The Role: You will be responsible for organising and creating a varied activities programme, from organised hobbies to group events, all of which will promote independence, choice and dignity for our residents. You will get to know our residents, their interests and life stories, so you can deliver events that truly enrich their lives and ensure they feel connected with their community. Whilst motivating colleagues to support events in the home, you will also be responsible for actively promoting activities in the community and encourage family and friends to become involved. You will take a lead in providing the marketing team with regular information, photos, posters and news about special moments, along with events and activities planned at the home. Responsibilities 5.1 Coordinate and display a calendar of activities for the home, to include both indoor and outdoor activities. 5.2 Develop a monthly programme of activities and ensure that these are available seven days of the week. 5.3 Responsible for producing and maintaining the annual activities planner; to include national days such as Dignity in Care, National Care Home Day, Dementia Awareness etc. 5.4 Research and implement activities which are effective and appropriate for people living with dementia. 5.5 Establish relationships with the local community and promote resident involvement. 5.6 To formulate a meaningful occupation and activity profile for each individual resident in liaison with the resident, relatives and care colleagues. 5.7 Review occupation and activity profiles on a regular basis with the residents and relatives and adjust if necessary. 5.8 In liaison with the Home Manager, plan and coordinate outside entertainers. 5.9 Coordinate celebrations for resident s birthdays To maintain accurate and legible records of individual resident s involvement in occupation and activities To participate in resident of the day by arranging a special activity of the residents choice andor a special meal of choice etc To ensure that photos are taken of activities which take place and display in the home for residents and relatives to enjoy Ensure that the appropriate consents are completed and signed for the display of photos and inclusion on the company website and Facebook To provide the marketing team with regular, up to date information of planned events, news, celebrations, outcomes following events and special moments To be a champion of company ethos Make Someone s Day 5.16 To liaise with care colleagues and the Home Manager in respect of resident medical or nursing care information and report any concerns immediately To establish and maintain regular resident s and relative s meetings along with the Home Manager. 3 5.18 Encourage, mentor and support care and housekeeping colleagues to be involved in and provide meaningful occupation and activities for residents In liaison with the Home Manager and Administrator, plan and organise fund raising events. 6.0 General Requirements In addition to the above, there are some general requirements that apply to all job roles in the Care Home: 1. Take responsibility for the safeguarding of adults and follow the company safeguarding and whistleblowing policies as required. 2. Participate in staff meetings as required 3. Participate in training activities as required 4. Participate in quality assurance systems as required 5. 4 Person Specification for role of Health and Wellbeing Coordinator CRITERION QualificationProfessional Membership Level 3 in Health & Social Care or a commitment to work towards Awareness of dementia care or a willingness to work towards First Aid Experience & Knowledge ESSENTIAL DESIRABLE Experience in arranging social activities in a care setting, H & S, Risk Assessment Experience in andor knowledge of the care of older people and people living with dementia Good knowledge of the social aspectmodel of care, holistic approach Previous experience of setting andor participating in groups and committees Experience of organising fund raising events Skills & Abilities Creative Thinking Excellent verbal and written communication skills ability to adapt to meet the communication styles and needs of individuals Excellent team worker strong ability to motivate others in the development and delivery of meaningful occupation and activities for older people.
