1 Memorandum of Agreement University of Hawai`i-West O`ahu (UHWO) University of Hawai`i at Hilo (UHH) UHWO Pre-Nursing Pathway UH Hilo Bachelor of Science in Nursing (BSN) This agreement will facilitate the pathway from the UHWO Pre-Nursing Pathway to the UH Hilo Bachelor of Science in Nursing (BSN). This agreement accords with the UH system "Memorandum of Agreement: Transfer of General Education Core Requirements (May 2010), and the UH Hilo Academic Affairs General Education Transfer policy (Fall 2011). The Pre-Nursing Pathway enables UHWO students to complete all the general education and pre-core for nursing requirements at UHWO and apply the credits toward the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN. Appendix A specifies prerequisites for students to enter and course requirements needed to complete the BSN program. Admission criteria and eligibility are also specified in Appendix A. Students will initially be admitted to UH West O`ahu as "Pre-Nursing Pathway" students (meaning Pre- BSN program) and will apply to the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN after having completed the minimum requirements for BSN admission. UHWO Pre-Nursing Pathway students will be provided with support services and opportunities needed to prepare for admission into the Nursing program. The establishment of a Learning Community and related Title III grant supported activities is detailed in Appendix B (See attached: ʻʻIke Mauli Ola Nursing Pathway Partnership ). A detailed chart of the anticipated Title III funding support ( ) to be contributed toward the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN is featured in Appendix C. It is expected that UHWO will have a BSN program, in place in West O`ahu, in the academic year, thus the last cohort to transfer from UHWO to the UHH BSN program, will be in the academic year. For this reason, there is no expectation that this MOA will be extended beyond the academic year. For admission to the BSN program students will need to apply and meet the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN criteria for admission. A maximum of 20 UH West O ahu students will be accepted for each Fall cohort. If UH West O ahu enrolls less than the maximum 20 seats designated, the unfilled seats will be offered to UH Hilo BSN applicants at the discretion of the UH Hilo School of Nursing. 2 Under the terms of this Agreement, UH West O`ahu agrees to: 1. Offer all current general education and pre-core requirements for the UH Hilo Nursing BSN program, with the exceptions of NURS 48 Pathophysiology and NURS 75 Applied Human Nutrition online which UH Hilo will offer. UH Hilo will receive the tuition revenue from these online courses. 2. Identify a UHWO Student Services Advisor(s) who will partner with UH-Hilo's advisors to ensure timely and accurate advising on pre-admission, admission, degree requirements and other related advising information for students in the UHWO Pre-Nursing Pathway.. Identify a UHWO faculty member who will serve as the faculty advisor to students in the ʻIke Mauli Ola Nursing Learning Community. 4. Form a UHWO admissions and selection committee responsible for the intake and initial review of applications for the ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN and will forward recommendations to the UH Hilo Director of Nursing. 5. Develop a one credit UHWO seminar course to support students accepted into the ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN program and require students in the BSN cohort to enroll each semester. The seminar will include community engagement activities, academic support opportunities, and social networking experiences. 6. Meet with UH Hilo faculty and/or administration minimally every semester, or on an as needed basis, to discuss potential and planned curricular changes. 7. Cooperate with UH Hilo on the matriculation and transfer processes for the BSN program. 8. Identify, review, and approve clinical sites and clinical faculty on O`ahu where students can complete all their clinical practicum courses and notify UH Hilo so that the hiring and related processes including MOAs can be completed. 9. Upon termination of Title III funds, UH West O ahu will ensure the continued funding support of one 12 month, 1.0 FTE Nursing Instructor position responsible for the Nursing Simulation Lab and the review and approval of clinical sites and faculty on O ahu. Salary range: Approximately $82,400 will be allocated for the salary of the Nursing Instructor position with additional fringe benefits amounting to an estimated $8, Upon termination of Title III funds, UH West O ahu will ensure the continued funding support of one 9 month, 0.5 FTE position the purpose of providing continued IT Simulation Lab Support and delivery of Distance Education. Salary range: Approximately $2,175 will be allocated for the salary of the IT Support position with additional fringe benefits amounting to an estimated $5, Upon termination of Title III funds, UH West O ahu will cover the following additional costs: (a) Consumable and Program supplies for Nursing Simulation Lab (b) Program supplies for Nursing prerequisite course offerings 12. Purchase equipment for synchronous distance delivery of Nursing coursework using Title III funding. A maximum of $20,000 ($10,000 per campus) will be allocated towards Polycom or other appropriate technology by Spring BSN Nursing Pathway and Partnership: MOA & Appendix A Page 2

