Our specialty hospital consists of 60 private rooms, including a 12-bed intensive care unit. Sagecrest focuses on producing optimal outcomes for critically ill and medically complex patients. All of our patients are given individualized treatment plans by an interdisciplinary medical team comprised of physicians, respiratory therapists, rehab therapists, and nurses who specialize in long-term acute care. We are dedicated to the provision of patient care at the highest levels with a focus of specialized clinical protocols which result in improved outcomes. 3

Sagecrest Hospital of Grapevine Our Programs CARDIOPULMONARY DISORDERS Conditions resulting in trauma, a severe medical illness, or post-operative complications- such as the development of an acute illness superimposed on an underlying disease, cardiac disorders, lung disease, and malnutrition. Many of these patients require prolonged respiratory support with an emphasis on weaning and recovery. NEUROLOGICAL / MUSCULOSKELETAL Treatment for brain and spinal cord injuries, degenerative disease, fractures, joint replacements, amputations, and multiple traumas. These disorders are usually complicated by medical instability or deconditioning following a lengthy illness or surgery, and can prevent the patient from meeting acute rehabilitation criteria. MEDICALLY COMPLEX Conditions having multiple system involvement requiring ongoing assessment and medical management. These include vascular disorders, unstable diabetes, hypertension, arrhythmias, pulmonary edema, renal insufficiency requiring dialysis, infectious processes requiring IV therapy, complicated post-surgical recoveries, and select cancer conditions. VENTILATOR MANAGEMENT & WEANING PROTOCOLS Our hospital specalizes in the treatment of medically complex ventilator-dependent patients. Utilizing cutting-edge technology alongside an early intervention & early mobility approach to patient care that was developed over a 20-year period, our weaning protocols have been proven to achieve ventilator weaning success rates in excess of 70%. 4

Sagecrest Hospital of Grapevine Visitor Information At Sagecrest, we understand the importance of family in the healing process. It is our goal to involve the family in our clinical team approach. A bedside conference is held within 72 hours following admission with the patient, their family, and our team. We encourage the utilization of our case managers, social workers, and clinicians for com-munication on current treatment plans as well as discharge planning. Pet visitations are strongly encouraged in authorized outside areas. Regular Visiting Hours: unrestricted ICU Visting Hours: Open visitation, 1 to 2 people can be in ICU at a time and 1 overnight visitor We provide free wireless internet to our patients & families. The visitation for children under 12 years of age may be prohibited during pandemic alert periods (such as flu season), as stipulated by the Center for Disease Control. 6
