2 Procedure No.: PA-021FB-0002 Page: 2 of 6 1. PURPOSE The staging board is designed to account for BASF employees directly involved in emergency response (Fire Brigade Members). The staging officer, as assigned by the Incident Commander (IC), is responsible for making sure all BASF personnel are accounted for before Fire/Hazmat Entry Teams leave the staging area (main control room) and if they have already assumed a response position in the field. NOTE: The Process Chief (PC) is responsible for reporting to the Incident Commander the accountability status of all BASF process and maintenance personnel working on the unit and are not part of the assigned emergency response team. Contractors on the unit will give information to Security as per their accountability (head count) procedure. Security in turn will notify the Incident Commander 2. DEFINITIONS 3. SCOPE The staging board is a magnetic, dry erase board developed to follow the organization of a Level 1 incident. The board is accompanied by primary and secondary nametags issued to all fire brigade members. All members of the fire brigade should have both tags clipped to their turnout gear. The primary nametag is magnetic, and is used for initial staging in the main control room. The secondary tag will be used for staging in the field, (hot zone) if needed. The secondary tag needs to be clipped on the fire brigade member s fire fighting gear.

Primary Tag v Clip on the main-compartment zipper of turnout gear C. Secondary Tag v Clip on the snap hook of fire fighting pants 4.2 Staging Officer Duties for Personnel Accountability The Staging Office reports directly to the Operations Chief and assigns personnel into the field/emergency scene as per the Operations Chief. All Fire Brigade Members report to staging area to obtain assignments and dress in turnout gear unless otherwise notified by the IC Staging Officer should obtain briefing from Operations Chief. No one leaves Staging without knowing: q Personal Protective Equipment (PPE) Required q Name of person to meet in the field q Specific location to meet in the field Each Fire Brigade Member responding to the incident (as noted per the ER positions on Level 1 organization chart) will place their primary nametag under their designated responsibility on the staging board.

Staging Officer Duties for Personnel Accountability cont d Staging Officer is responsible for making sure all nametags are properly placed on the staging board The Staging Officer is responsible for supplying items such as Air Monitoring/Gas Detectors, Chemical Protective Suits, SCBA and or SCBA cylinders when any of the Control Room supply is used in an emergency. Staging Officer can request assistance from Maintenance and or other ERT personnel to get and or transport the requested equipment. NOTE: The Port Arthur Fire Department can assist with refilling SCBA cylinders and calling for additional equipment from local Mutual Aid. The Incident Commander with input from the Operations Chief will make the decision to call for assistance from the Port Arthur Fire Department Staging Officer should write in Location/Assignment for each Fire/Hazmat Entry Team dispatched in the field NOTE: The staging board is portable. Once staging is complete in the staging area, the staging board can be moved to the EOC in the administration building conference room.

Staging Officer Duties for Personnel Accountability cont d Staging Officer should act as a historian to the major events that occur in the field, i.e., Staging Activated Engine deployed to name location Engine flowing water from Deck Gun Attack Team # 1 entering Hot Zone Fire Out or Leak Stopped, or Valve Blocked Release Stopped Overhaul being performed to ensure no other problems exist ALL CLEAR Announced 5. RESPONSIBILITIES 5.1 The Staging Officer is responsible for establishing and managing an area for assembling of standby equipment and personnel before deployment to the incident scene. It is the responsibility of the EHS Specialist to train Emergency Response Team members on this document and ensures it is updated as revisions are made to the procedure. It is the responsibility of each Emergency Response Team member to read and understand this procedure prior to performing the task and to ask questions of the Shift Coordinator and or Training Coordinator. 6. 6 Attachment A Reports to: Operations Section Chief Staging Officer Checklist Check Report to Staging in the Control Room Turnout Gear storage area with a radio. Establish staging area layout and determine any requirements for support, equipment, and security. Obtain briefing for Operations Section Chief. Utilize Staging Board to document actions and conversations throughout response. Submit a summary of the board information to the Incident Commander upon termination of response. Establish appropriate check-in function for responders. Respond to requests for resources. Immediate resource requests from the Operations Section Chiefs should be relayed directly to the Logistic Group. Coordinate mutual aid and the staging and assignment of mutual aid resources to the Tactical Units. Ensure that only those resources (personnel and equipment) needed for tactical operations, are deployed to the field. Ensure support personnel who are dispatched to assist in emergency operations are briefed on the following: Proper PPE Name of person to meet in the field Specific location to meet in the field Maintain staging area in orderly condition. Request maintenance service for equipment at staging area, as needed.
