A Healthy Community is our Business. At Kaiser Permanente, we talk a lot about total health. But what does this really mean?
It means teaching prevention and wellness in our community. It means providing safety nets for those in need. It means creating opportunities for higher education and sharing our knowledge and research. It means fostering diversity. It means playing a leadership role in health care reform, being an advocate for physical activity, and creating a cleaner environment. It means looking at the big picture. In all these ways, Kaiser Permanente truly stands for total health. With the rising tide of health care reform, we are more intently focused on providing high-quality care, expanding our reach to welcome new members, and embracing innovation to bring down the cost of medical care. Fortunately, these objectives are very familiar to us. The key to our success from the beginning is how our integrated model of high-quality, affordable health care leverages the power of medical and dental professionals working as a team using a sophisticated electronic health record. But equally important is our work in the community. We touch thousands of lives each year through a wide range of programs, partnerships, and assistance to help those in our community, including the most vulnerable. Our mission at Kaiser Permanente has been clear since 1945 to provide high-quality, affordable health care services and to improve the health of our members and the communities we serve. While the majority of our resources are focused squarely on providing medical care and health services, we also see the big picture when it comes to fostering wellness. We know that individual health outcomes are influenced by not only personal behaviors, but by social and environmental factors as well. This insight was recently echoed by the Robert Wood Johnson Foundation s Commission to Build a Healthier America, which provided important research linking the health of individuals to where they live. As a nonprofit organization, Kaiser Permanente makes carefully selected investments each year to address environmental and social factors that will improve the overall health of our communities. It is with this higher purpose in mind that we stand firm in our commitment to: Providing access to care for low-income individuals Promoting healthy eating and active living Developing and sharing research and knowledge Creating stronger safety nets through partnerships Encouraging employees to participate in activities for the civic good In 2012, we contributed a total of $91,710,949 toward building healthier communities in our region. This report highlights the many ways in which our work has touched lives in the Northwest over the course of the year. You may already be familiar with many of our large community initiatives, while other programs provide critical behind-the-scenes support that may not always generate the same awareness. Through our Care and Coverage programs, we help uninsured and underinsured people access medical and dental care with Kaiser Permanente providers. In 2012, we made some significant strides in expanding coverage to those in need with the following important programs: Health Share of Oregon In Oregon, the first collaborative care organization, Health Share of Oregon, brought together 10 independent health care providers including Kaiser Permanente to create a collaborative network of care for more than 180,000 Medicaid consumers. The success of this is rooted in some very familiar practices we ve been employing for more than 65 years at Kaiser Permanente. Primarily, by focusing on prevention and the drivers that contribute to an individual s total health mind, body, and spirit we can create healthier community members at lower costs. Charitable Coverage The Child Health Program is a fully subsidized insurance program for eligible low-income students. Currently, 397 schools participate within the Multnomah Education Service District, Salem-Keizer Public Schools, Hillsboro School District, Beaverton School District, and North Clackamas School District. The Transitions Program is a fully subsidized health insurance program for eligible lowincome families. Eight college campuses currently participate within the Portland metropolitan area. Access To Care Project Access NOW connects lowincome, uninsured community members with primary care providers. The Virginia Garcia High Risk OB Program provides prenatal care for nonmember high-risk obstetric patients by referral in collaboration with the Virginia Garcia Memorial Health Center. The KP Community Surgery Program matches low-income, uninsured patients who do not qualify for other public assistance programs with Kaiser Permanente hospitals, surgeons, physicians, nurses, and medical technicians. Community Investment Impact We contributed $22,423,092 to helping Oregon and Washington Medicaid patients through our services. Our dentists provided 50 days of treatment for 456 low-income children and adults for a total value of $219,762 in contributed care. 4,885 low-income children received full medical coverage through the Child Health and Transitions programs. Access to quality health care is essential for a strong community. Our Safety Net Partnerships help local clinics, health departments, and other community partners improve care and expand their reach. In 2013, our efforts in this area have focused on assisting our safety net partners to adapt and respond to health care reform through grant funding, technical assistance, and training for safety net leaders. Some highlights from 2012 include: Oral Health Initiative In 2012, we continued our support of a three-year initiative designed to increase