3 Summary This report and grades represent our assessment of the quality of the areas of performance which were examined during this inspection. Grades for this care service may change after this inspection following other regulatory activity. For example, if we have to take enforcement action to make the service improve, or if we investigate and agree with a complaint someone makes about the service. Young people and their families are fully involved in planning service delivery and evaluating the care provided. The organisation is sector leading in the range of training opportunities designed to improve the value of the staff team. The service works extremely hard to improve the health and wellbeing of young people. What the service could do better The service should continue to make further improvements to the environment. The service should continue to progress with the introduction of the new format of care plans. What the service has done since the last inspection The service had purchased screening for the outside Astroturf area. This had increased the privacy of young people. 5 1 About the service we inspected Kibble Safe Centre is a secure accommodation service that provides 18 secure beds for young people aged from 11 to 18 years. Located in Paisley, Kibble Safe Centre is administered by a voluntary Board of Trustees on behalf of the Elizabeth Kibble trust. Before 01 April 2011, this service was registered with the Care Commission. On this date, the new scrutiny body the Care Inspectorate took over the work of the Care Commission, including the registration of care services. Information in relation to all care services is available on our website at The service is inspected by us at least twice per year on an announced and unannounced basis. As well as being inspected by the Care Inspectorate, the Safe Centre is also subject to joint inspections by Education Scotland at four-year intervals, and is registered with the Registrar of Independent Schools. The service has indoor recreational facilities including a swimming pool and small gym. Programmes are put in place for young people by the Specialist Intervention Services (SIS) Team. The accommodation for the young people is a single bedroom which includes a toilet and shower en-suite with fixtures and fittings designed to limit self harm. Living areas are suitably equipped and furnished, with a small satellite kitchen and dining area. Perimeter security is provided by the buildings and is supplemented by CCTV monitored by a designated team of operations staff. Requirements and recommendations If we are concerned about some aspect of a service, or think it needs to do more to improve, we may make a recommendation or requirement. - A recommendation is a statement that sets out actions the care service provider should take to improve or develop the quality of the service based on best practice or the National Care Standards. - A requirement is a statement which sets out what is required of a care service to comply with the Public Services Reform (Scotland) Act 2010 ("the Act") and secondary legislation made under the Act, or a condition of registration. Where there are breaches of Regulations, Orders or conditions, a requirement may be made. Requirements are legally enforceable at the discretion of the Care Inspectorate. 7 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection We wrote this report after an unannounced inspection by two Inspectors, Mark Causer and Janis Toy. We visited Kibble Safe Centre on 09 and 10 of January We gave feedback to the Executive Director, Board Member and the Quality Assurance Manger on 10 January As requested by us, the provider sent us an annual return and self assessment form. In this inspection we gathered evidence from various sources, including relevant sections of policies, procedures, records and other documents, including: additional evidence from the service's self assessment; records of complaints, accidents, incidents and restraints; young people's records including personal plans; records of young people's meetings; discussions with managers and staff; discussions with young people; observation of staff working with young people; observation of the environment. The focus of this inspection was to evaluate progress made by the service in achieving the requirements and recommendations of the previous inspection in June 2012 and should be read in conjunction with this report. Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. 8 Inspection Focus Areas (IFAs) Inspection report continued In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. Where we have examined an inspection focus area we will clearly identify it under the relevant quality statement. Fire safety issues We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may consider what action to take. 9 What the service has done to meet any recommendations we made at our last inspection All recommendations were fully met or were being developed further by the service. The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We check to make sure this assessment is accurate. The service had already submitted a self assessment prior to the previous inspection. This identified what they did well and areas where they could improve. Taking the views of people using the care service into account The inspectors interviewed 8 young people and spoke to 2 other young people in the passing. All young people with one exception indicated high levels of satisfaction with the service. All indicated that they had opportunities to comment and offer their opinions of the service. All stated that staff were extremely supportive and caring. Many young people gave examples of staff being respectful towards them. Young people were very happy and relaxed with staff. Young people made a range of positive comments, some of which are listed below: * "Staff are good they are very nice." * "My room OK, I have not put up posters because there's no point as I am going home." * "The food is very good." * "We all have the chance to change the menu." * "There is no bullying. If anything crops up staff sort it out." * "I don't like the food but I'm a fussy eater." * "My room is very comfortable." * "We have mix up nights were we play other games with other units." 