How to Develop Successful Plans of Removal F When caring for persons living with dementia, a proactive approach to behaviors is always best practice. However, there are times when an acute behavior occurs that we must develop a person-centered plan of care to support the resident. Learning to develop plans that ensure the safety and well-being of the resident requires knowing the resident. This session will assist facilities in applying the Dementia Well-Being model of care in the development of their care plans and plans of correction. At this session, OLTC surveyors will provide examples and discuss lessons learned. Objectives: Define and discuss what a plan of removal is. Discuss how to determine causative factors of behaviors. Discuss how to successfully develop, implement, monitor and adjust the plan of removal. AHCA/NCAL s Solutions for Quality Care in Assisted Living F Unlock your Assisted Living community s potential by taking advantage of your AHCA/NCAL membership. Use the association s quality and data resources to implement quality improvement programs, pursue operational excellence and enhance relationships with entities like hospitals and other networks. This session will provide an overview of several key AHCA/NCAL member benefits, including LTC Trend Tracker, the Quality Initiative for Assisted Living, and the AHCA/NCAL Quality Awards Program, as well as how these benefits work in tandem.

7 Objectives: Identify key areas in Assisted Living to improve organizational performance and improve resident outcomes through the Quality Initiative. Understand the importance of collecting and reporting quality data in Assisted Living, and how to utilize LTC Trend Tracker. Learn about the benefits of applying for an AHCA/NCAL Quality Award, a program based on the core values and criteria of the Baldridge Performance Excellence Program. Alarms and Restraints Focus on the Effect on the Resident F SNFs typically have the goal of being restraint free, but often miss the boat when determining whether a device actually has the effect of restraining the resident. The determination of whether a device is a restraint is not dependent upon how the physician s order is written, but is actually determined by the device s effect on the resident. SNF Interdisciplinary team members must carefully review device use to determine not only whether the device is a restraint, but also whether the device is beneficial/necessary for that resident and in harmony with the resident s preference for care. The new Requirements of Participation added position change alarms to the list of possible restraints. Residents can hesitate to move because of fear or embarrassment related to the sounds of position change alarms. Learn how to correctly code the MDS items and how the new survey critical element pathway for restraints leads the surveyor through the restraint determination. Antibiotic Stewardship: A Stepwise Approach F F F Infection prevention and antibiotic stewardship are core activities to improve resident safety and quality of care. In this session, we will share an implementation guide developed to assist homes in implementing a successful program. We will share resources and lessons learned. Discuss appropriate resources and best practice protocols for antibiotic stewardship. Are You Setting Yourself Up to Succeed? F F F Everyone is a leader to someone. Resident Interviews Timeframes, Rules, Uses Resident interviews are at the heart of resident centered care. Errors in dating the interviews in Z0400 have resulted in reduction of payment for case mix or Medicare reviews.