3 1. Provide adequate space, technology and personnel for proctoring BSN testing and related exams. 14. Provide adequate space, technology, and IT support for online teaching. Under the terms of this Agreement, the University of Hawai'i at Hilo agrees to: 1. Identify a Nursing school faculty advisor(s) at UH Hilo who will partner with UHWO advisor(s) to ensure timely and accurate advising on pre-admission, admission, degree requirements and other related advising information. 2. Upon formal acceptance to the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN students will be assigned a UH Hilo School of Nursing faculty advisor.. Meet with UHWO faculty and/or administration minimally every semester, or on an as needed basis, to discuss potential and planned curricular changes and delivery of coursework. 4. Cooperate with UHWO on the matriculation and transfer processes for the BSN programs. 5. Waive the Student Life Center fee and other fees in accordance with University of Hawai i system policy, for UHWO students enrolled in the BSN program as a part of the Ike Mauli Ola Nursing Partnership. 6. Review recommendations submitted by the UHWO admissions and selection committee for the ʻIke Mauli Ola Nursing Partnership BSN and select final candidates for admission to the Ike Mauli Ola Nursing program. 7. Accept up to 20 UHWO students into the ʻIke Mauli Ola Nursing Partnership BSN cohort each Fall for the 2017/2018, 2018/2019, 2019/2020 and 2020/2021 academic years. 8. Ensure space availability for UHWO Pre-Nursing Pathway students in prerequisite online nursing courses offered by UH Hilo, NURS 48 Pathophysiology and NURS 75 Applied Human Nutrition. 9. Hire faculty, instructors or lecturers necessary to deliver all UH Hilo BSN Nursing coursework to students accepted into the BSN cohort at UHWO and assigned clinical sites on O ahu, through Spring Placement of positions will be determined by UH Hilo School of Nursing. 10. Hire three 1.0 FTE Nursing instructor positions in Fall 2017, and three 1.0 FTE Nursing instructor positions in Fall Placement of positions will be determined by UH Hilo School of Nursing. If it is not possible to recruit and retain the specified six 1.0 FTE Instructors, the curriculum will be delivered by a mixture of Instructors and Lecturers. 11. Upon the recommendation of UHWO, hire and pay for clinical supervisors for students in the ʻIke Mauli Ola Nursing BSN cohort at sites identified and approved by UHWO. 12. Upon termination of Title III funds, UH Hilo will cover the costs of curriculum development, up to $10,000, for students enrolled in the ʻIke Mauli Ola Nursing Partnership BSN program for the duration of this MOA. 4 1. If fewer than anticipated qualified applications and acceptances are received for the UHWO Ike Mauli Ola cohort, the empty seats will be occupied by UH Hilo students and based on O ahu. This Memorandum of Agreement is effective beginning May This Agreement will be reviewed at least once a year in either May or June, and as necessary, in order to support the transfer, matriculation and graduation of BSN students from the University of Hawai i at Hilo. Changes to the curricular and admission requirements of either program subsequent to this agreement may require revisions to the agreement. This MOA will terminate on June 0, Should both parties agree to terminate the Agreement prior to June 0, 2022, UHWO and UH Hilo will honor the Agreement stipulations for students currently enrolled in the program at the time of termination. Both parties agree to make a good faith effort toward developing a UHWO Nursing BSN program to provide access for students on the Leeward side of O ahu to obtain a Bachelor of Science degree in Nursing. 5 Appendix A Preconditions and Prerequisites: BSN Recommended Class Scheduling for UHWO I. Pre-Nursing Pathway - 61 Credits This advising sheet has been developed to assist students seeking to prepare for admission and if accepted, to also prepare to meet the requirements for graduation, for the Bachelor of Science in Nursing degree program offered by UH Hilo. The coursework indicated reflects the most efficient academic pathway. Please note that although course titles and numbers listed reflect UH West O ahu coursework, the requirements indicated are in alignment with the academic program requirements of the UH Hilo campus, which can be found in the UH Hilo General Catalog accessible online at The goal of academic advising is to further enhance the educational mission of the university, and create quality, accessible advising partnerships with all students in a positive environment that supports student success. Admissions Criteria and Eligibility The minimum criteria for admission to the ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN program are as follows: 1. Timely submission of UH Hilo Common Application Form (Designate NURH for the major.) 