2 A Message from the President Thank you for your interest in VMP Trinity and thank you for considering us for your short-term rehabilitation, ventilator care or long-term skilled nursing needs. We are happy to provide you with the information in this brochure to help guide you in your decision making process. As you will see on the upcoming pages, VMP Trinity offers exceptional and specialized care with state-of-the-art equipment and a caring staff. Please consider visiting us for a personal tour. 3 Short-Term Care What is short-term care? After a hospital stay, you may need special medical care or physical rehabilitation to help you regain the strength to carry on your normal activities of daily life. You will receive this care at VMP Trinity in our Short-Term Rehabilitation Care Program on our Rehab Plaza. What kind of therapy will I receive? The Rehabilitation Department at VMP Trinity offers fully integrated, in-house therapy with physical, occupational and speech therapists. This in-house therapy programming ensures treatment provided is well coordinated with the rest of the interdisciplinary team, which includes Rehab, Nursing, Social Services, Campus Activities, Dietetics and Pastoral Care. 4 Types of Therapy Physical Therapy Physical Therapists work with patients who have experienced a physical injury or who have any variety of conditions such as arthritis, tendonitis, fractures, knee or hip replacement or a progressive neurological condition. After a baseline evaluation is completed, your therapist will meet with you to discuss your individualized treatment plan, goals and expectations. Occupational Therapy Occupational Therapists focus on functional skills designed to increase your independence. Their primary emphasis is working on improving daily activity skills such as personal grooming, dressing, unpacking groceries, washing dishes, housekeeping and more. This therapy helps you remain independent when you return home. Speech Therapy A Speech Language Pathologist evaluates three main areas: communication, swallowing capabilities and cognitive skills. If your evaluation indicates the need for speech therapy, a treatment plan will be developed and integrated with your physical and occupational therapy goals and objectives.

5 Short-Term Rehabilitation What Should I Bring for my Short-Term Stay? We will make this as easy as possible for you: Insurance cards Loose fitting and comfortable clothes (for 7-10 days) Robe and slippers Outer wear appropriate for the season Comfortable shoes Eyeglasses, contacts or hearing aids Who Makes the Decisions About my Care? You do, along with support from your family members and all of our healthcare team members. The team meets with you and your family on a regular basis to discuss your plan of care and when you can expect to return home. We understand that, but when the time is right to consider another option, a place where you can have a comfortable home and the security that comes with extensive on-site healthcare, we are here to help. Our philosophy is simple: provide excellent healthcare and services at the most reasonable cost possible. What Living Options do I Have? VMP Trinity offers a full array of services to meet the changing needs of seniors. Residential options include apartment living for active and independent adults, assisted care in a residential apartment setting for those who may need help with daily tasks, and long-term units for those in need of skilled nursing services. INDEPENDENT LIVING 142 apartments with several floor plans (studio, 1 and 2 bedroom) Secure, park-like setting Beauty shop, bank, convenience store, chapel with worship services, computer/business center, art and textile areas, social gathering places Senior Community Club, activities, day trips, Packer parties, Brewer tailgates and other planned events Medical clinic, Wellness Registered Nurse

7 ASSISTED LIVING 80 apartments are co-mingled within part of our independent living complex, which means the transition to assisted living does not require a move. Same amenities as those provided for independent seniors Programs and activities specifically designed to match each resident s outside interests and hobbies An RN Clinical Coordinator oversees the nursing staff, which includes LPNs and specially trained medication administration CNAs. SKILLED NURSING CARE 58 private and semi-private rooms Care is provided under the direction of an RN Director of Nursing and RN Clinical Coordinators. We prepare individualized, detailed care plans designed to meet specific medical, psychosocial, rehabilitation, dietary, mobility, personal, spiritual and comfort needs of residents. These care plans are reviewed daily by our interdisciplinary team. VENTILATOR CARE What is the Mission of VMP Trinity s Ventilator Program? We have developed our program to enhance the lives and independence of ventilator assisted residents by providing the best patient-centered quality of care in a homelike environment. How Do You Accomplish This? Our quality care begins with our experienced and talented staff. We have board certified pulmonologists, recreational therapists, board certified dietitians, licensed social workers, case managers, ventilator certified RNs and LPNs, wound certified RNs, clinical psychologists and psychiatrists and rehabilitation therapists working together to meet the varied needs of not only our patients, but family members as well. What Sets VMP Trinity Apart from Other Ventilator Units? We are the largest ventilator program in Wisconsin and the only ventilator facility in the southern half of the state. Our team of licensed respiratory therapists, who are able to handle complex respiratory patients, including ventilator, CPAP and BiPAP, are in the facility and on the unit 24 hours per day, 7 days per week. We also have a very successful ventilator liberation program with a goal of sending patients back to their homes and to a better quality of life.
