1 POSITION DESCRIPTION Position details: Title: Department: Reports to: Location: Paediatric Anaesthetist Paediatric Anaesthesia Service Clinical Director, Paediatric Anaesthesia Starship Children s Health Date: April 2018 Purpose of Position: To provide expert anaesthesia opinion and service in accordance with nationally recognised standards to paediatric patients, based at Starship Children s Hospital. There will be additional service requirements at other locations where the Division of Paediatric Anaesthesia is responsible for providing service to paediatric patients. This includes, but is not limited to Auckland City Hospital and Greenlane Surgical Unit (GSU) at Greenlane Clinical centre. Key Accountabilities: The key accountabilities for this position fall under the following general areas of: Clinical duties Administrative duties Quality and Educational responsibilities Clinical Duties: To provide pre-operative, peri-operative and post operative management of paediatric patients undergoing surgical procedures in accordance with professional standards as identified by the Australian & New Zealand College of Anaesthetists. To provide a similar level of service to patients requiring anaesthesia services outside the operating room, including but not limited to anaesthesia and sedation for CT scans, MRI scans, radiotherapy, interventional radiology, cardiac catheterisation and other procedures as deemed necessary. To provide the above service based at Starship Children s Hospital but in addition to participate in the delivery of Paediatric anaesthesia at other locations within ADHB ( Adult Anaesthesia, ACH and GSU), as deemed necessary in discussion with the Clinical Director of Paediatric Anaesthesia and OR s To provide consultative supervisory and direct service to assist with the acute pain management of paediatric patients at Starship Children s Hospital to support. This aspect of service is mostly managed by the lead clinical for Acute pain, pain nurses and anaesthetic trainees but additional SMO input and assistance is intermittently Page 1 of 5
2 required. To provide immediate and remote medical supervision of anaesthetic trainees in accordance with guidelines of the Australian & New Zealand College of Anaesthetists. To assist in the provision of a consultative service for the pre-anaesthetic assessment and management of paediatric patients. To participate in on-call and after hours roster as detailed in the individual contract, to enable the department to provide a comprehensive on-call and emergency service, 24 hours a day, 7 days a week, 365 days of the year. Administrative and Professional Duties: To participate in administrative duties of the department as required to allow the normal functioning of such a department. To maintain those administrative responsibilities required to ensure a high standard of professional care in accordance with the Australian & New Zealand College of Anaesthetists (ANZCA) guidelines, the NZMA Code of Ethics and any other statutory or regulatory requirements. Contribution to hospital committees, health commissions, professional associations and the activities of other organisations as deemed necessary. To contribute to departmental and organisational Quality Improvement activities. To maintain personal knowledge and skills by involvement with an appropriate continual professional development (CPD)) programme to ensure ongoing eligibility for organisational accreditation and Medical Council vocational registration. This usually involves satisfying the requirements of ANZCA CPD program Quality and Educational Responsibilities: To ensure that individual education and competencies are maintained through appropriate use of CPE funds and attendance at appropriate meetings (departmental, organisational, national and international). To participate in the continuing quality improvement programme of the department which should include but not be limited to Critical Incident reporting, audit processes, and M&M meetings as deemed necessary. To participates in educational programmes for trainees and specialist staff of the department of Anaesthesia. To assist and participate in appropriate educational activities for other medical groups within and outside Starship Hospital in the area of Paediatric Anaesthesia and related activities. PERSON SPECIFICATION Education & Qualifications Completed training in a recognised Anaesthesia programme Essential FANZCA or equivalent Desired Additional training and experience in Paediatric Anaesthesia 1 year post fellowship training overseas 2 years post fellowship training, one of which should be undertaken overseas. OVERVIEW Specialist Paediatric Anaesthetist Position Starship Children's Hospital Starship Children's Hospital is a modern 180 bed hospital run as part of Auckland District Health Board's medical services. It is situated in Auckland's Grafton district, adjacent to the Auckland City Hospital and across the road from the School of Medicine. Page 3 of 5
4 The department of Anaesthesia provides anaesthetic services to Starship Children's Hospital and elsewhere in the Grafton site of ADHB (such as MRI, radiotherapy and interventional radiological procedures). Most paediatric surgical sub-specialties are represented at Starship Children's Hospital and include cardiac services, general paediatric surgery, genitourinary surgery, orthopaedic, ENT and dental surgery. Some paediatric subspecialties, for which the department is responsible, are delivered outside Starship Children s Hospital. These include Neurosurgery and Transplant Surgery on level 8 at Auckland City Hospital. In addition and increasing amount of paediatric same day surgery is being undertaken at SSSU at the Greenlane Surgical Centre. This involves Paediatric Ophthalmology, ENT, general and orthopaedic surgery. (Paediatric plastic surgery for the region is not provided by ADHB and is mostly provided from Middlemore Hospital, a separate District Health Board. An increasing number of paediatric plastic surgery lists will be undertaken at Starship). The Department of Anaesthesia runs an acute pain service with the assistance of a pain nurse working alongside a group of specialists. It is expected that any specialist paediatric anaesthetist will be involved to assist with the acute pain service, although leadership for this service is undertaken by a lead clinician for the Acute Pain Service. Work with chronic pain patients is not expected unless arranged separately in the employment contract. Starship Children's Hospital has 7 operating rooms and performs just about procedures per year. There is a daystay unit attached to the operating room through which patients are admitted prior to surgery. Those patients undergoing daystay surgery return to this unit prior to discharge to home. A proportion of the surgery is performed as day case surgery, and an increasing number of patients hospitalised post operatively, are now admitted on the day of surgery (DOSA). In addition to this, approximately 1200 patients per year undergo anaesthetics for nonoperative procedures in remote sites. These include CT scans and cardiac catheterisations within Starship Hospital plus MRI, radiotherapy and interventional radiological procedures elsewhere on the Grafton (ACH) site. The department consists of 22 specialists representing 15+ FTE, 6 anaesthetic trainees and 1 fellow. Assistance is provided by anaesthetic technicians and theatre nursing staff. Currently the cardiac anaesthesia service is provided by 7 specialist anaesthetists from the above group and there is no plan, at the present time, to increase this number. A successful applicant joining the department will be expected to take part in all other aspects of departmental work. Page 4 of 5
