Position details: POSITION DESCRIPTION Title: Outreach Mental Health Nurse Department: Mental Health Reports to: Clinical Team Leader - Assertive Community Outreach Service (ADHB) Location: Community Mental Health Service (joint position with Auckland City Mission) With operational coordination and support from: Team Leader Homeless Outreach and Support Service (Auckland City Mission) Date: Review date: Service Descriptors February 2018 February 2020 1. ACOS: The Assertive Community Outreach Service (Auckland District Health Board) provides a range of mental health assessment, treatment and support services to adult residents in the ADHB area (Auckland City). ACOS provides specialist assessment for individuals with an existing or suspected mental illness, with associated risk and/or impairment. The ACOS model or service delivery is engagement and recovery focussed, multi-disciplinary in approach and offers a broad range of evidence based support options and/or interventions. 2. HOSS: The Homeless Outreach and Support Service (Auckland City Mission) provides integrated services for people experiencing homelessness and rough sleeping. The HOSS team work alongside people experiencing multiple and complex needs in the community. Engagement with service users on collaborative goals, including but not limited to housing need, occurs via key working/case management model. Purpose of position: This position will be a partnership between Auckland City Mission and Auckland DHB Mental Health Services, to meet the needs of service users who are experiencing homelessness and comorbid health and social problems.. The role will provide specialist mental health nursing for service users of the HOSS team, linking directly with Auckland City Missions primary care clinic and ADHB health services (in particular ACOS community mental health team) to provide health care which is safe, appropriate and effective. This care will be based on comprehensive assessment and traumainformed practice, and will be service user centred, culturally sensitive and evidence-based. (Specific Annual Performance Objectives to meet these accountabilities will be agreed separately during the performance planning process) Key accountabilities Management of Nursing Care: Uses nursing knowledge and skills to assess, plan, implement, monitor and evaluate service users/ clients/ family / whanau health needs.

Provides direct nursing care for service users to achieve best health outcomes. Evaluates service user outcomes and reports variance to clinical pathway or best practice guidelines. Educates and provides information to service users/family/whanau to improve knowledge of disorder, self management and prevention of complications and promotion of recovery. Contributes to effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs. Accurately documents assessments of service users health status and response to nursing interventions and other treatments. Accurately documents all nursing contacts in HOSS notes and ACOS health records, as required. Applies diagnostic reasoning and professional judgement to nursing practice issues/ decisions. Uses warm handover and liaison to support HOSS & ACOS service users contact with other agencies. Role models culturally safe nursing practice. Fosters the provision of positive service users outcomes and person-centred care. Interpersonal Relationships: Collaborates and communicates with multidisciplinary teams (HOSS and ACOS) to co-ordinate care to achieve best health outcomes. Promotes and participates in effective teamwork and collaborative relationships within the multidisciplinary teams. Maintains and supports others to develop, effective therapeutic relationships with service users and family/whanau. Inter professional health care and quality improvement: Identifies situations of clinical risk and takes appropriate action to ensure a safe environment for service users, families/whanau and staff. Contributes to evidence based nursing practice in this specialty area. Demonstrates commitment to quality improvements, risk management and resource utilisation. Provides input into clinical standards / protocols and policies and undertakes clinical audits as required. Evaluates the effectiveness, efficiency and safety of clinical practice. Participates in the implementation of nursing models of care appropriate to patient/client population needs. Assists in the implementation of initiatives to address differential access to healthcare services for Maori. Acts to identify and minimise organisational risks. Follows ACM and ADHB Policy guidelines.

Contributes to and participates in ADHB and ACM policy development for this specialty area. Participates in case review and debriefing activities as required. Education & Qualifications General & Specific Requirements PERSON SPECIFICATION Essential RN; RPN; RCpN Registration with Nursing Council (NZ).
