3 Mountain Creek SHS P&C Association is offering 10 scholarships to students attending Mountain Creek SHS as year 7 students in The funding of the 10 scholarships is to reward outstanding young students who have succeeded academically, are good role models to their peers, and also are strong in one or more areas of sport, art, dance, drama, music, public speaking, debating and information technology. Students applying for the scholarship must: 1. Complete the attached application form. 2. Return the completed application form to: The Head of Junior Secondary Mountain Creek State High School P.O. Box 827 Mooloolaba Qld Closing Date: Friday 14 July Winners of the scholarships will be announced in Term 4. Recipients of the scholarships will be notified in writing as will unsuccessful applicants. Mrs Cheryl McMahon (Executive Principal), in conjunction with the P&C Association, will present the winners with their scholarships during a family welcome afternoon in Term Recipients of the scholarships will be advertised in MCSHS publications as well as the local community. 6. Successful applicants of the scholarship will be required to sign an acceptance form acknowledging the conditions of the offer. 7. Scholarship winners will not incur costs (included in the following 4 areas) associated with completing year 7 in 2018 at Mountain Creek SHS. i) The parent contribution to the Student Resource Scheme ($375): Text book hire for the year Classroom resources other than texts Year 7 adventure days Excursions, including bus costs and entry fees Student Planner ii) IT Levy - $80 Scholarship Application 1

5 CRITERIA Academic Achievement Conduct Leadership Programs of Excellence Confirmation by your parents and Principals EXPLANATION You must include in your application a copy of your most recent report, signed by the Principal of your school. Your conduct in the classroom and playground must be excellent at all times. If your most recent report does not indicate your conduct, you need a letter from your classroom teacher indicating your level of conduct, countersigned by the Principal. You must demonstrate that you are a positive role model to your peers. Please outline your leadership role at your school eg. School captain, captain of a sports team, leadership positions within your classroom, participation in school and community activities (fund raising, red cross door knock appeal etc). Mountain Creek State High School has several programs of excellence: Academic Sport Public Speaking and Debating Information Technology Creative Industries (Dance, Art, Drama & Music) STEM Science, Technology, Engineering and Maths As academic achievement is addressed in criteria 1, you do not have to address it again. You are to outline the skills you have which contribute to the school s areas of excellence. You may not have high levels of skills in all of the areas. The application must be approved (counter-signed) by your parents/guardian and the Principal at your school. DOCUMENTARY EVIDENCE All applications must include documentary evidence. It is the responsibility of the applicant to ensure that appropriate documentation is provided. Documents, resumes and other attachments will NOT be returned once the successful applicants are announced.
