1 American Health Lawyers Association Continuing Education Request Form Annual Meeting June 25-27, 2018 AHLA ID: Firm/Affiliation: Name: Type of Credit State Bar/License Number (CLE, CPE, CCB) Please read carefully This form must be completed, signed, and submitted to AHLA to receive Continuing Education Credits. Indicate the type of credits you are seeking above and include the state(s) and bar/license information. Indicate each session you attend. If you did not attend a particular session in its entirety, please darken BOTH the bubble corresponding to the session title, and the bubble corresponding to the statement which indicates that you did not attend the entire session. Indicate the number of minutes you were in attendance at the session in the space provided. You will receive credit based on the actual amount of time in each session. If you do not indicate the amount of time, you will automatically receive the full credit. If you have a filing deadline approaching and need your request processed within a specific time frame, please make that notation on the top page of this request. Credit is not given for welcome/introductory remarks, luncheons, and other non-substantive discussions (this sometimes applies to keynote addresses). Credit Hours Information CLE: The maximum number of credits available for attorneys is credits for a 60-minute hour, including up to 4.75 of Professional Responsibility (2.0 ethics credits/2.25 diversity/1.0 substance abuse) or 20.7 for a 50- minute hour, including up to 5.70 of Professional Responsibility (2.4 ethics credits/2.7 diversity/1.2 substance abuse) for 50-minute states. Please note that the actual number and type of credits approved may vary from state to state. NY and NJ licensed attorneys must sign-in and out of every session, in addition to filling out this request form. CPE: AHLA is a registered with the National Association of State Boards of Accountancy (NASBA) as a sponsor of continuing professional education on the National Registry of CPE Sponsors. State boards of accountancy have final authority on the acceptance of individual courses for CPE credit. The credit maximum is 20.0 hours. In accordance with NASBA requirements, in addition to filling out this form, you MUST sign in and out of each session. CCB: Compliance Certification Board has approved this program for 20.7 CCB Continuing Education Units. A certificate will be available within 30 days of the program. The credits you receive will be based solely on the individual credit form you return to AHLA. Certificates of Completion (including any necessary additional information) will be available within 30 days following the program. All attendees who submit a form will receive an with instructions on downloading their certificates through AHLA s website. You do not have to be a member of AHLA to log on and download a certificate. 1
