In order to provide the highest level of care, Faith Hospice employs boardcertified or board-eligible physicians in hospice and palliative medicine, as well as specialized nurses, social workers, chaplains and aides. Each experienced at delivering responsive, compassionate care as needed, from beginning to end. Everything we do is driven by our unwavering commitment to treating loved ones and their families with warmth and respect.

In-home Hospice Care The comfort of your home, the compassion of our care. That s what Faith Hospice promises and provides. Our board-certified or boardeligible physicians, nurses and aides are each trained and specialized in hospice care. Each delivering the medical care needed with love and compassion. Additionally, we re able to offer a number of care-related support services volunteers to provide respite for caregivers, aides to assist with personal care, chaplains to offer spiritual support and social workers to help with the practical and emotional needs. Trillium Woods Faith Hospice is one of the few hospice organizations in West Michigan to offer the option of a warm and welcoming hospice inpatient facility. Our medical staff specializes in hospice care, whether the need is a short-term stay that focuses on treatments so patients can return to their home or long-term stays. We even can provide respite stays to relieve caregivers. Whatever the need, Trillium Woods offers a peaceful, family-centered atmosphere with 24/7 medical care. Plus, each of our 20 private rooms feature convenient pullout sofas, private bathrooms and peaceful balconies or patios. Trillium Woods also offers a non-denominational chapel for worship, prayer or meditation. Bereavement Services Faith Hospice is dedicated to providing emotional and grief support to every member of your family for up to thirteen months after a loved one passes. Certified grief counselors provide individual counseling, as well as host bereavement support groups. And to help the youngest of hearts heal, our Camp Solid Rock Hospice Grief Camp is available for children 5 to 13 years of age.

Pastoral Services Because faith is the foundation of our efforts, we have a team of chaplains from various denominations ready to tend to the spiritual needs of loved ones and families. Chaplains visit with patients and family members wherever and whenever they are needed to provide comfort, help resolve spiritual issues and offer prayer and meditation. Palliative Care Not exclusive to hospice patients, our Palliative Care services provide relief and comfort to anyone suffering with the symptoms of a serious or chronic illness such as pain, shortness of breath, nausea, loss of appetite, fatigue, depression, insomnia, treatment side effects and more. Palliative care can be administered as needed by our team of certified and specialized palliative care physicians, nurses and social workers to help those affected cope physically and emotionally. We can also help guide caregivers and family members through the complex medical system to better understand care options available.
