All hotel accommodations will be made during online registration. Registration Period: September 5 - October 20, 2017 Conference Registration Cost: $115/Arizona attendee Includes Thursday pre-conference workshops and Friday evening entertainment Saturday Afternoon Excursion: $15 Housing For 3-Nights*: Quad Rate $145 per person Single Rate $580 per person *Rates inclusive of all taxes and fees. All conference attendees that are staying overnight are required to stay at the official conference hotel and book housing throughout the WRLC 2017 registration portal. Daily commuting is permitted. 7 Excursion Options Another Phoenix must see, the Arizona Science Center is home to four floors filled with world-renowned exhibits in the areas of science, technology, and the world around us! This excursion includes access to all exhibits at the Science Center including the limited-time special exhibition, Pompeii. Explore one of Phoenix's hidden gems! The Phoenix Art Museum is the largest art museum in the southwestern US and is home to over 18,000 objects of modern and contemporary art. This excursion features a exclusive presentation by a museum executive on the business of art as well as full access to the museum including the limited-time special exhibition, Past/Future/Present: Contemporary Brazilian Art from the Museum of Modern Art, São Paulo. Transportation to and from the museum will be provided. Experience first-hand about all Arizona State University's WP Carey School of Business has to offer! Special presentations will be given by WP Carey's Assistant Dean and WP Carey Student Ambassadors on their experiences with ASU campus involvement, majors, and classes. A private tour of ASU's newly renovated Downtown Phoenix campus will follow the presentations., info@wrlc2017.org

8 Excursion Options The Musical Instrument Museum (MIM) provides a one-of-akind experience with more than 6,500 instruments on display from around the world. Located in Northern Phoenix, this worldrenowned museum opened just 7-years ago is one of Phoenix's most well kept secrets. This excursion will feature a special presentation from the MIM's Marketing Manager on museum operations and marketing a non-profit organization as well as full access to the museum. Transportation to and from the museum will be provided. *Please note this excursion is extremely limited in space, so register early! The Westgate Entertainment District is the place to be for students interested in Sports and Entertainment Marketing! This excursion features tours of the Gila River Arena and University of Phoenix Stadium as well as private presentations from front office staff for both the Arizona Coyotes and Arizona Cardinals! Attendees will also have the opportunity to spend time at the shops and restaurants of Westgate and the neighboring Tanger Outlets. Private transportation to and from the Westgate Entertainment District will be provided., info@wrlc2017.org

9 Code of Conduct For Delegates The following shall be regarded as severe violations of the Western Region Leadership Conference (WRLC) Code of Conduct: Should a conduct code violation occur for item 1 through 7 below, regardless of when exposed, the violating student(s) will be sent home and may not be eligible to attend any other state, regional, or national conference during that school year. If the violation warrants it, law enforcement may also be called. Determination of penalties for violations will be at the discretion of the Chapter Advisor, State/Association Director, and/or School District Officials. 1. Alcohol, Drugs and Tobacco: A student shall not possess, use, transmit, be under the influence of, or show evidence of having used an alcoholic beverage, other drugs, substances or tobacco products capable of or intended, purported, or presumed to be capable of altering a student's mood, perception, behavior or judgment; other than properly used, over-the counter pain relievers and medications prescribed by a physician for an individual student and must be on record with the advisor. Nor shall the student possess, use, sell or transmit paraphernalia associated with drugs, alcohol, or chemical substance in any form (including tobacco), at any time, or under any circumstances, on public or private properties. 2. Curfew: Committing serious violations of curfew regulations as outlined in item 9 below. 3. Willful Companionship: Being in the willful companionship of someone who violates any portion of the conduct code or failing to report any direct knowledge (other than hearsay) of the conduct code violations. 4. Personal Conduct: Cheating, dishonesty, or taking unfair advantage of others; participating in social activities other than those with conference participants; conducting acts and/or possession of weapons capable of causing bodily harm or fear of life, defacing or stealing any public or private property (for which financial responsibility will rest solely with offending individuals or their chapter); breaking the law; other serious violations of personal conduct regulations. 5. Serious Violations of the student conduct code of the school district or school that the student represents. 6. Private Transportation: Driving or riding in a private automobile during a conference, unless accompanied by an authorized advisor. (Delegates are to be housed at the conference site) Occasionally a chapter advisor, under special circumstances, may allow a student to drive or ride in a private automobile to a conference. These students are required to complete a "Permission to use Private Transportation" form to the chapter advisor prior to the conference. Permission to drive / ride applies to transportation of the student named on the form and only to and from the conference site. Once a driving/riding delegate has arrived at the conference site, s/he shall not be in a private automobile again until leaving the site at the end of the conference. 7. Abusive Behavior and Lewd Conduct: A student shall not engage in any lewd, indecent, sexual, or obscene act or expression or possess such materials. A student shall not engage in verbal, physical or sexual harassment, hazing, or name calling. The use of slurs against any person on the basis of race, color, creed, national origin, ancestry, age, sex, sexual orientation, or disability is prohibited., info@wrlc2017.org

