1 Proposal CFHI Internships for Rice University Center for Civic Leadership Students CFHI Contact person: Robin Young, Assistant Director, Africa & Asia ext. 7 Child Family Health International is a leader in global health education, facilitating international internships that meet rigorous ethical, safety, and academic standards. CFHI s socially responsible approach and reciprocal, long- term partnerships with communities, ongoing support of local healthcare infrastructure, US and local staff structures, as well as time- tested policies and procedures protect the wellbeing of both students and host communities. Our global partnerships with physicians, clinical and public/community health sites are established and reliable, and CFHI offers the expertise needed to administer global health programs to match academic needs. 1
2 CFHI Global Health Education Programs CFHI On- Site Partners At each program site, CFHI has two local leaders who work a team to run all elements of the program. CFHI local partners have worked with the organization for many years and are in regular communication with CFHI headquarters in San Francisco. CFHI conducts regular site visits by our US- based staff to each program. The local Medical Director oversees and arranges all clinical and public health- related activities and is also responsible for coordinating healthcare services for participants as needed. They oversee a cadre of preceptors, who are the one- on- one mentors and supervisors for interns within both clinical and public/community health sites. They maintain strong partnerships with on- site preceptors (supervisors) at each clinical/public health internship site. The CFHI Local Coordinator manages the logistics of housing, transportation, and cultural immersion throughout the program. He or she is responsible for arranging airport pickup and drop off, accommodations, transport to and from internships, and logistics for all program activities. The Local Coordinator acts as the go- to person for any questions or issues related to cultural immersion. The Local Coordinator conducts a weekly reflection meeting and addresses any issues with accommodation or transportation logistics with the participants. Global Health Competencies CFHI programs are designed to provide participants with concrete skills for global health work and domestic work within multicultural and international populations, and to broaden and deepen interns understanding of the context of this work and the populations being served. By the end of their program, interns will be able to: With respect to patients and populations, articulate the relationship between health and the social determinants of health. Demonstrate an understanding of cultural and ethical issues in working with and learning within the context of underserved populations. Demonstrate an understanding of barriers to health, as well as healthcare delivery strategies and systems in low- resource settings and how these influence access to care. Demonstrate a commitment to professional behavior while working collaboratively with health care team members and being respectful of differences in knowledge, practices, and culture. Identify the major causes of morbidity and mortality affecting the populations encountered during the Global Health Education Program or Internship. 2
3 Program Components Pre- Departure Orientation: Each participant will also have access to the following pre- departure material in their online profile: Reader #1 Logistics- Includes important information regarding travel to their internship site including visas, recommended vaccinations and other on- site logistics. Reader #2: Global Health ethics- Summary of global health ethics and 5 peer- reviewed scholarly articles that will inform their experience in- country. Reader #3 - Description of the country s healthcare system with emphasis on the specific program topic along with 5 peer- reviewed scholarly articles. GAPS Instructions (Global Ambassadors for Patient Safety): An online module series offering various case studies to prepare students for their clinical experience. Globesmart: An interactive tool to help increase cross- cultural communication skills and build meaningful personal and professional relationships in new cultural contexts. CFHI staff is also available to answer any questions via or phone. On- Site Orientation: Upon arrival, CFHI s local team will hold an orientation meeting. At this orientation, the following topics are discussed: Health and safety, emergency procedures, cultural immersion, local transportation, house rules, clinical/research schedules. Lectures, Cultural activities, Educational field trips: Site visits, interdisciplinary and experiential components gave students an in- depth look at various clinical and public health challenges faced in and around the program site. This program will include lectures from local health experts including the program Medical Director and preceptors, and participation in various health education and outreach activities in the local community. 3
4 Internship Opportunities for Rice University Center for Civic Leadership students through CFHI programs OMNI MED: Community Health Workers & Global Health Mukono District, Uganda Uganda s health system is being built, in part, by nonprofit organizations working in partnership with the government. A key resource of the Ugandan healthcare system are Village Health Trainers (VHTs), who are elected by their local villagers and serve three primary roles: to spread basic preventive health information, to refer sick patients to local or sometimes distant health centers, and to track health information for the Ministry of Health. VHTs play a vital role in Uganda's health system and CFHI s partner Omni Med has perfected the approach of garnering the support of international interns to train and support VHTs. The nonprofit organization Omni Med trains 1 VHT for every households locally through a weeklong