STANLEY V. AND MARY G. MAHALICK MEMORIAL SCHOLARSHIP Mahalick Memorial is providing scholarships for deserving students. The Center for Scholarship Administration, Inc. (CSA), a non-profit, independent organization is the administrator of the program. QUESTIONS AND ANSWERS ABOUT THE PROGRAM Q: Who is eligible for the scholarship? A: Applicants must be a high senior enrolled at Mahanoy Area High School or Marian Catholic High School. Applicants must reside in the Mahanoy Area School District at the time of graduation. Applicants must demonstrate good moral character. Applicants must have a minimum cumulative GPA of 2.5 through Fall 2010. Applicants must have a financial need. Q: When does the program start? A: The applications will be available December 6. Interested applicants may apply online www.csascholars.org or request a paper application by contacting CSA by mail at 4320 Wade Hampton Boulevard, Suite G, Taylors, SC 29687 by phone at (864) 268-3363; or by email at allisonlee@bellsouth.net. Q: When is the deadline for submission of materials? A: All materials must be submitted to CSA with a postmark no later than February 7. All materials must be mailed in ONE envelope. If you applied online and received a confirmation of submission you may send just the additional materials required. Q: What constitutes a complete application packet? A: 1. A completed application form, including the signed Terms of Agreement. 2. Official transcript with grades and cumulative GPA posted through Fall 2010. 3. Personal Statement regarding importance of scholarship, college and or career plans 4. Federal Tax Form 1040 (pages one and two only) and W-2 forms for both parents (if applicable) for the latest year these forms were filed. Q: What is the value of the scholarship award? A: The number of recipients and the amount of the scholarships will be based upon the market value of the scholarship fund assets each year as determined by the Trustee. Funds are to be applied to tuition, fees, books, supplies.
Q: What schools may I attend? A: You must be a full time student enrolled at any accredited, public or private, four-year College or university of their choice located in the United States. Q: What if I am unable to attend consecutive semesters? A: Extenuating circumstances requiring a student to sit out a semester must be explained in writing to CSA. You will be notified of the final decision. Q: How are winners selected? A: Awards Advisory Committee considers respective ability, educational goals and career ambitions (AND FINANCIAL IF REQUIRED). Q: How will I be notified if I am chosen as a recipient? A: All applicants will be notified of the decisions of the committee by mail. Q: Is this scholarship renewable? A: Scholarships may be renewed on an annual basis for up to three additional years (or until the bachelor s degree is obtained, whichever is earlier) provided they maintain their undergraduate status and a cumulative GPA of 2.5 on a scale of 4.0. A GPA falling below the required minimum at the end of ANY semester may result in permanent loss of the scholarship. If a scholarship is renewed for an academic year and any semester is not used within that year then it is forfeited. Recipients will receive information on the renewal procedures from CSA each year. Recipients must submit a renewal application and transcript or grade report in order to be considered for renewal of their scholarships each year. This scholarship may be renewable for up to three additional years of undergraduate work, or until a bachelor s degree is awarded, whichever is earlier. Current recipients will receive information on the renewal process from CSA. The scholarship will be renewed based on (a) availability of funds; (b) student remains in good standing with the institution; (c) student maintains the required minimum cumulative GPA; (d) student is enrolled full time, and (e) student applies by the deadline for renewal of the scholarship. Current recipients who fail to comply with requirements B, D and E may lose their scholarship permanently. If a scholarship is renewed for an academic year and any semester is not used within that year then it is forfeited. A GPA falling below the required minimum of 2.5 on a 4.0 scale at the end of ANY semester may result in a decision not to honor the award for the subsequent semester. If this is the decision then the award is terminated on a permanent basis and the student will not be allowed to apply for renewal at any time in the future. However, if a decision is made to honor the award and it is paid for the subsequent semester, then the student will be allowed to apply for consideration of renewal provided the GPA is at the required level at the end of the semester in question.

I authorize my school to provide the Scholarship Selection Committee with any and all requested information concerning my enrollment, grades, SAT/ACT scores or any combination of the above, and any other information deemed necessary by the Scholarship Selection Committee to enable them to make an informed decision regarding the selection of recipients. I give Wells Fargo Bank/CSA permission to list my name as a scholarship recipient on the Wachovia Bank/CSA website if I am chosen as a recipient. I understand that this is a competitive scholarship program. Scholarships shall be awarded on an objective and non-discriminatory basis, with neither race, creed, color, sex, age, religion, national origin nor disability being considered. Selections are based on information received from the application and the additional materials postmarked by the deadline. Decisions of the committee are final and justification for recipient selection(s) by the Scholarship Selection Committee, CSA, or Wachovia Bank staff will not be disclosed under any circumstances. I understand that if I do not submit all the required materials by the postmark deadline that I will not be considered for a scholarship.
