1 CODE AED (AED Machine Housed in Secretary s Office Metal Cabinet) 1. Determine if student/adult is unresponsive or unconscious a. Yell for HELP b. Specifically tell a person to call office for assistance, state CODE AED with location of the situation i. Julie call the office and tell them we have a CODE AED in the cafeteria c. Office personnel will follow office procedures listed below 2. Employee finding the unresponsive/unconscious student/adult will determine if victim has any signs of life (moving, breathing, coughing, etc.). If NO SIGNS of life, immediately start chest compressions. 3. Secretary or COE worker will bring the AED machine to the location of the student or adult in need of assistance while the other person immediately calls Betina Robin, Hope Webre, Holly Hebert, and Sarah Henagan will report to Code AED Site and replace person doing chest compressions as soon as they hear the Code AED announcement. a. Barbara Mitchell will take Sarah Henagan and Hope Webre s students to her room b. Kate Gary will split Holly Hebert s class between all first grade classes 5. Julie Laviolette and Pam Ardoin will report to the Code AED site and assist as needed. Additional staff will be called if needed. 6. Once the AED machine arrives at the site, Betina Robin, Holly Hebert, Hope Webre, and Sarah Henagan will take over and provide life saving assistance until 911 arrives. 7. If on campus, Brandie Champagne will also report and assist as needed. Refer to Responsibility Chart on the Back of this Sheet

3 Crisis Plan for Playground Emergencies Playground Emergencies: K, 1, and 3 Recess Duty teacher will send a student to Ms. Phillis Castille s room (sun on door) Student will tell Ms. Castille to call the office to indicate there is an emergency on the playground. Once call is made Ms. Castille will ask Ms. Cross to monitor her homeroom so she may go out to the playground and assist with the emergency. Duty Teacher will blow the whistle and call Freeze so all other students on the playground may sit where they are so duty teacher may attend to the emergency. Playground Emergencies: 2 nd Grade Recess Duty teacher will send a student to Ms. Cathy Meche s room (rainbow on door) Student will tell Ms. Meche to call the office to indicate there is an emergency on the playground. Once call is made Ms. Meche will ask Ms. Latiolais to monitor her homeroom so she may go out to the playground and assist with the emergency. Duty Teacher will blow the whistle and call Freeze so all other students on the playground may sit where they are so duty teacher may attend to the Playground Emergencies: 4 th Grade Recess Duty teacher will send a student to office Student will tell office there is an emergency on 4 th grade playground. Once call is made COE worker or other office personnel will go out to help assist with the emergency. Duty Teacher will blow the whistle and call Freeze so all other students on the playground may sit where they are so duty teacher may attend to the emergency.

4 7/24/13 Updated Parks Primary School Fire Drill/Off Campus Evacuations Faculty and Staff Procedures 1. Recognize the fire alarm/off campus evacuation announcement for procedures to take effect. (Should you notice smoke or fire in any area of the school, notify the office immediately, and remove all students from the dangerous area.) 2. Remain calm and retrieve crisis folder and roll book to take with you. Custodial staff will turn off all power/gas to the affected building/buildings. Administrative staff will call 911 and notify central office personnel. (The secretary will also notify the church ( ) for use should it require an off campus evacuation.) 3. Designated personnel will check restrooms and hallways. 4. In the case of an on-campus fire drill, students are to be escorted in single file to the designated areas: Fourth Grade will report to the fence line along the front of the school and Mouton Street. The Literacy Lab, Library, Computer Lab will follow the sidewalk and report to the bus load area and stand along Mouton Street. (In an off campus evacuation, proceed to Main Hwy and then down to the church parking area.) PK/K will report to the fence line along the Main Hwy in front of the Literacy Lab and the PK/K building. (In an off campus evacuation, proceed through the side gate near Ms. Shauntel s room and walk down Main Hwy to the church parking area.) 2/3 will exit through outside doors and line up along the fence line on the side of the 2/3 building. (In an off campus evacuation, escort the children through the emergency gates next to the storage shed.) 1 st Grade Building will exit through rear door of building & line up along fence line walking alongside the 2/3 playground. (In an off campus evacuation, escort the children through the emergency gates next to the storage shed.) PKA, Tutors, Nurse, and Speech will use the ramp near the 1 st grade hall and line up along the fence on the side of the 2/3 playground. (In an off campus

5 evacuation, escort the children the emergency gates next to the storage shed. Cafeteria will exit through all four doors, converge to the 3rd slab and move toward the fence next to the 2/3 building. (In an off campus evacuation, move toward the emergency gate next to the storage shed.) OFF CAMPUS PROCEDURES: 1. In the case of an off campus emergency, follow fire evacuation and proceed to church. Once at church sit in designated pews. Do not release students and allow them to go to the checkout area. They will be called and be picked up by a designated person. 2. Advise students/others that a fire drill or off campus evacuation procedures are in effect. 3. Once at the designated area, take roll and remain in designated area until the all clear has been given. Do not release students to a parent, friend, neighbor, etc. in an off campus evacuation. 4. Students should remain quiet in order to hear further instructions. 5. Notify administrative or crisis staff of any students not accounted for after the all clear. In the case of an off campus evacuation, only authorized personnel in a designated location will release students through a check out system. Note: Should this occur while students are outside, all teachers and staff must report to student areas to secure their students and move to the designated areas and follow plan as described.

