Telephone: 01738 473469 Tay Ward is part of the Medicine for the Elderly Unit within the Perth and Kinross Health and Social Care Partnership and is accessed via Taymount Terrace at Perth Royal Infirmary. Tay ward provides rehabilitation for patients aged 65 years and over, working closely with many different health professionals that make up the multidisciplinary team to provide appropriate care and support. Discharge Planning begins on admission to Tay Ward. The team anticipates goals that need to be reached by working and discussing with you/your relative. The team has weekly case conferences to discuss progress and a target of a planned date of discharge is made and reviewed regularly. This is to ensure that all your needs are met for discharge to the most appropriate setting. Members of the Team Consultants: There are four consultants who rotate through Tay Ward. You/your relative will always have a main consultant but may be looked after by another consultant as they rotate. Ward consultants are: Dr Donna Clark Dr Dirk Habicht Dr John Harper Dr Neil Henderson They are responsible for the medical care of patients and are supported by junior doctors based on the ward. Consultants are present on the ward every day where they are updated on all patients and twice weekly they conduct ward rounds to review patients. Page 1 of 6
