-220, Reports of Survey for Air Force Property, is supplemented as follows. This manual establishes specific guidance and delegations of authorities for Luke Air Force Base regarding reports of survey. It applies to all personnel assigned to the 56th Fighter Wing to include Air Force Reserve Command and to Air National Guard units. Refer recommended changes and questions about this publication to the Office of Primary Responsibility (OPR) using the AF Form 847, Recommendation for Change of Publication, and route AF Forms 847 from the field through the appropriate functional s chain of command. Ensure that all records created as a result of processes prescribed in this publication are maintained in accordance with Air Force Manual (AFMAN) 33-363, Management of Records, and disposed of in accordance with the Air Force Records Information Management System (AFRIMS) Records Disposition Schedule (RDS). This publication may be supplemented at any level however, all direct supplements must be routed to the OPR of this publication for coordination prior to certification and approval. The authorities to waive wing and unit level requirements in this publication are identified with a Tier ( T-0, T-1, T-2, T-3 ) number following the compliance statement. See AFI 33-360, Publications and Forms Management, for a description of the authorities associated with the Tier numbers. The use of the name or mark of any specific manufacturer, commercial product, commodity, or service in this publication does not imply endorsement by the Air Force.

2 AFMAN23-220_LUKEAFBSUP 8 FEBRUARY 2017 SUMMARY OF CHANGES This interim change revises AFMAN 23-220_LUKEAFBSUP paragraph 9.3.6. (1) updated amount of the loss or damage from $2,000 to $5,000. (2) updated paragraph with the new AETC policy that items valued under $5,000 will not require a Report of Survey. A margin bar ( ) indicates newly revised material. 4. 1. 4.1. (Added) The responsible officer concurs or non-concurs with the recommendations of the investigating officer (IO) regarding financial liability. 4.1.4.2. (Added) The responsible officer will recommend closure or further processing of the DD Form 200, Financial Liability Investigation of Property Loss. 4.1.4.2.1. (Added) Recommending closure will stop processing of the DD Form 200 after the inventory adjustments are complete. 4.1.4.2.2. (Added) Recommending further processing will ensure the appointing and approving authorities review and make recommendations on the DD Form 200. 4.1.4.2.3. (Added) Further processing is required for classified items and weapons, regardless of value and when recommending financial liability. 4.1.4.2.4. (Added) Further processing of all other investigations will be at the discretion of the responsible officer. 4.1.5. When further processing is not recommended by the responsible officer, the 56th Fighter Wing Reports of Survey Program Manager (ROSPM) will close the ROS after the adjustment of accountable records by filing a memorandum in the case folder. Authority for the ROSPM to not process the ROS is delegated in paragraph 8.4.4. 8.2.2. The 56th Fighter Wing Commander (56 FW/CC) delegates the ROS approving authority to the 56th Mission Support Group Commander (56 MSG/CC). 8.2.3. The 56 FW/CC delegates the reviewing authority for any ROS that is initiated by Air Force ROTC units to the 56 MSG/CC. 8.3.5. The 56th Logistics Readiness Squadron Commander (56 LRS/CC) will perform a trend review annually of ROS data from units to ensure responsible organizations recommend appropriate processing of the DD Form 200. This data will be provided to 56 LRS/CC by the ROSPM in October and will include data for the previous fiscal year. 8.4.4. The 56 FW/CC delegates the authority to the ROSPM to approve not processing any ROS beyond the adjustment of accountable records, if the responsible officer recommends further processing is not appropriate or necessary. Recommendation for further processing will be included in block 12b. of DD Form 200 IAW paragraph 14.1.10. The intent of this delegation is to decrease the workload involved in processing unnecessary or indiscriminate ROS. 9.1. The approving authority (56 MSG/CC) designates the 56th Maintenance Group Commander, 56th Operations Group Commander, and the 56th Medical Group Commander as appointing authorities. The 56 MSG/CC will perform duties of appointing authority for

AFMAN23-220_LUKEAFBSUP 8 FEBRUARY 2017 3 any ROS initiated within 56 MSG. authority for tenant units. The 56 MSG/CC is designated as the appointing 9.3.6. Per AETC policy normal retail type items used at base level valued under $5,000 will not require a Report of Survey unless they are categorized as classified, sensitive, or pilferable, and there is no evidence of negligence, willful misconduct, or deliberate unauthorized use. 9.3.7. The approving authority (56 MSG/CC) authorizes appointing authorities to approve or disapprove DD Forms 200, when the gain or loss of inventory in Supply System Stock is less than $100,000. 10.2.2.5. The approving authority (56 MSG/CC) designates 56 LRS/LGLO as the 56 FW ROSPM. 56 FW ROSPM (56 LRS/LGLO) may delegate, in writing, the name of the individual(s) who will execute these responsibilities. 11.7. All delegations of authority set forth herein by the 56 FW/CC or the approving authority (56 MSG/CC) are made by position title as opposed to by name. The intent is to eliminate multiple updates due to personnel changes. Additionally, the delegations included in this supplement are directed by HQ AETC/A4RMP to ensure their permanence, that they are known to all concerned, and eliminate the need to include them in turn-over of files when personnel change. 11.7.1. (Added) The 56 FW ROSPM will: 11.7.1.1. (Added) Brief unit commanders on ROS responsibilities and assist as requested. 11.7.1.2. (Added) Brief the IO on ROS responsibilities and assist as needed. 11.7.1.3. (Added) Maintain an electronic log containing: 11.7.1.3.1. (Added) The ROS number. 11.7.1.3.2. (Added) Date the ROS number is issued. 11.7.1.3.3. (Added) Name and phone number of the organizational point of contact. 11.7.1.3.4. (Added) Type of form (DD Form 1131, Cash Collection Voucher, DD Form 362, Statement of Charges for Government Property Lost, Damaged, or Destroyed, DD Form 139, Pay Adjustment Authorization, DD Form 200) 11.7.1.3.5. (Added) Dollar value with (L) for loss or (D) for damage. 11.7.1.3.6. (Added) Description of item to include National Stock Number, Serial Number, or other identifying markings. 11.7.1.3.7. (Added) Name of person(s) mentioned on DD Form 200 as responsible for loss or damage. 11.7.1.3.8. (Added) Name of financial liability officer and date of appointment, if applicable. 11.7.1.3.9. (Added) Dollar amount of financial liability annotated (A) if assessed or (R) if relieved of liability.

(Added) Explain any unusual circumstances or provide pertinent comments. 11.7.1.4. (Added) In instances where the member is held financially liable for loss or damage and is projected for separation of service, expedite the ROS completion and the collection action upon notification from the IO. 11.7.2. (Added) The Department of Defense issued Common Access Card will be utilized to electronically sign all documents related to or generated in the performance of this program. This includes all prescribed and adopted forms that have electronic signature capability. 11.7.3. (Added) An early close-out memorandum will be completed by the 56 FW ROSPM when assets are recovered during the investigation or for clerical errors on accounts that do not require an adjustment. The original copy will be filed with the ROS package and a copy of the letter will be sent to the IO and responsible officer. 14.1.10. Block 12b. The responsible officer must provide a statement to recommend that further processing is or is not appropriate and/or necessary. 14.4.2.4. When individuals elect to make restitution, the 56 FW ROSPM will prepare a DD Form 1131, for cash payment, or DD Form 362, for payroll deduction. Once completed, the individual and unit commander will sign and return the DD Form 362 to the 56 FW ROSPM. BROOK J. LEONARD Brigadier General, USAF Commander
