Children s ITS notifies departments, units, and clinics when Cerner has scheduled or unscheduled downtime and instructs them to follow downtime procedures. When Cerner is back online, ITS notifies departments, units, and clinics that they can resume standard procedures. It is important to note that the downtime application is ONLY a patient location Tracking Board. No order notification, interdepartmental communication, or documentation will occur here. VERY IMPORTANT: Do not open or use the FirstNet Downtime Application before a downtime begins, and before you receive notification from ITS to do so. Logging into the FirstNet Downtime Application (Cerner is Down) 1 From any Star Net page, click on EMR Center under For Employees in the lefthand navigation. 2 Click the EMR Downtime Resources link. 3 Click Downtime Viewer under Open your downtime application. 4 Click on the Emergency Department folder, then click on your campus folder. For FirstNet Downtime Viewer, click ED-M or ED-S. Click MPLS EMR Last 30 Days or STP EMR Last 30 Days for patient visits within the last 30 days, when: o o The patient has an active encounter at Children s The previous records are at the same facility (hospital campus) as the current encounter. For example: If the patient is currently in St Paul ED and was discharged from the St Paul PICU last week, you can access the PICU data. But, if the patient is currently in the St Paul ED, but had a Gen Peds Mpls visit 2 weeks ago, you would not find the Gen Peds visit, since it is at a different Children s facility. 5 After clicking the appropriate folder to access Downtime Viewer, a loading sequence begins that may take 1 or 2 minutes. Do not click the icon again! 2
6 When the login screen displays, enter your Children s network login and password. Click OK to begin another loading sequence that may also take a few moments. Be patient! 7 In the 724 Access Downtime Viewer, click on FirstNet Search, then click the Tracking Group down arrow, and select your campus. The action buttons at the bottom left of the window allow you to add or update information on the screen: Add - add a new patient to the board. You can also search the last 30 days to find the patient. Edit - open the record you have highlighted to add or change information on the record. Patient Ed - access the patient education information. Note: The FirstNet Downtime Application contains the same Patient Ed information that is available in FirstNet. Depart - discharge the patient. Undo Depart - undo a discharge if done in error. 3
Tracking Board The Tracking Board shows the patients in the ED at downtime. Note: Pre-arrivals in FirstNet do not pull into the Downtime Tracking Board. Click any column header to sort the information. Columns cannot be hidden. Scroll to the right to view additional columns of information. Double-click on the patient name to open the chart. Click here for details about navigating the patient chart in the 724Access Viewer. To view comments on the Tracking Board, scroll to the right to view the Comments column. If a paper clip icon displayed, a comment is attached to the patient. Note: Only comments entered during downtime are viewable from Downtime Viewer. is 4
Add a Patient 1 To add a patient during downtime, click the Add button. 2 Select the Location from the dropdown list. 3 Patient First and Last Name, and Chief Complaint are free-text fields. 4 DOB, Sex, and Acuity fields display dropdown lists to choose from. 5 In the Providers section you can select each staff person who is working with the patient. Click on the appropriate button, then select the provider, and click Save. 6 Click on Add when finished. The patient is now displayed on the Tracking Board. The icon means the patient was added during downtime. 5
Edit Patient 1 To edit a patient already on the Tracking Board, first click on the patient you want to edit, then click the Edit button. The Edit window contains the same options as the Add window. Edit the information you need to, and click the Save button when finished. 6
Patient Education and Follow Up 1 To access the downtime Patient Education and Follow Up Instructions, first click on the patient you want to document instructions and follow-up care for, and then click the Patient Ed button. 2 The FirstNet Patient Education window will display and open to the Instructions tab. Note: The downtime Patient Education looks the same as it does in the FirstNet application. The Include Signature Line box is checked by default. 7
3 Click the Follow Up tab. The Follow Up tab is all free-text entry. Begin typing in the Provider/ Clinic window to open the Address, Follow Up By, and Comment windows. 4 Click the Print button to print a copy of the followup instructions. Note: Clicking on the Print button will create a take-home copy of instructions, and a signature page to keep with the paper record. Depart Patient 1 To depart a patient during downtime, first click on the patient you want to depart, and then click the Depart button. Note: Be sure there is a Reg done in the Comment field before discharging a patient. 8
2 The Depart window displays. The Patient Name and Depart Date/Time default to current date/time. The Discharge Disposition field is a dropdown list. The Discharge Diagnosis is a free text field. 3 Click Depart button when finished, to remove the patient from the tracking board. To display discharged patients, click to check the Include Departed Patients option. Discharged patients display with an asterisk next to their name. Printing the Downtime Report and Closing the Downtime Viewer When the ED receives notification that the downtime is over and they can resume regular operations, the Charge Nurse should print a report of the downtime application activity and the current Tracking Board before closing the Downtime Viewer application. 1 Click to display all current patients as well as those that have been discharged from the board. Note: All patients discharged from the last 8 hours will display on the report. 2 Click the Print List button. 9
3 In the Print List window, click the columns you want to print. 4 Click Finish. 5 The Print dialog displays. Click on the appropriate printer. Click Print. Note: Use the list to reconcile to the FirstNet Tracking Board. Once the downtime is over, you must close the downtime application so Children s IT can back up the downtime data. Click File, Exit or click the X in the upper right corner. 10
