1 Wellness Incentive Living a healthy lifestyle contributes to quality of life. That s why Mecklenburg County is committed to the health of its employees. Mecklenburg County has designed a wellness incentive, mytotalhealth Reward, to help you identify your health risks and achieve your personal health goals. The mytotalhealth Reward is a financial incentive (reduced wellness rate) that lowers your premiums, saving you $600 a year. You can earn the reward by completing two simple steps. 1 Take a Health Risk Assessment 2 Complete a Biometric Screening Review the information in this brochure to learn more about completing each wellness activity. To receive the mytotalhealth Reward, both wellness activities must be completed prior to September 15 th, 2017. Additional information will be posted periodically on MeckWeb. What about Health Coaching? You can take advantage of a new wellness feature designed to assist you in developing and maintaining healthy behaviors. Health Coaching is optional and not required to receive the wellness incentive. Visit page 4 to learn more about this new wellness feature. 2 STEP ONE: Health Risk Assessment The Health Risk Assessment is an online questionnaire about your health habits and history. Once you answer all of the questions you will receive a score and a personalized health summary that can help you understand your health needs. Understanding your health needs is the first step in maintaining a healthy lifestyle or making improvements in your health. Follow the steps below to begin your journey towards achieving your personal health goals. Take Your Health Risk Assessment If you ARE currently enrolled in the County s Medical Plan follow the instructions below: 1. Visit mycigna.com 2. Login with your username and password or select Register Now if this is your first time logging on as a Cigna Member and follow the prompts to create an account. 3. Select the My Health tab. 4. Select Take My Health Assessment or Get Started. 5. Complete the Health Risk Assessment until you receive a score. If you ARE NOT currently enrolled in the County s Medical Plan or you are a New Hire follow the instructions below: 1. Visit https://my.cigna.com/?token=3323264 2. Select Register Now and follow the prompts. 3. Select Take My Health Assessment. 4. Complete the Health Risk Assessment until you receive a score. If you have any problems creating or accessing your Cigna Account to complete the Health Risk Assessment, please call Cigna at 1.855.221.0273.
3 STEP TWO: Biometric Screenings A Biometric Screening is a measurement of physical characteristics such as height, weight, body mass index (BMI), waist circumference, blood pressure, blood cholesterol and glucose. Knowing your numbers is an important step in reducing your chances of developing heart disease, diabetes, and other major chronic conditions. The Biometric Screening serves as a benchmark and can help you evaluate any changes in your health status over time. Options for Completing the Biometric Screening You can obtain a Biometric Screening by going to one of the following: Primary Physician: Schedule an annual physical with your primary physician and have them complete the Wellness Screening Form provided on the last page. You can also have your physician complete the Wellness Screening Form using numbers from a previous physical you had during the 2017 calendar year. We will not accept numbers from a screening taken before January 1, 2017. Follow the instructions on the form for submission. Minute Clinic: Schedule a biometric screening at local Minute Clinic and have them complete the Wellness Screening Form provided on the last page. Follow the instructions on the form for submission. OurHealth MyClinics: Schedule a free appointment beginning April 17th at any of the five OurHealth MyClinic locations below and have them complete the Wellness Screening Form provided on the last page. Dates and locations are TBD. Please consider one of the options listed above to prevent ineligibility of the mytotalhealth Reward.
4 OPTIONAL: Health Coaching Health Coaching is a process designed to help you develop and maintain healthy behaviors. You will have the opportunity to speak with a trained health and wellness professional who is able to assist you in transforming your personal health goals into action. Having the support of a professional health coach can help you implement the changes you want to make to live a healthy lifestyle. What to Expect During Health Coaching During the health coaching call you will discuss your personal health and wellness goal(s) with a Health Coach. The Health Coach will schedule coaching session to check-in with you periodically to assess your progress towards your target goal. The number of health coaching sessions will vary for each person, as it is based upon your individual goal. To recap, your Health Coach will: 1. Assist you in setting a personal goal. 2. Follow-up with you periodically. 3. Help you make progress towards your goal. Getting Started You can engage in Health Coaching by completing one of the two options listed below: 1. Call the number provided below speak to a Cigna Health Coach to set your own personal health goal(s). 2. Telephonically participate in one of the following Lifestyle Management Programs by calling the number provided below: o Stress Management: The stress management program helps you understand the sources of your stress and learn coping techniques to manage stress both on and off the job. o Weight Management: This program helps you manage your weight using a non-diet approach. Get support to help build your confidence, become more active, eat healthier and change your habits. o Tobacco Cessation: The tobacco cessation program helps you get and stay tobacco free. 5 TRACKING YOUR WELLNESS ACTIVITIES How will you know that you have completed both wellness activities to receive the mytotalhealth Reward? MyCigna.com keeps a record of all your activities. You can track your completed wellness activities by logging into your account on mycigna.com and viewing in the My Incentive box. One point is awarded for each completed activity. Please allow up to two weeks for the website to update and credit you for the completed wellness activities. To receive the mytotalhealth Reward, both wellness activities must be completed prior to September 15 th, 2017.
