3 Goals and Objectives of the Proposed Network The MPHCAeHCC is a state-wide initiative to create a Fiber or Equivalent telecommunications/telemedicine network that will connect participating MPHCAeHCC HCPs located at sites operating in the geographic areas serving patients in the states of Mississippi and Louisiana. The goal is to assist regional and statewide health care providers and their patients gain increased access to an electronic information system and exchange of information that will be fully utilized to accomplish its objectives of improving patient safety (alert for medication errors, drug allergies, and emergency response), improve healthcare quality (make available complete electronic medical records, test results and x-rays at the point of care, integrate health information from multiple sources and providers, incorporate the use of decision support tools with guidelines and research results, etc.), and create a health information system for the purpose of sharing common patient medical information among MPHCAeHCC HCPs to improve quality of care and maximize cost efficiencies. The current and future circuit expectations are listed in the MPHCA RFP for each location. Strategy for Aggregating the Specific Needs of HCPs (including providers that serve rural and urban areas) Within a State or Region All HCPs within the MPHCAeHCC network have elected to be part of the USAC Health Connect program. By doing so, the MPHCAeHCC will be able to establish a private network. Also, for the underserved areas served by rural HCPs, we will be able to connect the HCPs via a VPN. In addition to speed and reliability, the MPHCAeHCC network is designed for security. Every site connected to the network will be provided with a managed firewall service. The firewall must be able to analyze traffic up to layer 3 of the OSI model. Connection into the HCPs Data Center will be shared between all of the members. All facilities will traverse the network to access production applications hosted in the Data Center. Connectivity to the data center is critical to daily operations, giving end users access to electronic health information and hosted applications. By aggregating these specific needs and collectively procuring these services, the HCPs participating in the MPHCAeHCC understands that they will see an economy of scale savings. Strategy for Leveraging Existing Technology to Adopt the Most Efficient and Cost Effective Means of Connecting Providers Not only will MPHCAeHCC ensure that existing infrastructure will be utilized when possible to reduce cost, but will also ensure that there is a plan for transitioning the existing infrastructure to the most current and suitable infrastructure when needed.

4 MPHCAeHCC will work with the service providers to establish a strategic network plan to address all current equipment, and any new equipment that is required within the MPHCAeHCC Network. The plan will be used to establish equipment standards and baselines for proper implementation and greater usability. How the Broadband Services Will Be Used to Improve or Provide Health Care Delivery MPHCA/MPHCAeHCC members currently provide comprehensive, primary and preventative health care services to over 288,047 users annually through a state-wide network of approximately 191 clinical sites. Currently about 113 (88 from the previous contract and 25 currently) clinical sites are participating in the consortium. The Vision, Mission and Goals of MPHCAeHCC is grounded in our overall vision, mission as it relates to the provision of quality services and supporting access to care for all Mississippians. Further, these will help MPHCAeHCC to fulfill the task set before it and to reach its vision for rural and urban state-wide health care digital and electronic communications and information exchange led by the organization. Our vision is to transform the current health care delivery system into a model that partners with Regional hospitals, various clinics and other providers to augment respective primary care capabilities and referrals to speciality care, inpatient and other supportive/collaborative services that will increase access to health care services, and improve population health outcomes. Our mission is to ensure that participating rural/urban healthcare facilities have high quality, affordable digital connectivity in order to fully utilize their Electronic Health Records (EHRs) and share Telemedicine and Telehealth services that will provide the residents of the state of Mississippi, both rural and urban, with the best health care possible. The broadband services will be used to assist regional and statewide health care providers and their patients to gain increased access to electronic information systems and exchanges of information that will be fully utilized to accomplish the objectives of improving patient safety (alert for medication errors, drug allergies, and emergency response), improve healthcare quality (make available complete electronic medical records, test results and x-rays at the point of care, integrate health information from multiple sources and providers, incorporate the use of decision support tools and guidelines and research results, etc.). and create a health information system for the purpose of sharing common patient medical information among MPHCAeHCC HCPs to improve the quality of care and to maximize cost efficiencies. Previous Experience in Developing and Managing Health IT (including telemedicine) Programs MPHCA s Information Technology (IT) department has more than twenty years (20) of project experience in IT networking and systems implementations. Implementing different Broadband with service providers and managing healthcare systems. MPHCA IT has worked with members to obtain broadband services outside of the HealthCare Connect Fund, Some examples of the projects and technologies include Implementation of a MPLS network, and MetroEthernet.

5 John Lunardini, Communications Director will manage and coordinate oversight for the project, in coordination with fiscal management with the CFO and report directly to the COO. The MPHCA team has significant experience, given that the organization has previously designed and implemented the functional statewide Telemedicine and video conferencing network that is hosted at MPHCA s site and currently connects all twenty (20) MPHCA HCPs organization sites. Currently there are 9 of the 20 MPHCA s HCPs organizations that are participating in the consortium, that make up the 103 clinical sites. MPHCAeHCC has established a formal process of addressing the development and management of its technology efforts via the MPHCAeHCC Advisory Board appointed Technology Committee, a formal standing technology committee consisting of Chief Information Officers from MPHCAeHCC member HCPs. Project Management Plan Outlining the Project s Leadership and Management Structure, and a Work Plan, Schedule and Budget MPHCA s Information Technology department will be the lead on the project with oversight from the MPHCAeHCC Advisory Board and the MPHCAeHCC Advisory Board appointed Technology Committee. The project will consist of these basic steps: 1. Post RFP and 461 (06/02/2017) 2. Review RFP responses/bids (28 days (06/30/2017)) 3. Select Winning Vendor (15 days (07/17/2017)) 4. Sign contract for services (30 days (07/31/2017)) 5. Formulate timeline and project schedule (10 days (08/11/2017)) 6. Circuit Installation (09/04/2017) 7. Test and Turn up to Live (11/01/2017) MPHCA and MPHCAeHCC participating HCPs will be responsible for their 35% portion. Sources of these funds are from federal 330 grant funds, patients services revenue, and other collections. As the administrative entity, MPHCA will pay the service provider from its operating account and rebill each participating HCP for their proportionate share of the 35% portion requirement. USAC funding will be utilized to help support the network services as stipulated by the USAC Health Connect Fund program requirements.