access, support education, and improve policies that help break down barriers for people who need critical dental care. Clinical Quality Improvement Getting chronic illnesses under control, like diabetes and hypertension, is a key component of managing medical costs and improving health. In 2012, we launched a three-year Clinical Quality Improvement initiative to improve safety net providers ability to effectively treat chronic illnesses and address persistent health disparities. Behavioral Health Integration Total health includes both mind and body. In 2012, we began a funding initiative that provides support for mental and behavioral health through a joint partnership with Providence Health Plans and the Oregon Community Foundation. Transforming our Health System Creating health goes far beyond the health care delivery system. Kaiser Permanente is working with the Oregon Primary Care Association to develop new models, including the Health Instigator, to identify gaps in the safety net and address the barriers to health in our communities, with special emphasis on the social determinants of health. Community Investment Impact Our Clinical Quality Improvement in the Safety Net initiative provides $1.2 million in monetary support over three years. More than 6,000 patients are expected to benefit from the eight funded projects, which include the use of community health workers, improved data systems, and new models for team-based care. Nearly 20,000 patients were served by our 11 grantees in the Oral Health Initiative in the first year alone. Services included dental screenings, sealants, fluoride varnishes, and other critical dental procedures. We support health care providers and safety net leaders to participate in local and national learning opportunities through the Institute for Healthcare Improvement, Kaiser Permanente Clinical & Leadership Education, and the Care Management Institute. Total contribution: $2,483,226 I am so appreciative of the years of strong partnership. Kaiser Permanente s support will help Multnomah County implement a new clinic, opening in 2013, providing access to vital health care services to the Southeast Portland community. Our work in the community is focused on bringing about environmental and policy changes that make communities and neighborhoods healthier places to live, work, and play. Specifically, our goal is to ensure that healthy choices are available and accessible to all especially in our underserved, and often overlooked, neighborhoods. In 2012, we contributed to improving community health in the following ways: Healthy Food Access Initiative We supported a three-year initiative launched in 2010 supporting organizations that increase access to healthy foods. In 2012, we awarded eight grants to organizations that focused on encouraging local corner stores to offer healthier food options, supporting farm-to-school practices, and supporting community agriculture. Sunday Parkways Our grant to the city of Portland supports this series of five Sunday events that bring together an eclectic mix of bicyclists, walkers, runners, and families. The events build community interaction and cohesion, promote physical activity and nutrition, and create safer neighborhoods. Playworks This program creates opportunities for daily physical activity in low-income schools and educational and social environments where students learn how to interact with their peers in appropriate and respectful ways. Educational Theatre Program Kaiser Permanente is proud to support educational programs such as The Amazing Food Detective for K through third grades and Active Voices, a pilot program we started in high schools in 2012. Coalition Building and Partnerships We work with community partners to bring Kaiser Permanente s unique assets as an integrated health system to improve community health. Some of our partnerships are Cowlitz On The Move, Oregon Nutrition Policy Alliance, and Healthy Kids Learn Better Coalition. Community Investment Impact The Healthy Food Access Initiative invested $516,182 for the second year in a row to the eight grantees, helping facilitate initiatives that increase community access to healthy foods. As a primary supporter of Sunday Parkways, our major contribution helped open up city streets and brought together 100,000 Portlanders over five Sundays to build more cohesive communities. A $225,000 grant to Playworks allowed us to reach over 24,000 students at over 40 low-income schools to facilitate increased physical activity. The Educational Theatre Program reached over 25,700 students and 1,100 educators at over 131 schools with key messages about the importance of good nutrition and exercise. Total contribution: $1,240,182 Promoting Healthy Communities to Live, Work, and Play. We are a small nonprofit with the large mission of promoting active transportation in our community. Kaiser Permanente s partnership including grant funding, technical assistance, and most importantly devoted KP employee board members and advocates have been essential to fulfilling our mission and shared commitment to improving the health of the community. Kaiser Permanente understands the importance of supporting health care education, encouraging workforce diversity, and facilitating health research. We support these objectives through several initiatives, including the Health Care Career Scholarship Program, Continuing Medical Education Program, Graduate Medical Education Program, and funding to the Kaiser Permanente Center for Health Research. In 2012, we had a big impact with the following projects: Health Care Career Scholarship Program Kaiser Permanente Northwest has a long history of supporting health care education, workforce