3 Please include required letters of Recommendation with your application. References must not be a relative. This application must be accompanied by an Essay of approximately 250 words on Why You Chose Nursing as a Profession, and Why You Need this Scholarship. Include a passport size photo of yourself. High School Students must include a Transcript with class rank, GPA, and SAT or ACT scores. A letter of acceptance to a college or university as well as acceptance into a nursing program is required. For entering freshmen, acceptance into a Pre- Nursing Program will meet this requirement. (See Requirements) College students must include a transcript of completed college work and work in progress. (See Requirements) A letter of endorsement from the Dean of Nursing or equivalent official is required for Continuing Education Scholarship applicants. (See Requirements) The completed application with all required attachments, letters, etc, must be received in the Volunteer Office of Lexington Medical Center by MARCH 15, In fairness to students who apply according to the requirements, LATE APPLICATIONS CANNOT BE CONSIDERED, and INCOMPLETE APPLICATIONS WILL BE DISCARDED. Preference given to prior Scholarship winners. *Qualified Home School Seniors who reside in Lexington County may apply. Seniors who reside in Lexington County and attend Private Schools, Charter Schools, or Virtual Schools may apply.

5 AUXILIARY NURSING SCHOLARSHIPS Auxiliary Nursing Scholarships Lexington Medical Center Auxiliary is accepting applications for those receiving their nursing education at a variety of levels. How to Apply Applications are available Online (see below), and at the Volunteer Office at Lexington Medical Center, High School Guidance Offices, and through Health Occupation teachers, as well as on line. Deadline Applications are due on March 15, 2018, completed (incomplete applications will be discarded) Requirements and Who Should Apply Medhurst/Norton/Telfer Scholarships $ each Awarded to high school seniors who reside in Lexington County or attend a Lexington County School, and also plan to earn a Baccalaureate Degree in Nursing. High school senior who has been accepted into a Baccalaureate Degree Program in Nursing (letter of acceptance into Nursing required) Completed Application Two Letters of Recommendation High School Transcript with GPA, class rank, and SAT or ACT Scores Ann Bethea Scholarships $ each Awarded to high school seniors who reside in Lexington County or attend a Lexington County School*, and also plan to earn an Associate Degree in Nursing. High school senior who has been accepted into an Associate Degree Program in Nursing (letter of acceptance into Nursing required) Completed Application Two Letters of Recommendation High school transcript with GPA, class rank, and SAT or ACT Scores

Sandra Dodd Continuing Education Scholarships Amounts vary Awarded to students pursuing Baccalaureate degrees in Nursing. Prior scholarship winners are encouraged to apply. College student with sophomore or higher standing who has gained or will gain, full acceptance into a Baccalaureate Degree in Nursing Program Letter of endorsement from the Dean or Other Official of the Nursing Program One additional letter of recommendation College transcript with GPA Cook/Price Mature Student Scholarships $ Each Awarded to mature nursing students in two-year (2) or four-year (4) programs. To be eligible applicants must have been accepted into a Nursing Program Acceptance to an Associate or Baccalaureate or Pre-Nursing Program in Nursing (a letter of acceptance required) Complete application Two letters of recommendation Transcript of prior academic work with GPA (if available) Expected date of graduation Auxiliary Volunteers Memorial Scholarship $ One Scholarship will be awarded each year. No separate application is needed. The Scholarship Committee will select the recipient from the applications to the different scholarships. Important Information About Applying Incomplete or Late Applications Incomplete applications will be discarded Late applications cannot be considered for scholarships

7 Eligibility Only students planning a Degree leading to becoming a Registered Nurse are eligible. Preference will be given to prior scholarship winners for the Sandra Dodd Continuing Education Scholarships and employees of Lexington Medical Center for the Cook/Price Mature Student Scholarships. For Freshmen attending schools that do not accept students for full admission to the Nursing program until the Junior year, acceptance to Pre-Nursing will meet the requirement.
