Job Title: Reports to: Responsible for Location Swan Care and Support Job Description Awake Night Care and Support Assistant Care Services Assistant Manager/ Senior Care and Support Assistant N/A Essex Main Purpose of Job To provide personal, practical and emotional support to customers living in their own homes, proving personalised care and support that enables encouraging customers to reach and maintain an optimum level of independence. Principal Accountabilities 1. Provide personal care for the customer, assisting with personal care tasks where necessary, e.g. personal hygiene, continence management, dressing, assisting with medication as per MAR Charts. 2. To deliver care and support tasks as defined in the customer s assessment of need, care plan and risk assessment. In addition the completion on nutritional intake and continence monitoring charts. 3. Provide assistance with domestic tasks within the customers home, e.g. laundry, bed making, cleaning, shopping and meal preparation as directed. 3. Provide assistance, if specified in the care plan, with managing personal finances. 4. Provide assistance, if specified in the care plan, with medication, within your trained level of competency. 5. To ensure that all records are kept up-to-date to ensure compliance with CQC standards. 6. Ensure that all assistance is provided in a sensitive manner following consultation and consent from the customer and or their representative. 9. relevant monitoring documents as appropriate. 10. To report and record information, circumstances, changes or incidents concerning a client which give cause for concern. 11. Take account of and regularly review health and safety hazards and risks within the customer s home. 12. To abide by the highest standards of behavior at all times, in line with Swan Care and Support s Code of Conduct. 13. To participate in training as directed to ensure Swan Care and Support is delivering safe, effective and caring services. 14. To participate in regular supervisions in line with Swan Care and Support s policies and procedures. 15. To participate and contribute to mandatory staff team meetings. 16. To liaise with family members, friends and other professionals as appropriate. 17. To provide information, support, guidance and training to clients with respect to the skills needed to secure and maintain their independent accommodation.
