1 Chapter 17 Part 2 Body Mechanics Comfort & Safety Protect the person s skin from friction and shearing when moving and lifting (these can cause infection and pressure ulcers. Reduce friction and shearing by rolling or lifting the person. A cotton sheet serves as a lift sheet Information you will need Before lifting or moving a person, you need this information from the nurse and the care plan: Position limits and restrictions How far you can lower the head of the bed Any limits in the person s ability to move or be repositioned 1

What procedure to use How many workers are needed to safely lift and move the person What equipment is needed-trapeze, lift sheet, mechanical lift How to position the person If the person uses bed rails What observation to report and record Other measures Ask co-workers to help before starting Cover and screen the person to protect their right to privacy. Protect tubes or drainage containers connected to the person. Use the bed correctly Protect the person from falling when the bed is raised. Cont. Follow the rules of body mechanics Keep the person in good alignment Position the person in good alignment after lifting or moving 2
3 Raising the person s head and shoulders Moving the Person up in Bed Logrolling Is turning the person as a unit, in alignment, with one motion. The spine is kept straight. Older persons with arthritic spines or knees Persons recovering from hip fracture Persons with spinal cord injuries Persons recovering from spinal surgery 2 or 3 staff members are needed to logroll a person. 3 are needed if the person is tall or heavy. Sometime a turning sheet is used P
4 Sitting on the side of the bed Many older persons have circulatory changes. They may become dizzy or faint when getting up to fast. They may need to sit on the side of the bed for a few minutes before a transfer or walking. Problems with sitting and balance often occur after illness, surgery, and bedrest. Provide support and protect the person from falling and other injuries. Transferring persons Means moving the person from one place to another. Persons are often moved from beds to chairs, wheelchairs, shower chairs, commodes, toilets, or stretchers. Applying transfer (gait) belt Is used to support persons who are unsteady or disabled It helps to prevent falls and other injuries Goes around the person s waist You grasp the belt Called a gait belt when used to assist person with walking (ambulation) 4
5 Many agencies require staff to use these belts when transferring or walking a person Your clinical instructors will demonstrate how to apply and transfer or walk you re a person. Transfers Assess the situation Can the person help How far? What equipment will I need? Move furniture, tables, etc. is necessary Transfer belt should be snug Nonskid footwear Cont. take off & landing points secure If using a lift, is equipment working properly? Do you know how to use it? Body mechanics: Work together Make use everyone understands what to do 5
6 Cont. Count to coordinate move Transfer person toward strong side. Ex. if you are transferring a person from the bed to the chair and the person s left side is weak, you would transfer them toward their right side, so they would be able to assist by grasping on to the arm of the chair with their right hand. Cont. Assist from the weak side Tighten transfer belt when you are ready to do the transfer to minimize discomfort Make the belt more snug than you think you need to, when they stand up it loosens Make sure you have enough help. Positioning Person s that cannot reposition themselves need assistances. Whether in bed or chair, the person is repositioned every two hours Some are repositioned more often Follow the nurses instruction and careplan 6
7 Fowlers position Is a semi-sitting position The head of the bed is raised degrees Persons with heart and respiratory disorders usually breathe easier in this position. Supine Position Is the back-lying position Also known as the dorsal recumbent position The bed is flat The head and shoulders are supported on a pillow Prone Position The person lies on the abdomen with the head turned to one side This position is NOT used for the elderly, unless specifically ordered. This can compromise a person with heart, or breathing difficulties. 7
8 Lateral Position Is a side-lying position on right or left side Four pillows are used: A pillow is under the head and neck A pillow is under the upper leg and thigh A pillow is under the upper arm A pillow is positioned against the back Repositioning in a chair or wheelchair 8