2. Timely submission of nursing application form to ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN point of contact on the UH West O ahu campus.. A minimum 2.7 cumulative college grade point average (GPA) 4. A 2.7 minimum cumulative GPA for all courses listed as a Natural Science degree requirement including PHRM/NURS 20 and NURS 48 for the BSN program. 5. A minimum of 15 credits of UHWO coursework for fall 2017 admission and a minimum of 0 credits of UHWO coursework for admission in years 2018, 2019, and 2020, with priority given to students who have completed Pre-Nursing general education and pre-core at UHWO. 6. Completion of all college prerequisite courses prior to the start of the Fall term of entry into the BSN program (Note: Biology, Chemistry, PHRM/NURS 20 and NURS 48 classes must be passed with a C or better grade) 7. Only 16 credits of nursing prerequisite courses can be outstanding with a maximum of 8 credits in the required sciences by the end of the semester prior to application. Letters should be from teachers or immediate supervisors who have no familial relationship with the applicant and can give a first-hand account of the student s abilities. 10. In order to meet the objectives of the Title III Grant, preference will be given to students who have participated in the Ike Mauli Ola Learning Community and/or have Native Hawaiian ancestry. III. 11 Appendix B Title III: ANNH grant supported program: PIKO Project `Ike Mauli Ola Nursing Pathway and Partnership The purpose of the `Ike Mauli Ola Nursing Pathway and Partnership is to increase and assist students throughout the educational pipeline to become baccalaureate-prepared nurses. The goal is to increase student retention in health sciences and support programs that increase diversity in the health sciences and health related fields. A diverse, culturally-aligned, and competent workforce is necessary to increase access to quality health care, and improve health equity in Hawai`i. This project targets critical areas in which Native Hawaiian students may need additional support to continue toward careers in nursing. Recognizing that Native Hawaiian students may require additional supports for retention and persistence this project utilizes an assets bundle approach (Johnson and Bozeman) that articulates relevant factors that impact education retention and achievement, science socialization, network development, family expectations, and material resources. The program intends to develop an innovative and comprehensive strategy to support, retain and graduate Native Hawaiian students in health care careers. Addressing the needs for increasing the number of Native Hawaiian students in health science and health related occupations will positively benefit the local community and state. A health service and nursing workforce that provides the supply and distribution of well-educated and well-prepared nursing professionals and reflects the cultural and social values and needs of the communities in which they serve is critical to achieve the goal of high-quality, safe and accessible care. Ensuring that all populations have access to high-quality, culturally-aligned and competent care is necessary to reduce health disparities and improve health equity. 12 Focus on student success: The `Ike Mauli Ola Nursing Pathway and Partnership expands enrollment and retention of students entering health science and health related fields, builds programs that retain students, develops a BSN joint program between UH West O`ahu and UH Hilo, improves academic performance of students in the BSN program, increases the number of Native Hawaiian students successfully graduating from the BSN program, and increases the number of Native Hawaiian and other graduates with cultural competencies to practice in underserved communities of Hawai`i. The UHWO `Ike Mauli Ola Nursing Pathway and Partnership will work with UH Hilo to develop a BSN nursing partnership and pathway for UHWO students. The main objective of the nursing program is to train professional nurses with a Baccalaureate degree in nursing for improved healthcare in the Leeward O`ahu region. Developing a BSN Program in conjunction with UH Hilo will increase the number of nurses prepared to meet the challenging health care needs of the people of West O`ahu, increase the efficiency in the educational preparation of nurses and increase access to nursing education for Native Hawaiian students throughout the State of Hawai`i. The outcome of these programs will be the graduation of professional nurses who will be well educated, culturally sensitive, and caring and who will lead, expand, and promote health care at the highest levels in the West O`ahu region and throughout the State of Hawai`i. `IkeMauli Ola Nursing Pathway and Partnership Through a unique and innovative partnership between University of Hawai`i West O`ahu and University of Hawai`i Hilo, students interested in a career in nursing will be able to take prerequisite nursing classes on the University of Hawai`i West O ahu campus and then may apply University of Hawai i West O ahu and University of Hawai i at Hilo BSN Nursing Pathway and Partnership: Appendix B Page 2