6 Parks Primary School Lock Down Lock and Hide (Threatening) Faculty and Staff Procedures 1. Recognize the announcement for lockdown procedures to go into effect. Note: Should you notice a threatening situation that will require a lockdown, the staff member should secure their area, notify the school of a lockdown by dialing 621 using the phone intercom feature, and notify the office of reason for lockdown. 2. Remain calm. 3. DO NOT open doors at all 4. Advise students/others that lockdown procedures are in effect. 5. Lock all doors and follow appropriate procedure below: ARRIVAL/DISMISSAL A. Whistle/siren for freeze and state Duck & Cover B. Duck and Cover under benches (head under bench) C. All remain silent D. No eye contact E. Adult goes down CLASSROOM A. Turn off the lights B. Lock doors DO NOT OPEN DOOR C. Close mini-blinds/curtains be sure to leave a small clearing at the bottom D. State Duck & Cover - Have students get on the floor, head down under desk/tables/out of sight and block openings by pulling chairs to them. All should be silent. E. Students should follow teacher directives without questions. F. Do not allow any student to leave the room at any time. G. After the all clear is given, tape red/green paper on or near entrance/exit door windows. OUTDOOR PLAYGROUND/TRANSITION A. Whistle/siren for freeze and state Play Opossum B. Drop flat to ground with hands on head C. All remain silent D. No eye contact E. Adult goes down CAFETERIA A. Turn off the lights B. Lock doors DO NOT OPEN DOOR C. Whistle/siren for freeze and state Duck & Cover D. Duck and Cover under tables (head toward middle of table) E. All remain silent F. No eye contact G. Adult goes down H. No one moves, leaves, or talks I. After the all clear is given, tape red/green paper on or near entrance/exit door windows.

Remain in lockdown until the all clear is given either by school administration or law enforcement. Remember to tape green(no critical injuries) or red (critical, emergency needs) to entrance/exit windows. 7. Keep roll book for accountability. Check roll and notify office of any students not accounted for.

Note: Should you notice a threatening situation that will require a lockdown, the staff member should secure their area, notify the school of a lockdown by dialing 621 using the phone intercom feature, and notify the office of reason for lockdown. 2. Remain calm. 3. Retrieve students/parents/staff from nearby areas including the restrooms, sidewalks, hallways, etc. Identify the person/people taken into your classroom. Notify their classroom teacher & the office to their whereabouts when able. 4. Day continues as scheduled following indoor recess and rainy day procedures with students supervised at all times. 5. Advise students/others that lockdown procedures are in effect. 6. Lock all doors and follow appropriate procedure below: ARRIVAL/DISMISSAL A. Whistle/siren for freeze and state Lock & Learn Rainy Day B. Car rider area proceeds into main building C. Bus area into cafeteria CLASSROOM A. Lights remain on. B. Check outside for students & Lock Doors C. Close mini-blinds/curtains be sure to leave a small clearing at the bottom D. Students can remain in desk and continue instruction. E. Minimize the amount of noise as much as possible. F. Be on alert for further announcements. G. Do not allow any student to leave the room unsupervised H. 15 RESPONSIBILITIES OF EACH CRT MEMBER PARKS PRIMARY Principal Julie Laviolette Assistant Principal Tassala Jones Pre-Planning Define crisis for your school Select Crisis Response Team (CRT) Develop appropriate procedures for handling crisis situations Train CRT Set up an emergency cabinet/drawer in office and in two other locations for CRT materials Have a list of telephone numbers and contact persons and copies placed in the emergency cabinet/drawer Each teacher and other support staff are given a copy of the School CRT Plan at a beginning of the year staff meeting New teachers are appraised and given a copy of the School CRT Plan immediately upon being employed Substitute teachers are appraised of the various crisis response and emergency procedures Designate a command post in the school (usually the office) where the principal or assistance can be located at all times and there is access to a phone, fax, radio, etc. Designate a secondary command post on

16 campus and a secondary command post off campus Monitoring of situation Liasion with police Coordination of activities from office Prepare for media inquiries (Refer to Human Resources Supervisor) Identify procedures for contacting parents Work with CRT to reassure students/employees Assign a written chain of command and give every employee a copy. Develop a plan of emergency coverage of classes for CRT Members who are called to a station during an emergency. They need to be assured that their classes will be covered. Use as many non-classroom people as possible. Ensure that first aid equipment/infectious disease supplies are available in the command center Develop a plan for student check-out Prepare a written statement for the secretary to read to all callers regarding a school crisis Prepare phone tree Prepare a chain of command in your absence Hold practice crisis alert drills Establish procedures for staff in-services Discuss memorial services, tree planting, ritual, etc.

Shelter in Place A. Lights remain on. B. Stay put. C. Students can remain in desk and continue instruction. D. Cover cracks on windows and doors with jackets, blankets, paper, etc. E. Turn off air conditioning systems. F. Prepare for evacuation. (Gather roll book and emergency folder.) DO NOT use cell phones and other electronic devices which may cause sparks.

24 Parks Primary School Weather Drill Faculty and Staff Procedures 1. Recognize the announcement for weather drill procedures to take effect. Note: Should you notice any threatening weather in the area, notify the office immediately, and remove all students from the school grounds and into enclosed school buildings as quickly as possible. 2. Remain indoors and calm. Retrieve crisis folder and roll book. Do not allow students to leave the buildings. 3. Designated personnel will check restrooms and hallways. 4. Close blinds. Classroom teachers in a main building will have students seated in the duck and cover position in the hallway and close classroom doors. Portable classroom teachers will move their students to the designated area and have their students seated in the duck and cover position. (Time Permitting) Move to 1 st /Support: PKA, Speech, Nurse, Tutors Note: If weather threat is immediate, students/staff will remain in the portables and take cover under desk/tables away from doorways and windows. 5. Advise students/others that weather drill procedures are in affect. Students should remain quiet in order to hear further instructions. 6. Once in hallway or at designated area, take roll and remain in position until the all clear has been given. 7. Notify office of any students not accounted for after the all clear.
