1 OBJECTIVE: To provide a $ 4000 scholarship (paid $1000 /year over four years) to a deserving high school senior from Narragansett, South Kingstown and Prout High schools who is a resident of South Kingstown or Narragansett. The recipient should reflect the Rotary motto of Service Above Self. The annual award is to be used for the recipient s continuing education in a program at a college or university. CRITERIA: The student to be selected for the scholarship will be judged on the characteristics of service to others, leadership, scholarship and need. The following will be considered in the determination of the award winner: 1. VOLUNTEERING: The quality of providing service to others by giving your time to organizations or projects which serve to enhance the welfare of others is an important factor. Some examples of service may be work in the community involved with elderly, scouting, church, or youth groups. 2. LEADERSHIP: Leadership is a characteristic of the Rotary ideal. You should not only be a participant in the activities, but be one who puts in the extra effort of assuming responsibilities, motivating others, and helping to lead in the achievement of the project or organization goals. Participation in school activities such as, school newspaper, school tutor program, cheerleader, school sports program, student government, etc. will be considered. 3. SCHOLARSHIP: Scholarship is important, but not an overriding criterion. Essentially, this may be important when deciding between two deserving students. The student should have solid academic skills and preferably be in the top one third of the class. 4. NEED: As with scholarship, this may be a deciding criterion. Naturally, we would like the award to go to the student who exhibits the greatest need. Other than the usual needs of tuition, books, fees, and tools or equipment, are there any other need such as family health concerns, single parentage, other siblings in school that we should be aware?

2 GUIDELINES 1. The Scholarship Award is up to $4,000 (paid $1000 /year over four years). The scholarship award is renewable based upon the recipient s annual request and submission of satisfactory academic progress via official transcripts. 2. The Award is granted to a graduating senior from Narragansett, South Kingstown and Prout High Schools AND is a resident of Narragansett or South Kingstown. 3. The Student must be attending post high school studies at a two or four year college or university. 4. The Award is based on criteria of service, leadership, scholarship and need. 5. Completed applications are to be returned to respective Guidance Counselors for processing. Students may download scholarship information and application through a link on the Rotary Club of Wakefield website Completed applications not submitted through respective guidance offices will not be considered. 6. Please note: Applications that contain information identifying the student or exceed page requirements WILL NOT BE CONSIDERED 7. For the Guidance Office: The cover page with the name of the student is to be removed and each application is to be numbered and indexed by the school Guidance Office. The applications together with a sealed envelope with a numbered index with corresponding student applicant names are then to be returned to the scholarship committee. 8. The Wakefield Rotary Club will notify the school of the selected scholarship winner within four weeks of receipt of all scholarship applications. The Scholarship award will be made by a member of the Rotary scholarship committee who will present the scholarship along with a certificate commemorating the award. 9. Annually, the winning student must contact the Scholarship Chair of the Service Above Self award committee by June 1st to renew the Scholarship. The recipient, (not their parent or guardian), must submit their official school transcript at the end of each academic year to the Scholarship Committee. Failure to contact the committee or chair will result in loss of the award for that year. Awards cannot go beyond 4 years or be made up if the student has failed to contact the Rotary Club of Wakefield in a given year. 10. Scholarship winners and renewing Rotary Scholarship Award recipients will be asked to make a short speech about their future educational plans and activities and report on their academic and career progress to a luncheon meeting of the Rotary Club of Wakefield annually.

3 Scholarship Application Cover Page Process: 1. Please complete this application cover page and return it to your school guidance office with the completed Wakefield Rotary Club Service Above Self Scholarship Application. 2. Please list your name on this cover page only. For our committee to make an unbiased decision, it is imperative that your name not appear anywhere other than the cover page. 3. Application should be type written, not less than a 10 font and not to exceed two (2) pages. 4. Do not use back page of paper. 5. Failure to follow guidelines may lead to disqualification. 6. Please note: Any Scholarship Application that contains identifying personal information in the body of the application will be disqualified. Timeline: 1. February 2017 Scholarship Application documents released 2. Thursday April 12 th Completed applications due to Rotary Scholarship Selection Committee 3. Thursday May 10 th Rotary Scholarship Committee informs school of Scholarship winners The following for school use only:

What are your future career and education plans? What studies do you intend to pursue in college? Why? 4. Describe how this scholarship would assist you in meeting your goals. How will you use this scholarship? List any special financial needs you have in addition to the obvious expenses of tuition, books, etc.
