1 Dear Friend of VNAHG: Each year at this time, we reach out to our friends in support of our Seasons of Hope program for the children and families of the patients we serve. This program brings home the true meaning of the season and the mission of VNAHG, to care for those in need. We are always amazed and inspired the benevolence of our volunteers, community partners, staff and generous supporters who bring hope and joy to our VNA families. Attached are the profiles of families who are currently under our care and identified our staff as in need of assistance to brighten their holiday. You can imagine the responses we see as we deliver your gifts the excitement and surprise in the face of a small child, the tears of joy from a senior living alone, the sigh of relief from a parent knowing their family will not go without. As you look through these profiles, we hope that we can once again count on your generosity during this season of giving. If you wish to adopt any of the families from this list, please contact the Seasons of Hope Adopt A Program at as soon as possible. Please do not respond . You will receive a confirmation call to confirm your adoption. Please: Do not shop until you receive confirmation. Gifts for your adopted family should be delivered from December 9 th though December 12 to the VNA Health Group, 176 Riverside Ave, Red Bank, NJ. Please do not deliver gifts before 12/9 or after 12/12. Delivery times are attached to this letter. Please note We ask that all gifts be delivered unwrapped, however you are welcome to use gift bags Each gift or gift bag should be identified with a label clearly indicating the number of the family, and the age, and gender of the gift recipient Each gift should be new Please do not include your identity as this program is anonymous. If you are not adopting a family but would like to participate, you may choose to make a donation providing a gift certificate from any one of the popular chain stores for the purchase of clothing or toys. Gift Certificates to food stores are greatly appreciated and would benefit many families. 2 Seasons of Hope Program 2016 Timelines for drop off of gift donations Gifts for your adopted family should be delivered to VNA Health Group, 176 Riverside Ave, Red Bank, NJ starting December 9 th no later than December 12 th Please do not deliver gifts before December 9 th o Friday, December 9 8:30 am till 6:00 pm o Saturday December am till 4-pm o Sunday December 11 2 pm till 4 pm o Monday December 12 8:30 am till 8:30pm Each gift should be new, unwrapped, or in a gift bag, with each gift or gift bag identified a label, which clearly indicates the family number you ve adopted, and the age and gender of the gift recipient. Please sign the Seasons of Hope log-in book in our front lob when you deliver gifts.
3 2016 Seasons of Hope Adopt-A- Program NA Health Group programs offer home visiting services during the critical early years of a child s life, to both first time parents and parents with one or more children. Healthy Families and Parents as Teachers serve families who live in Monmouth, Middlesex and Essex Counties. A trained Support Worker or Parent Educator meets weekly or bi-weekly with the family to provide support and education in their home. The visits are designed to improve family coping skills and understanding of infant behavior; foster parent child interaction; promote optimal child development and school readiness; and emphasize preventive health care. 7 2016 Seasons of Hope Adopt-A- Program NURSE FAMILY PARTNERSHIP PROGRAM The Nurse Partnership (NFP) program reaches out to assist low-income women in their homes during their first pregnancy and throughout the first two years of their children s lives. Specially trained nurses provide support and health education to accomplish three goals: improve pregnancy outcomes, improve child health and development, and improve families economic self-sufficiency. The VNA works collaboratively with the United Way of Central Jersey to offer NFP in Middlesex and Somerset Counties. The VNA also offers NFP in Monmouth and Ocean County. Holiday Needs When choosing from this list please be sure to identify child s age, gender and clothing size on your unwrapped packages along with the NFP #.
