This transcript will be used by the Scholarships and Awards Committee to review your application for UPEI awards; it is not necessary for you to send in an additional transcript for any awards on this form. SUBMISSION PROCESS Please check off all awards for which you meet the eligibility criteria, based on the award description, in the three sections below. You are only required to submit one Application Form, Personal Statement, Student Life Form and Financial Need Form for simultaneous consideration of all awards that you select. 1

Robertson Scholarship (Hunter) - Prince County Awarded to a student entering undergraduate studies for the first time from a Prince County high school Earl and Annie Duffy Memorial Scholarship Awarded to the student with the highest academic standing from Kinkora Regional High entering UPEI Engineers PEI Entrance Awards Awarded to two (2) deserving students entering the first year of the four year UPEI Sustainable Design Engineering degree program who have achieved high academic standing in secondary school H. Wade MacLauchlan Raised Expectation Awards and Medal Awarded to five (5) students graduating from a PEI public school who have demonstrated the greatest academic improvement from grade 10 to completion of grade 12 and directly entering UPEI in the fall John Tremblay Awards in Business Administration Awarded to a student who graduated from a PEI French-language high school; based on good academic standing, with preference given to students entering the Business Administration program at UPEI Lettie M. Passmore Scholarship Awarded to a first-year student entering UPEI with high academic standing in high school Marilyn Buist Entrance Scholarship in Nursing Awarded to a first-year student entering Nursing at UPEI, and is intending to enrol in a full course load Percy and Helen Murphy Memorial Scholarship Awarded to the student with the highest academic standing from Souris Regional High entering UPEI Prince of Wales College Scholarship Awarded to the student entering UPEI from a PEI high school who best exemplifies PWC's tradition of academic excellence; for the scholarship to be renewed, the student must maintain a minimum average in a full course load (30 semester hours) of 75% after first year, and 80% after second and third year 2

Ramsay Memorial Trust Scholarship Awarded to a student from Westisle Composite High entering either the first year of Nursing or Business Administration at UPEI with the highest academic standing from high school UPEI Faculty Association Scholarships Awarded to two (2) Island high school students entering UPEI, based on high academic standing LEADERSHIP AWARDS Audrey J. Tinney Nursing Scholarship Awarded to a student entering full-time, full course load studies in the Nursing program at UPEI who has been involved with volunteer and community work experiences; must be a resident of PEI for a minimum of five years, with mature students being eligible to apply Doris H. Anderson Leaders of Tomorrow Scholarship Awarded to a student entering UPEI directly from Morell Regional High School, who has proven leadership ability in their school and community; student must have a minimum average of 80% in Grade 12 and register in a full course load (10 courses) from September to May in their first year of studies Earl D. Foster Entrance Scholarship Awarded to a Colonel Gray student entering the Bachelor of Science Program; preference given to a Math or Chemistry major who is involved in the community, with an admission average between 75% and 85% Evelyn M. Cudmore Memorial Scholarship Awarded to a full-time UPEI student majoring in Child and Family Studies based on demonstrated volunteer leadership and experience in the possible areas of: Red Cross; water safety program instruction; youth camp counsellor, leader, or swim instructor; or other notable volunteer activity in outdoor education Frederick M. Davison Award for Excellence Awarded to a first year student who has high academic achievement, though not necessarily the highest aggregate in school, and best exhibits a dedication and a desire to make a difference in the community Halifax Friends of UPEI Entrance Scholarship Awarded to a full-time first-year Nova Scotia student entering UPEI, with preference for a student from the Halifax region; recipient will have an average of 85%+ and participate in community service activities Hart-Gorrill Entrance Award Awarded to an Island high school student of good character as demonstrated through extracurricular activity, who will register in a full course load (10 courses) from September to May in the Faculty of Arts Joseph Daly Leaders of Tomorrow Scholarships Awarded to students entering UPEI directly from a Canadian high school outside PEI, who have proven leadership ability in their school and community; students must have a minimum average of 80% in Grade 12 and register in a full course load (10 courses) from September to May in their first year of studies 3

LeBlanc Alumni Award Awarded to a PEI student entering first year who has a strong academic record and financial need Ed and Pat Lawlor Award in Mathematical and Computational Sciences Awarded to two (2) students entering first year in the School of Mathematical and Computational Sciences Eugenie Ann Pineau Memorial Awards Awarded to two (2) students entering the Transition Program at UPEI, based on financial need John and Isabel Copan Entrance Scholarship Awarded to a full-time first-year student entering UPEI from Nova Scotia with preference for a student from Pictou County, who is in financial need and has a good high school academic record John S. Macdonald Memorial Entrance Scholarship Awarded to a student from Montague Regional High School with an 80%+ average in Grade11, who displays an interest and enthusiasm for life-long learning and commitment to their community Mary C. McInnis Entrance Scholarship in Nursing Awarded to an Island student entering the School of Nursing with strong academics and financial need Rotary Club of Montague Scholarship Awarded to a student entering an undergraduate program for the first time who is normally a resident in Eastern Prince Edward Island and demonstrates financial need; scholarship may be renewable for three subsequent years provided the student maintains 70% or greater in the previous academic year 4

There are 36 weeks of study in the September to April academic year for which students can claim expenses.