11 3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 5 - Very Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service strengths All aspects of this Quality Statement were met, some aspects were exemplary and the performance of the service was graded excellent. This is characterised by the manner in which the service had positively engaged young people and carers in evaluating and developing service provision. The service continued to have in place strategies which allowed young people and carers' participation in the assessment of the quality of care provided. These strategies were examined and were reported upon in the previous inspection report: 'We found that the service had a variety of innovative approaches that encouraged young people and their families to play an active part in maintaining and improving the quality of care at Kibble. Every young person we spoke with told us that they had a say in the day to day life of the service. One example of young person's views being listened to in relation to their care was that one young person said that they were uncomfortable wearing the short pyjamas which are all that are usually allowed in Safe Centre. They, supported by key staff consulted with parents, managers and other professionals purchased alternative nightwear. This had contributed to the young person becoming more settled at night, reducing the need for very regular observations. Young people attended weekly unit meetings where they identified and discussed issues that were important to them. Where necessary these issues were given priority at staff and management meetings to ensure a quick response to issues raised. Observations of young people's meetings demonstrated a high level of participation with young people taking responsibility for setting the agenda, chairing the meeting Kibble Safe Centre, page 11 of 23

12 and taking minutes. Issues discussed and resolved recently included changes to menu options, the purchase and repair of games consoles, the personalisation of rooms and changes to activity times to take account for other commitments that young people had undertaken as part of their care plan agreements. School Council Meetings took account of decisions made at unit meetings and vice versa. The issues raised were discussed at Board meetings on a regular basis and as there was regular contact between young people and the board members. The young people told us that they felt that their ideas and concerns were therefore being taken seriously at all levels. We found that on occasion young people used the formal complaints procedure to raise issues of concern. Young people told us that they were all aware of the complaints procedure and that they felt the responses they received were quick and fair. Complaints were reviewed by the Executive Director of the Safe Centre and a written response was provided very quickly depending on the level of investigation required. We found that responses were insightful and took account of the personal circumstances of young people when making the complaint. We found that the each unit had a development plan that took account of the views of young people, parents and others. Aims included the involvement of young people and others to reach achievable targets. These included the upgrading of the visitors' room which had been raised in parental questionnaires, improvements to the games room that had been identified through unit meetings and the continued presence of management staff on the floor of each unit. Young people confirmed that they had appointed a peer representative who signed the unit development plan to acknowledge their agreement with the priorities set. Young people had regular scheduled time with their key worker to review and update their Kibble Journey which was their individualised care plan. We reviewed these documents and confirmed young people's input and highlighted priorities going forward including mobility, programme work, contact and changes to their rooms, activities and other day to day issues. Families completed questionnaires on a regular basis and where appropriate contributed to plans and participated in family work sessions. We spoke to one parent who told us that staff keep them closely involved with any events to do with her child. This had contributed to a growing confidence in the service, and a high level of satisfaction with the quality of care and support it provides. Young people confirmed they had regular access to advocacy services through Who Cares? Scotland and that they felt this was a useful resource. Kibble Safe Centre, page 12 of 23

13 The individual units had a number of questionnaires that they issued throughout the year. These included initial questionnaires for all young people. This allowed young people to feed back on the admission process. We were left with a strong feeling that the service welcomed and valued the opinions of young people. We felt that there were trusting relationships with people at all levels of the organisation and that this led to a feeling of confidence, that issues raised would be responded to sensitively, appropriately and positively.' Areas for improvement The service should maintain their excellent commitment to the participation of young people and their families in the day to day development of the service. Service strengths Inspection report continued All aspects of this Quality Statement continue to be met and while there are areas for improvement, the performance of the service was graded very good. The service continued to have in place strategies which allowed young people to make individual choices and ensured that they were supported to achieve their potential. These strategies were examined and were reported upon in the previous inspection report: 'Each young person had a learning profile. We found that homework was set daily Monday to Thursday and that completion was part of the service's incentive scheme. We observed residential staff supporting young people to complete their homework. We found that the service had made improvements in developing a 24 hour curriculum. This linked activities such as homework and out of school certification including lifesaving to educational achievement. This extension of the curriculum was welcomed by young people who told us that they felt better prepared to pursue interests and employment when the left the service. Young people had access to a diverse range of activities designed to improve their skills and develop their confidence. These included baking, gardening, magic workshops, drama and DJ'ing. We found young people were provided with a wide range of opportunities to participate in sport, skills development and work Kibble Safe Centre, page 13 of 23

