8 Opportunity at Initial Service Re Design Process mapped the service user pathway with a small focus group, losing unnecessary tasks and interventions. Designed the Care Navigator JD and competencies to support the new pathway. Conducted assessment centre interviews based on these competencies Appointed to two posts to support the Acute Mental Health Team in July 2014

9 What happened next? A period of induction The admin and Navigator away day..another process map! Identification of tasks that clinicians NEEDED to undertake, and those that the admin team and the Care Navigators could do.

10 What happened next? We put together a clearly defined structure of who should do what, releasing clinicians to do what they do best and the Care Navigators and admin team doing what they do best. The right people doing the right jobs!

12 What do the Care Navigators do? Support the governance and management of the whole service user pathway, to ensure that each step in a service users journey meets the high standards it should for quality and safety. Undertake the functions for the safe transition of service users to other services in conjunction with key stakeholders ( the most risky time, so we need to get it right!)

13 What do the Care Navigators do? Work closely with a centralised bed management team and transfer facilitator on the identification of beds ( a task previously performed by clinicians, taking up hours of the day) Monitor clinical tasks and collect historical data along the whole of the care pathway ensuring service users receive safe transition between services. Monitor and support the management team to ensure we meet our KPI s.

15 How have we seen the role grow? Work with the AMHT, CMHT and Inpatient services closely on barriers to discharge and we proactively support discharge from hospital. A real focus on reducing the number of out of area placements

16 What s the added value? Clinicians can focus on clinical work Clinicians can have the systems and supports in place to remind them of where the person is on the pathway All correspondence to stakeholders is standardised, helping to protect our reputation! Transitions through services have a robust system that doesn t rely on individual team members to manage alone