10 Code of Conduct For Delegates The following shall be other violations of the Code of Conduct: Should a conduct code violation occur for items 8 12 below, regardless of when exposed, the violating student(s) may be sent home and may not be eligible to attend any other state, regional or national conference during that school year. Determination of penalties for violations will be at the discretion of the Chapter Advisor, State/Association Director, and/or School District Officials. 8. Conference Conduct: Failing to wear the supplied conference ID badge and wristband (when provided) at all times from arrival at the conference until departure at the end of the conference; leaving sessions prior to their conclusion (except in the case of emergency); failing to attend all general sessions and assigned activities (including workshops, competitive events, committee meetings, etc.) for which a delegate is registered (unless engaged in a specific assignment taking place at the same time); not abiding by the rules and regulations of DECA or school/district policies from the time he/she leaves his/her home or school for any activity and the time he/she returns to the same home or school following the activity. 9. Curfew: Failing to be in your assigned hotel room from the curfew time designated in the conference program until 6 a.m.; causing any noise or other disturbance audible by anyone in the hallway after designated curfew time; ordering any food after the designated curfew time; causing any other unnecessary disturbance or participating in any other inappropriate activity after the designated curfew time. 10. Dress: Failing to abide by the dress regulations established for the conference as outlined in the Dress Code 11. Personal Conduct: Failing to report accidents, injuries or illnesses immediately to the local DECA Advisor; failing to keep adult advisors informed of activities and whereabouts at all times; failure to comply with established grievance process for disputes about competitive event results/processes (including personally confronting judges or event administrators); using tobacco products outside of local school district policies and state law; having a member of the opposite sex in a room if no adult chaperone is present and the room door is not visibly open (e.g.. a garbage can may be used to adequately prop open a door); having a delegate or advisor of the opposite sex in a room without a third person present and the door visibly open. 12. Hotel Conduct: Failing to meet the professional standards of housing facilities; accruing incidental room charges (i.e., phone calls, room service, pay per view movies, etc.) without settling the account prior to check out; throwing objects out the window or into the hallway; moving hotel furniture from rooms (e.g., onto the balcony); failing to follow hotel rules and regulations. Individual School District and/or State DECA Policies may take precedence., info@wrlc2017.org

11 Conference Dress Code The following guidelines have been developed to clarify the dress code used at this year s WRLC: Business Professional Attire Thursday: Opening General Session Friday: All Workshops Saturday: All Workshops, Competitive Events, and Closing General Session Males: Suit including Dress Slacks and a Jacket or Sport Coat Tucked In, Collared, Button Down Dress Shirt Necktie or Bow-Tie Dress Belt Dress Socks Dress Shoes Females: Pants Suit OR Business Skirt (Knee Length Recommended) OR Dress (Knee Length Recommended) Blouse or Button Down Dress Shirt Dress Belt (if applicable) Dress Socks or Nylons Dress Shoes - Flats or Heels Are Acceptable Conservative Accessories DECA Casual Attire Friday: Evening Entertainment Session Saturday: Excursions Slacks, Khakis or Jeans Appropriate Length Shorts or Skirts Polo or Button Down Shirt DECA T-Shirts or Sweatshirts Clothing and Shoes Should Be School Dress Code Appropriate No gang related clothing or accessories allowed! All clothing must be in good repair and proper size. Undergarments may not show outside of over garments. *Activities such as DECAFit, swimming, sunbathing, jogging, etc. (should your chapter allow these activities during conference down-time) warrant appropriate, tasteful recreational attire for that activity., info@wrlc2017.org

