1 Region 2 DeKalb County Council of Parent Teacher Association Scholarship PACKET Greetings: PTSA Presidents, Principals, and Counselors From: Region 2 DeKalb County Council of Parent Teacher Association Scholarship Purpose: The R2DC scholarship has been created to reward PTSA card carrying high school senior scholars with support for college expenses. We will award one qualifying scholar from each of our Region 2 DeKalb Council of PTAs High Schools. (Arabia Mountain HS, Clarkston HS, DECA, DeKalb School of the Arts, Lithonia HS, Martin Luther King HS, Miller Grove HS, Redan HS, Stephenson HS, Stone Mountain HS, Southwest DeKalb HS and Tucker HS) To ensure we have all the information needed for evaluation, please complete this application in full and follow the instructions. The essay, application form and all required documents must be received via hand delivery, mail or courier, on or before March 16, No Exceptions! ATTN: Region 2 DeKalb County Council of PTAs, 1701 Mountain Industrial Blvd. Stone Mountain, Georgia Hand delivered applications will be received on March 16 th, 2018 between the hours of 2:00pm and 5:00pm. Please Note: The amount of the scholarship will be determined after all donations are received. The guidelines are listed below. This form must be filled out by the scholar, and signed by the scholar, parent(s) or guardian, and submitted.

2 Criteria: PTSA Presidents will ensure the senior scholar is a paid PTSA member, on file no later than February 21, 2018 The student must not have any disciplinary infractions during the 11th and 12th grades; and must provide letter on school letterhead. Must have a minimum GPA of No Exceptions! The student must be college bound and provide proof of enrollment upon receiving R2DC scholarship. Must submit a copy of his/her PTSA membership card with the application. The student s high school PTSA must to be in good standing with the Region 2 DeKalb County Council of PTAs and Georgia PTA. Contribute to the Region 2 DeKalb Council of PTAs Scholarship Fund no later than February 21, 2018 Audit Report Filed and submitted to Georgia PTA and R2DCC (tye@georgiapta.org and r2tres2dekalbcouncil@gmail.com Filed and submitted IRS 990 (tye@georgiapta.org and r2tres2dekalbcouncil@gmail.com ) Sent paid membership to Georgia PTA Paid Region 2 Dekalb County Council Dues Three (3) Letter(s) of RECOMMENDATION on letterhead - One (1) letter must be from a teacher No Exceptions! Two (2) recommendations can be from a principal, counselor, advisor, coach, or mentor. All letters of recommendation must be in a signed sealed envelope and submitted with the application. A copy of the student s OFFICIAL TRANSCRIPT must be submitted with the application, in addition to a copy SAT or ACT TEST SCORES. Applications received without this information will be disqualified. Each student is asked to submit a copy of his/her Senior Picture for posting. If you are the recipient of the Bill Gates Millennium Scholarship, this scholarship will be forfeited and given to the runner up from your school. This means membership, Council dues, list of officers, current year Audit Report must be on file with Georgia PTA; as well as Form 990 filed with the IRS, by February 28, 2018 according to Georgia PTA No Exceptions!!!

3 Interviewing Process Disclosure: Scholar s essays will be reviewed by an impartial team who will select two (2) candidate(s) per school to be interviewed. The interviews will take place at the School System Administrative Instructional Center (AIC), 1701 Mountain Industrial Blvd., Stone Mountain, GA Failure to appear will result in disqualification of the applicant. Illness and emergency are excusable. Important Note - Since our prospective awardees are on their way to college or technical school, it is expected that all essays contain proper grammar, punctuation and clarity of content or points will be deducted by panel judges. Four (4) Questions Essay / Instructions: All essays must be submitted on a separate sheet of paper, and contain detailed responses to all of the following questions. Essays must be typed (12 font) double spaced. Each question is valued at 25 points and may not exceed 300 words each. Failure to comply with the above instructions will result in points being deducted. 1. Why volunteer? Explain your volunteering experience(s). 2. How did you become interested in your major? 3. How do you plan to use your studies to achieve your future career plans? 4. How would being awarded this scholarship help you achieve your educational goal(s)?