diversity, and community involvement. We ve partnered with the Oregon Area Health Education Center at OHSU to administer the Kaiser Permanente Health Care Career Scholarship Program. Continuing & Graduate Medical Education Programs Kaiser Permanente National Continuing Medical Education seeks to improve effectiveness of individual clinicians and physician leaders by providing educational opportunities designed to improve clinical, behavioral, and leadership skills. The Northwest Permanente Department of Graduate Medical Education serves as a major training resource for this region s future health care providers. Medical residents and fellows from eight institutions rotate within areas of Internal Medicine, Pediatrics, Family Medicine, and other key specialty areas. Center for Health Research Thanks to Community Benefit funding, senior investigator Gloria Coronado, PhD, was able to begin a study designed to increase colorectal cancer screening rates among Latinos and underserved populations. Funding also paved the way for senior investigator Nangel Lindberg, PhD, to conduct a survey around patient priorities among Latinos. Community Investment Impact We conducted 356 scientific studies in more than 23 subject areas, published 216 articles in scientific journals, and delivered 276 presentations to national and international audiences. We trained 848 residents, interns, fellows, nurses, and allied health professionals through our Graduate Medical Education Program. We awarded scholarships to 112 high school students and 24 college students. 2012 was the first year scholarships were extended to college students working toward a career in health care. Scholarships totaled $272,000 for the fourth consecutive year. Total contribution $9,048,060 Knowledge is power. And it must be shared. With the support of Kaiser Permanente and other scholarships I received, I was able to confirm my enrollment to the University of Portland. My college dream turned into reality. Our employees convert words into action by volunteering their time, professional services, and charitable contributions to create change in the communities we serve. Our efforts span a wide range from employee giving to staffing volunteer events around the community. Community Giving Campaign Each year, Kaiser Permanente Northwest employees help the region achieve its mission by donating to their favorite nonprofit organization. These nonprofits benefit the health and well-being of the communities we serve. KP Night Out A series of free clinics staffed by our physicians, dentists, nurses, pharmacists, and other clinical staff who offer access to health care for those in need. We kicked off this program with two major clinics last year and are building momentum to expand the program in 2013. KP Cares An online hub that makes it easier for all of our physicians, dentists, employees, and retirees to find meaningful volunteer opportunities in our communities. It is also a tool to support, manage, and track team volunteer activities, enabling those who want to lead efforts to recruit and recognize volunteers. KP Gives A volunteer grant program established to recognize the contributions that Kaiser Permanente employees make when they donate their time to charitable nonprofit organizations. The program makes grant funding of up to $15,000 available to qualified nonprofit organizations to which Kaiser Permanente employees and clinicians donate their time. Community Investment Impact Our Community Giving Campaign raised $932,673 in 2012 through employee gifts and a Kaiser Permanente matching gift. KP Night Out held two clinics in 2012, helping 74 medical patients and 13 dental patients in need of care. Kaiser Permanente staff contributed 160 hours of volunteer time and contributed $20,000 to the Clark County Food Bank to provide food to those in need. Our Surplus Property Donation Program distributed 35 in-kind donations to 23 organizations, totaling $63,282 in 2012. KP Gives provided grants totaling $387,600 in 2012. We supported 27 qualifying nonprofit organizations. Total contribution: $1,646,313 Serving patients at The Wallace Medical Concern as a physician and a board member teaches me how to be a better doctor and a better community advocate. It is a rewarding and humbling experience. Our workforce is a catalyst for change. The Northwest Health Foundation is an independent charitable foundation that administers grants on behalf of Kaiser Permanente. The Kaiser Permanente Community Fund at the Northwest Health Foundation supports organizations that address the social, policy, and physical factors that influence health, and health equity, in the communities we serve. In 2012, we provided support to our nonprofit and government partners in the areas of housing, workers rights, healthy food access, community design, and early childhood education. Community Investment Impact In 2012, we awarded $2.5 million in grants to 22 organizations. Since the fund was established in 2004, more than $22 million in grants have been awarded to 112 organizations. On May 21, 2012, we held our first grantee summit, gathering more than 250 of our funded partners who provided insightful and valuable ideas as we continue to build on our mission to create a healthier, more equitable region. Lifting Communities. The Northwest Health Foundation has been excited and gratified to partner with Kaiser Permanente Northwest to improve the health of our region over the past eight years. We ve constantly been impressed by the innovation and effectiveness of our funded partners, and we appreciate KP s willingness to invest with us in initiatives that are producing substantial benefits for the communities we serve.
