Vision: Living Every Moment Role Purpose The primary function of this role is to provide consistent clinical and operational leadership for the effective delivery of Lake Taupo Hospice clinical services. As part of the clinical team at Lake Taupo Hospice the Clinical Team Leader will provide leadership, actively promote and deliver high quality clinical care, clinical education and career progression planning for clinical staff. The Clinical Team Leader is also a key player in promoting and upholding the brand integrity of Lake Taupo Hospice and ensuring the Hospice continues to be recognised as a professional service provider of high quality, patient focused, palliative care. The Clinical Team Leader has a focus on improving and enhancing clinical practice, centred on the patient and their family/whānau. The Clinical Team Leader practices at a level that requires a high degree of competency in the theoretical and practical application of palliative care nursing and family DRAFT 24 September
2 support services, and demonstrates expert utilisation of this knowledge in implementing and advising on the holistic care of patients and their family/whānau. The Clinical Team Leader effectively manages the clinical team (Nursing and Family Support) and resources, role model s quality clinical practice, facilitates the use of evidence based practice standards, policy and guidelines, and fosters both education and research review. The role contributes to the strategic direction for clinical services and the wider organisation. The Clinical Team Leader networks across the health services at national, regional and local levels to contribute to palliative care clinical service development, and to inform internal and external stakeholders of palliative care developments. Key Accountabilities Key Areas of Accountability What is the activity? Provide clinical and professional leadership for the Nursing and Family Support teams (Clinical Team) Develop a positive culture within the Clinical Team that consistently reflects Lake Taupo Hospice values, is aligned with service-wide developments, and encourages a sense of commitment and shared purpose. Lead and motivate staff to develop a cohesive and highly functioning team and an environment that encourages learning. Act as a professional role model, advocate and mentor for staff. Develop and monitor appropriate systems and processes to ensure all stages of the patient s journey are managed efficiently and effectively, from referral to discharge. Work collaboratively with staff and involve them in case reviews, relevant forums, and the planning, delivery and evaluation of initiatives, to encourage and support a culture of shared leadership. Act as a clinical resource for staff to develop ongoing knowledge and skills across the team. Utilise a range of strategies to provide visible clinical leadership and oversight for the team to ensure that an excellent standard of care is consistently provided and patient safety is enhanced. Encourage and / or provide innovative and proactive solutions to emerging issues. Establish and maintain effective working relationships with colleagues and a range of internal and external stakeholders. Work collaboratively with DHB, primary care and age related residential care providers, Key Performance Indicators How is it measured? There is a high level of engagement, and the Clinical Team is highly regarded amongst their peers. Clinical Team maintain appropriate and accurate electronic records. Information is recorded, if not immediately, within a 24-hour period. Consistent audits are undertaken. Patients have continued confidence in the knowledge and skills of the Clinical Team evidence is collected to measure this i.e. surveys, patient stories, compliments or complaints. % of Role 30% DRAFT 24 September
3 and other relevant services, to support continuity of care, service integration and high-quality palliative care for patients and their families/whānau. Support Clinical Team to facilitate safe and culturally appropriate services to Maori, including access to Maori treatment options and involvement of whānau in planning and delivery of care. Recognise and facilitate the rights of all patients/clients and their whānau to participate in cultural activities. Efficiently fulfil all operational management functions Participate in strategic and operational planning processes for Lake Taupo Hospice and lead delegated areas of responsibility. Actively monitor and manage the Clinical Team budget ensuring costs are maintained within planned budget and informaning CEO/GM and Finance Administrator of any variations. Participate in budget development and monitoring processes, assist with development of the annual capital plan, and demonstrate fiscal responsibility. Ensure relevant initiatives are planned, implemented, monitored and reported upon in a timely manner. Promote collaboration within Lake Taupo Hospice Clinical Team and foster harmonious working relationships with all departments. Ensure rostering and staff allocation processes reflect best practice and enable equitable workloads. Maintain effective human resource