14 opportunities, indeed one young person commented that they "chuck opportunities at you all the time." We noted achievements in education, certification in swimming, life saving and first aid. Young people apprenticeship opportunities that involved independent travel, promoting trust and responsibility. We noted the range of programmes and focused work that helped young people address issues in their behaviour and helped them develop skills and confidence to express themselves in a more positive way. This skilled and planned approach had helped contribute to the settled nature of the centre and improved outcomes such as a reduction in physical restraint for individual young people. Young people told us of access to community based clubs, such as rugby when it was considered safe to do so. There was regular and innovative use of outdoor areas to enrich the experiences and learning of young people. Young people told us that some had enjoyed Bee Keeping and that there was regular camping on the all weather pitch. We found that opportunities were very well planned and resourced to build a depth and breadth of experience. Staff were constantly building on the level of challenge faced by young people resulting in many very positive outcomes.' Areas for improvement The service had developed an initial care plan for young people which had all aspects of their care incorporated in the 'Kibble Journey'. They were in the process of developing an electronic form which would have elements such as Health Assessment and the first level psychological assessment being completed by relevant professionals and these parts populated into the overall care plan. This would reduce any delays gaining information for all professionals involved in young people's care. We look forward to monitoring further progress at the next inspection. The service should continue with their plans to move towards a more integrated planning framework that includes 'Getting It Right For Every Child' including the SHANARRI wellbeing outcomes and Curriculum for Excellence. This should include further integration towards a more robust 24 hour curriculum. This included support and structure for staff and more focus on outcomes for young people. Service strengths We did not inspect against this Quality Statement but have included our evaluation of the quality of participation in Quality Theme 1, Quality Statement 1.1. Areas for improvement See Quality Theme 1, Quality Statement 1.1. Grade awarded for this statement: 6 - Excellent Number of requirements: 0 Number of recommendations: 0 Statement 3 The environment allows service users to have as positive a quality of life as possible. Service strengths All aspects of this Quality Statement continue to be met and while there are areas for improvement, the performance of the service was graded very good. This is characterised by the manner in which the service had positively engaged service users and carers in evaluating and developing service provision. The service continued to have in place strategies which meant young people had a positive quality of life. These strategies were examined and were reported upon in the previous inspection report: 'We noted recent re-decoration that had resulted in the units appearing comfortable and well maintained. Records and feedback from young people told us repairs were carried out quickly. We visited the units at various times throughout the day and evening and found young people to be relaxed in public areas. We were impressed by the young people's involvement in daily living tasks such as washing their clothes and preparation for meal times. We noted access to IT equipment with the appropriate Kibble Safe Centre, page 15 of 23

16 safety features built in. Each young person had access to computer facilities within the main school area. There was restrictive access as the school had a effective fire wall and young people were aware that they were unable to down load inappropriate material. Young people were encouraged and supported to have their own identities and were able to choose their own clothes and personalise their own room. Many of the young people had possessions from their homes. Young people were motivated by the school incentive scheme, this promoted positive behaviour and assisted them gaining exclusive benefits such as decorating their own room. We found good value in the space made available for activities. This was true of outdoor space attached to each unit. This allowed staff and young people greater flexibility within each residential unit. It also allowed outdoor activities to be more easily monitored. There was also innovative use of the all purpose pitch in the main outdoor recreation area. Indoors the gym and swimming were very popular. We noted the flexibility and the compromises reached in making best use of these facilities. This involved careful planning and attention to each young person's care plan. This ensured young people did not lose out on recreational opportunities when completing other tasks identified in their care plans. Young people indicated that these efforts helped keep them on task.' The service had attached screening to the fencing in their out door space. Young people indicated that they liked this screening as it looked like "spectators at a football match." This had increased their privacy when they were using the outside space for recreation. Areas for improvement There was a need to review the use of the 'viewing panel' when observing young people in their rooms. In discussion at feedback the service acknowledged that there should, when appropriate, be a knock on the young person's door to inform young people that staff will be using the viewing panel.
