1 CHECKLIST FOR RENEWAL OF CERTIFICATE/LICENSE All forms are available online at Click on LPDC Forms. 1. Complete your IPDP (Individual Professional Development Plan). 2. Submit your IPDP to your building principal for his/her approval. (Elementary administrators should submit to their Pastors, and secondary administrators should submit to the Superintendent). 3. Submit your principal-approved IPDP to the LPDC for their approval. Mail to: Jeanne Gissel, HR Asst. for Teacher Personnel, 197 E. Gay St., Columbus, OH Accumulate the required college credit hours or CEUs for renewal of your license taken after the date of issue of the license and prior to the expiration date on the license to be renewed. OR Accumulate the required college credit hours or CEUs for renewal of your non-tax or permanent certificate taken after the approval date of the last verification form and prior to the expiration date on that verification form. 5. Submit Request for CEUs form (Page 4) to the LPDC along with verification of attendance at workshops, conventions, etc. If your plan has changed: Submit an IPDP Revision Form to your building principal. Submit your principal-approved IPDP Revision Form to the LPDC for approval. If your plan has not changed: 6. Submit approved IPDP and LPDC Verification Form for Certification/Licensure to your building principal for his/her approval. 7. Submit your principal-approved LPDC Verification Form, official transcripts and/or CEUs, and check to Office of Catholic Schools for $50. Gay St., Columbus, OH Apply for license renewal on the ODE website in your SAFE Account ( Our Diocesan LPDC IRN is ODE will require a credit card payment of $200. THINGS TO REMEMBER FOR RENEWAL OF CERTIFICATE/LICENSE To renew a five-year license, you will need to show evidence of 6 semester hours, 9 quarter hours, 18 CEUs or a combination of all three to equal 18 CEUs. To maintain a permanent or non-tax certificate as required by the Diocese of Columbus, you must show evidence of the same. You must have a new FBI background check every five years. If you have a permanent or non-tax certificate, it will become inactive with the ODE if you do not have a background check done when required. CEUs or credit hours must be completed before September 1 of the year your license or certificate expires. If work is not completed by September 1, an additional 3 semester hours or 4.5 quarter hours will be required. Additional CEUs will not be accepted for license renewal, only college coursework. Any teacher who does not have a current Ohio license or certificate in the field in which he or she teaches by September 1 will not be allowed in the classroom after that date. When submitting evidence of completed work to the LPDC, you must include all necessary information. See the Request to LPDC for CEU Credit form. Any college coursework must be accompanied by an official transcript (photocopies or grade reports will not be accepted). You must have your principal s signature if you are a teacher, your pastor s signature if you are an elementary principal, or the superintendent s signature if you are a secondary principal before submitting your IPDP to the LPDC. To receive credit for coursework to move to BA+18, BA+30, MA+18 or MA+30 on the pay scale only college coursework taken at an Ohio Department of Education approved institution may be counted. CEUs do not count toward this goal. Whenever you complete an activity for CEUs, send it in! Do not wait until the last minute or your license renewal, and therefore your contract, could be at stake. Applications for CEU credit must be approved before applying for Renewal. Renewal will not be signed at the same meeting as CEUs for each person. Columbus Diocese LPDC Forms Page 1 Revised 2015

DO NOT SEND UNAPPROVED CEUs WITH THIS FORM. CEUs MUST BE APPROVED PRIOR TO SUBMITTING THIS FORM. Verification as indicated below is attached. The LPDC will grant CEU credit for professional activities in the following areas. Please attach any items which verify your attendance, such as a badge, invoice, agenda, handouts, notes, pictures, certificates issued, etc. Attach Request to Diocesan LPDC for CEU Credit form to the top of the packet and turn in to your principal/pastor/superintendent for his/her signature. 11 How to Maintain Teaching Status in the Diocese of Columbus Includes Five Year Professional Licenses, Permanent Certificates, and Permanent Non Tax Certificates Please note: All certificate and licensure coursework must be completed by June 30 of the year the license/certificate expires. If coursework is completed after this date, all certificates must be renewed and all licenses must be issued by the first day of the school year, or the teacher may not begin teaching. Renewal fee for LPDC renewal: $50, payable to Office of Catholic Schools (this is separate/additional to the fee charged by the ODE). Five Year Professional, Permanent, and Permanent Non Tax (Except High School Religion Teachers) Must complete 180 contact hours in any combination of 6 semester, 9 quarter hours, or 18 CEUs. Coursework and CEUs must be related to classroom teaching, the subject area of current teaching assignment, and goals listed on approved IPDP. Time Frame: Must renew every five years. IPDP: Must have a copy of current IPDP on file in the Diocesan Personnel Office. Renewal Process: Send proof of coursework or CEUs along with signed verification form (page 4 of the LPDC forms), plus processing fee of $50 to the LPDC for review and approval. After review and approval, the LPDC sign off on the renewal verification form. ODE Renewal Application must be completed and paid for online. Superintendent or designee will electronically sign off. There is no need to send a copy of your renewed license to the Office of Catholic Schools. ODE will send this. A new IPDP should be developed and sent to the LPDC for approval as soon as possible after the renewed license is issued, preferably by the September LPDC meeting. Diocese of Columbus Office of Catholic Schools Policy states, Teachers who have not filed current Individual Professional Development as required by the State of Ohio by April 15 will receive non renewal letters. High School Religion Teachers Must complete 180 contact hours of which 90 hours (3 semester hours, 4.5 quarter hours or 9 CEUs) must be in the area of religious education and pre approved by the Office of Religious Education and Catechesis. The additional 90 contact hours may be in Theology or General Education coursework or CEUs. Time Frame: Must renew every five years. IPDP: Must have a copy of current IPDP on file in the Diocesan Personnel Office. Renewal Process: Send proof of coursework/ceus to the Office of Religious Education and Catechesis, Office of Religious Education and Catechesis will issue renewal certificate. Send renewal form along with signed verification form (page 4 of the LPDC forms), plus processing fee of $50 to the LPDC for review and approval. After review and approval, the LPDC sign off on the renewal verification form. There is no need to apply for renewal on the ODE website.