I hereby give my permission for Student to attend the in Phoenix, Arizona November 16 to November 19, I hereby give the above-named advisor(s), the Chapter and/or State/Association DECA executive director, or his/her designee permission to authorize medical treatment for Student, as they deem reasonable or necessary, without additional permission from me. I agree to assume responsibility for all costs advanced by DECA in order to obtain medical care for Student. If the above named advisor or his/her designee questions Student s use of alcohol and/or drugs at a DECA sponsored activity, they have my permission to mandate that Student take a breathalyzer test to determine their blood alcohol level. In addition, they have my permission to search the Student s person, possessions, and room for alcohol and drugs. I consent to the use of Student s name and/or photographs in all DECA brochures, advertisements, web pages, and other publications approved by the Chapter DECA Advisors and/or State/Association DECA executive director or his/her designee. Student and I have read and agree to abide by the supplied Western Region Leadership Conference Conduct Code for Delegates. (Further clarification of the conduct code can be found in the Western Region Leadership Conference Registration Guide, which is available at Should a conduct code violation occur, law enforcement personnel and/or security may be called to assist, and a conduct code committee may be called to determine the appropriate consequences. Consequences may include: disqualification; being removed from office if in officer status; and/or the involved student(s) being sent home. Reasonable care shall be exercised to insure a safe, expedient, and financially feasible mode of transportation back to the home community of the student(s) involved. Should Student be sent home, I agree to be responsible for any costs related to his/her return trip home. In return for allowing Student to participate in DECA activities, I hereby agree to RELEASE, HOLD HARMLESS, and INDEMNIFY Arizona DECA, Arizona DECA Foundation, Western Region DECA, DECA Inc., White Tie Productions LLC and its employees, agents, contractors, officers, and affiliates from all claims for any injury or damages resulting from any cause, known or unknown, including negligence, which arises out of Student s travel to and from or during DECA-sponsored activities. I understand that I am also signing this Permission and Release on behalf of Student, a minor child, and I recognize that I may not release any claims Student may have. However, I accept full responsibility for all medical expenses incurred as a result of Student s participation in, or travel to and from, DECA events. I also agree to HOLD HARMLESS and INDEMNIFY Arizona DECA, Arizona DECA Foundation, Western Region DECA, DECA Inc., White Tie Productions LLC and its employees, agents, contractors, officers, and affiliates for any claims brought by or on behalf of Student. This Permission and Release is also binding as to any other persons, including family members, heirs, and executors, but does not apply to gross negligence or intentional acts. 14 STATEMENT OF ASSURANCE Conference: DECA 2017 Conference Dates: November 16-19, 2017 Conference Location: Phoenix, AZ Hyatt Regency Downtown/Phoenix Convention Center Each Chapter Advisor attending the must review the following statement of assurances, sign and date where indicated, and upload the completed document to the DLG registration system along with their chapter s online registration. Please see the Registration Guide for upload instructions. As the advisor responsible for the students of the chapter listed below, I confirm I have read and agree to the following: I have obtained the proper approvals and permissions to attend WRLC from my school s administration, district officials, and DECA association office as deemed necessary by the policies of those entities I will remain in compliance with all of my school district s policies for chaperoning students for the duration of the conference including travel and ensure that all chaperones under my supervision follow the same policies I will maintain a chaperone to student ratio of 1:15 at all times during the conference All advisors and chaperones supervising students at the conference are 21 years of age or older I will enforce the Conference Code of Conduct and Conference Dress Code as specified in the Registration Guide and Conference Program I will be accessible to my students in the case of emergency at all times during the conference including travel I understand that in order to participate in conference events attendees will be required to walk short distances between facilities in downtown Phoenix. I have discussed proper etiquette safety precautions in a busy, downtown metropolitan area with the students. I understand that the behavior of the students in my chapter is my sole responsibility as the advisor of the chapter and all disciplinary actions are to be handled by myself, school administration and/or by the DECA association office associated with the student WRLC Staff will immediately notify the signer of this document in the event of student behavior issues and expect an appropriate level of disciplinary action to be taken immediately in order to remedy the situation If I do not take sufficient remediation as deemed by the Conference Director in the event of behavioral issues with one of my students, I understand that the Conference Director reserves the right to involve the association director and local authorities if necessary. I have reviewed the Permission and Medical Release Form with my students and I will have a fully completed copy of the Permission and Medical Release Form for each student on my person for the duration of the conference including travel.