practices in accordance with Lake Taupo Hospice policies and procedures. Participate in planning strategies for recruitment and retention of nursing staff to maintain an appropriately skilled and qualified workforce. Provides a voice for provincial /rural palliative care and influences decision making at local, regional and national level. Work with the CEO/GM to address performance issues and with investigations that involve departures from professional practice standards. Meet regularly with the CEO/GM to discuss patient safety and quality issues, resource Clinical Team operates within allocated budget. Clinical Team delivers organisational and patient focused KPI s. Good level of team work and apparent. Absenteeism kept at minimum level. Appropriate qualified staff are available as required at all times. Leadership initiatives visible across Clinical Team. 4 utilisation, performance against budget, professional issues and developments. Ensure daily operational issues that arise within the team are effectively and efficiently managed. Work with the Volunteer Manager and volunteers to ensure the loan equipment service functions effectively. Where relevant, and in collaboration with relevant team members, facilitate and evaluate trials of new products and/or equipment. Decrease risk and enhance quality of care and patient safety through leading and contributing to quality improvement initiatives Promote evidence-based practice and a culture of patient safety and continuous quality improvement. Ensure that staff work to documented, appropriately approved policies, procedures and guidelines, and contribute to the development and review of these documents. Ensure that documentation meets legislative, ethical and professional standards. Support clinical audit processes and ensure aspects identified as requiring improvement are addressed in a timely manner. Actively lead and participate in a range of quality improvement activities and facilitate the integration of Hospice New Zealand standards for Palliative care into clinical practice. Ensure that incidents are investigated and resolved appropriately, and that actual/potential risks are identified and addressed. As appropriate, work with the CEO to address and resolve complaints. Ensure systems are developed and maintained to support the professional development of the team Support the Clinical Nurse Educator and designated preceptors in the development and implementation of a comprehensive orientation programme for nursing staff. In collaboration with the Clinical Nurse Educator, provide support for nurses undertaking preceptorship training and fulfilling this role. Robust best practice Palliative Care plan is in place for patients and family/whānau. Interdisciplinary initiatives visible across Clinical Team. Organisation is not exposed to unnecessary risk or costs associated with EEO noncompliance or treaty principles. Organisation is not exposed to Health and Disability commission complaints due to poor professional standards or practice. Incident, Accidents and Near Miss reports are investigated, and appropriate actions taken according to LTH policy. Risk register is maintained up to date. Effective working relationships in place in required areas. Effective professional networks are in place and maintained. 5 Work with Clinical Nurse Educator and Clinical Team members to ensure processes are established to assess and meet educational requirements of individuals within the Clinical Team, and the team as a whole. Assist staff to resolve ethical issues and support debriefing sessions for staff as required. Provide mentorship and coaching for staff. Undertake annual performance appraisals for members of the Clinical Team and support / monitor the achievement of objectives agreed upon during the year. Organisational Contribution Work with the CEO/GM to promote and implement the Lake Taupo Hospice strategic direction. Implement annual business, operations and financial plans that relate to the Clinical Team. Proactively engage as a member of the Clinical Team. Proactively lead by example and champion Lake Taupo Hospice policies and procedures. Proactively contribute to special projects when required. Work alongside the Fundraising and Promotions Manager to strengthen the public profile of hospice. Work alongside the Manager of Volunteer Services to foster and support the contribution of volunteers within the service Work alongside the Financial Administrator to develop the annual budget. Actively seek opportunities to improve systems, processes and work practices within own position and Lake Taupo Hospice as a whole. Actively promote the services of Lake Taupo Hospice externally. Integrate the Lake Taupo Hospice values into work practices and relationships. Health & Safety Promoting and supporting safe working practices within Lake Taupo Hospice at all times. Contributing to maintaining a safe and hazard free work environment by proactively identifying and managing hazards. Working collaboratively with Clinical Nurse Educator setting up training opportunities and continued development of Clinical Team, 12-month future planning.