13 to the UH Hilo Bachelors of Science in Nursing Program through the `Ike Mauli Ola Nursing Pathway and Partnership. The `Ike Mauli Ola cohort which will receive UH Hilo nursing instruction delivered on the UHWO campus, will be referred to the UHWO and UH Hilo `Ike Mauli Partnership BSN cohort. Students spend the first two years fulfilling General Education and pre-core requirements at UHWO campus and nursing courses are introduced in the third year from UH Hilo for those students who are accepted into the ``Ike Mauli Ola Partnership BSN. The third and fourth year UH Hilo coursework will be provided at the University of Hawai`i West O`ahu campus. The `Ike Mauli Ola Pre-Nursing Learning Community is supported by the Title III grant and will go towards supporting a cohort of Pre-Nursing students at UHWO. Each cohort participant is not guaranteed admission to the `Ike Mauli Ola BSN program and will need to apply separately for admission to the `Ike Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN. Any UHWO student who completes the admission requirements for UHWO is eligible to enroll in pre-requisite nursing courses and formally apply for admission to the `Ike Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN. 20 Memorandum of Agreement University of Hawai`i-West O`ahu (UHWO) University of Hawai`i at Hilo (UHH) UHWO Pre-Nursing Pathway UH Hilo Bachelor of Science in Nursing (BSN) This agreement will facilitate the pathway from the UHWO Pre-Nursing Pathway to the UH Hilo Bachelor of Science in Nursing (BSN). This agreement accords with the UH system "Memorandum of Agreement: Transfer of General Education Core Requirements (May 2010), and the UH Hilo Academic Affairs General Education Transfer policy (Fall 2011). The Pre-Nursing Pathway enables UHWO students to complete all the general education and pre-core for nursing requirements at UHWO and apply the credits toward the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN. Appendix A specifies prerequisites for students to enter and course requirements needed to complete the BSN program. Admission criteria and eligibility are also specified in Appendix A. Students will initially be admitted to UH West O`ahu as "Pre-Nursing Pathway" students (meaning Pre- BSN program) and will apply to the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN after having completed the minimum requirements for BSN admission. UHWO Pre-Nursing Pathway students will be provided with support services and opportunities needed to prepare for admission into the Nursing program. The establishment of a Learning Community and related Title III grant supported activities is detailed in Appendix B (See attached: ʻʻIke Mauli Ola Nursing Pathway Partnership ). A detailed chart of the anticipated Title III funding support ( ) to be contributed toward the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN is featured in Appendix C. It is expected that UHWO will have a BSN program, in place in West O`ahu, in the academic year, thus the last cohort to transfer from UHWO to the UHH BSN program, will be in the academic year. For this reason, there is no expectation that this MOA will be extended beyond the academic year. For admission to the BSN program students will need to apply and meet the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN criteria for admission. A maximum of 20 UH West O ahu students will be accepted for each Fall cohort. If UH West O ahu enrolls less than the maximum 20 seats designated, the unfilled seats will be offered to UH Hilo BSN applicants at the discretion of the UH Hilo School of Nursing. 21 Under the terms of this Agreement, UH West O`ahu agrees to: 1. Offer all current general education and pre-core requirements for the UH Hilo Nursing BSN program, with the exceptions of NURS 48 Pathophysiology and NURS 75 Applied Human Nutrition online which UH Hilo will offer. UH Hilo will receive the tuition revenue from these online courses. 2. Identify a UHWO Student Services Advisor(s) who will partner with UH-Hilo's advisors to ensure timely and accurate advising on pre-admission, admission, degree requirements and other related advising information for students in the UHWO Pre-Nursing Pathway.. Identify a UHWO faculty member who will serve as the faculty advisor to students in the ʻIke Mauli Ola Nursing Learning Community. 4. Form a UHWO admissions and selection committee responsible for the intake and initial review of applications for the ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN and will forward recommendations to the UH Hilo Director of Nursing. 5. Develop a one credit UHWO seminar course to support students accepted into the ʻIke Mauli Ola UH West O`ahu and UH Hilo Nursing Partnership BSN program and require students in the BSN cohort to enroll each semester. The seminar will include community engagement activities, academic support opportunities, and social networking experiences. 6. Meet with UH Hilo faculty and/or administration minimally every semester, or on an as needed basis, to discuss potential and planned curricular changes. 7. Cooperate with UH Hilo on the matriculation and transfer processes for the BSN program. 8. Identify, review, and approve clinical sites and clinical faculty on O`ahu where students can complete all their clinical practicum courses and notify UH Hilo so that the hiring and related processes including MOAs can be completed. 9. Upon termination of Title III funds, UH West O ahu will ensure the continued funding support of one 12 month, 1.0 FTE Nursing Instructor position responsible for the Nursing Simulation Lab and the review and approval of clinical sites and faculty on O ahu. Salary range: Approximately $82,400 will be allocated for the salary of the Nursing Instructor position with additional fringe benefits amounting to an estimated $8, Upon termination of Title III funds, UH West O ahu will ensure the continued funding support of one 9 month, 0.5 FTE position the purpose of providing continued IT Simulation Lab Support and delivery of Distance Education. Salary range: Approximately $2,175 will be allocated for the salary of the IT Support position with additional fringe benefits amounting to an estimated $5, Upon termination of Title III funds, UH West O ahu will cover the following additional costs: (a) Consumable and Program supplies for Nursing Simulation Lab (b) Program supplies for Nursing prerequisite course offerings 12. Purchase equipment for synchronous distance delivery of Nursing coursework using Title III funding. 22 1. Provide adequate space, technology and personnel for proctoring BSN testing and related exams. 14. Provide adequate space, technology, and IT support for online teaching. Under the terms of this Agreement, the University of Hawai'i at Hilo agrees to: 1. Identify a Nursing school faculty advisor(s) at UH Hilo who will partner with UHWO advisor(s) to ensure timely and accurate advising on pre-admission, admission, degree requirements and other related advising information. 2. Upon formal acceptance to the ʻIke Mauli Ola West O`ahu and UH Hilo Nursing Partnership BSN students will be assigned a UH Hilo School of Nursing faculty advisor.. Meet with UHWO faculty and/or administration minimally every semester, or on an as needed basis, to discuss potential and planned curricular changes and delivery of coursework. 4. Cooperate with UHWO on the matriculation and transfer processes for the BSN programs. 5. Waive the Student Life Center fee and other fees in accordance with University of Hawai i system policy, for UHWO students enrolled in the BSN program as a part of the Ike Mauli Ola Nursing Partnership. 6. Review recommendations submitted by the UHWO admissions and selection committee for the ʻIke Mauli Ola Nursing Partnership BSN and select final candidates for admission to the Ike Mauli Ola Nursing program. 7. Accept up to 20 UHWO students into the ʻIke Mauli Ola Nursing Partnership BSN cohort each Fall for the 2017/2018, 2018/2019, 2019/2020 and 2020/2021 academic years. 8. Ensure space availability for UHWO Pre-Nursing Pathway students in prerequisite online nursing courses offered by UH Hilo, NURS 48 Pathophysiology and NURS 75 Applied Human Nutrition. 9. Hire faculty, instructors or lecturers necessary to deliver all UH Hilo BSN Nursing coursework to students accepted into the BSN cohort at UHWO and assigned clinical sites on O ahu, through Spring Placement of positions will be determined by UH Hilo School of Nursing. 10. Hire three 1.0 FTE Nursing instructor positions in Fall 2017, and three 1.0 FTE Nursing instructor positions in Fall Placement of positions will be determined by UH Hilo School of Nursing. If it is not possible to recruit and retain the specified six 1.0 FTE Instructors, the curriculum will be delivered by a mixture of Instructors and Lecturers. 11. Upon the recommendation of UHWO, hire and pay for clinical supervisors for students in the ʻIke Mauli Ola Nursing BSN cohort at sites identified and approved by UHWO. 12. Upon termination of Title III funds, UH Hilo will cover the costs of curriculum development, up to $10,000, for students enrolled in the ʻIke Mauli Ola Nursing Partnership BSN program for the duration of this MOA. 23 1. If fewer than anticipated qualified applications and acceptances are received for the UHWO Ike Mauli Ola cohort, the empty seats will be occupied by UH Hilo students and based on O ahu. This Memorandum of Agreement is effective beginning May This Agreement will be reviewed at least once a year in either May or June, and as necessary, in order to support the transfer, matriculation and graduation of BSN students from the University of Hawai i at Hilo. Changes to the curricular and admission requirements of either program subsequent to this agreement may require revisions to the agreement. This MOA will terminate on June 0, Should both parties agree to terminate the Agreement prior to June 0, 2022, UHWO and UH Hilo will honor the Agreement stipulations for students currently enrolled in the program at the time of termination. Both parties agree to make a good faith effort toward developing a UHWO Nursing BSN program to provide access for students on the Leeward side of O ahu to obtain a Bachelor of Science degree in Nursing.