course, then offers significant ongoing training for all VHTs. Interns at this site accompany VHTs to do training and community health education in surrounding communities. They also participate in home visits to carry out health education and monitoring of basic health indicators. Interns will help maintain the VHTs' knowledge base by leading trainings during meetings. Participants will also see and participate in the full scope of Omni Med's local activities, including construction of protected water sources, construction of stoves, and distribution of insecticide treated nets (ITN), among others. Omni Med has long believed in studying its impact, having conducted two clinical trials measuring program efficacy thus far. Interns will be exposed to, and will help facilitate, ongoing research initiatives while on site. This is a rural program. Interns stay at the Omni Med/CFHI house in the Mukono district of Uganda, about 2 hours outside of Kampala. Language classes will take place either in the village or at the house. Ideal candidates will have experience with community- based or public health education/outreach, and/or global health impact evaluation. Fee for 9- week internship: $5,990. Fee does not include flight or visa. 4
5 Community Health Outreach, HIV, and Nutrition Kabale, Uganda Uganda has the 20th highest maternal mortality rate and the 15th highest infant mortality rate in the world, according to the World Health Organization, with the majority of people living on less than $2 per day. Given high incidence of malnutrition, anemia, HIV/AIDS, malaria, and other diseases, many pregnant women are at risk of complicated births that lead to illness or death. Approximately 45% of children under the age of 5 suffer from severe malnutrition. There s no shortage of food in Uganda, yet the majority of its poorest households are reported to be food insecure, with poor health and disease exacerbated by malnutrition. Become a part of the community in Kabale, a town located in southwestern Uganda, and intern at a local nonprofit organization. While in Kabale, interns will learn about their groundbreaking, integrated approach to addressing and improving maternal and child health and food insecurity. Interns in this program conduct health outreach alongside health workers in rural villages and explore how nutrition and HIV and other illnesses are closely linked. Interns assist staff in their efforts to treat and prevent maternal and child malnutrition through education and counseling. Visit local primary and secondary schools and participate in Nutrition Education Outreach using participatory drama and theater. Learn about sustainable agriculture, novel protein sources in the form of a rabbit breeding center, and permaculture with a local social enterprise organization. Participate in workshops with village community groups and explore methods of growing a diversity of foods. In this semi- rural program, interns stay in dormitories, located on the NGO premises. Kabale is a short walk away. Language classes will take place in Kabale or on- site at the NGO offices. Ideal candidates will have experience with or knowledge of maternal/child/infant health issues in a global context, and/or experience or knowledge of issues surrounding agriculture and food security in the context of low- resource global communities. Fee for 9- week internship: $6,290. Fee does not include flight or visa. 5
6 Public Health Delivery Innovations New Delhi, India India is the world s largest democracy and is considered a newly industrialized country. Yet despite its large workforce and immense growth, India continues to face the hurdles of poverty, illiteracy, malnutrition, and inadequate public healthcare. Funds for health in India have long focused largely on medical services, reflected in a lack of modern public health regulations and delivery of services. In Delhi, India, the second most populous city on the planet, join organizations working to implement social reforms and improve public health outcomes through educational campaigns and trainings with emphasis on the spread of HIV/AIDS in India, tuberculosis, and management of infectious diseases. Depending on their area of focus and interest, interns will learn about key public health challenges such as low rates of immunization, poor sanitation and waste management, unsafe drinking water, and the social determinants and impacts of communicable and non- communicable diseases. Understand the lingering impact of India s caste system and challenges in implementing public health initiatives within a culturally and linguistically diverse population. Interns can choose one of three tracks in this program, which will inform their placements: Sanitation & hygiene; gender and women s empowerment; or HIV/AIDs. Interns will engage in capacity- building activities including education, training, and outreach, as well as supporting regular operations of the organization or organizations where they are placed. In this urban program, interns stay in apartments in Saket, New Delhi. Hindi classes take place either at the apartment or at a nearby NGO office. Ideal candidates will have experience with, or knowledge about nonprofit organizations/ngos in low- resource, global settings, or India in particular. Demonstrated interest in issues of gender, sanitation/hygiene, and HIV/AIDS will also be useful for this internship. Fee for 9- week internship: $6,290. Fee does not include flight or visa. 6
7 Sexual Health as a Human Right Quito, Ecuador After a hard- fought campaign, in 1998 Ecuador became one of the few Latin American countries to explicitly guarantee sexual and reproductive health rights to all citizens in their constitution. Local nonprofits advocating for these rights for years began seeing increased support from both federal and state health ministries. Join a pioneering non- governmental organization, focused on sexual and reproductive health issues in Ecuador. Learn first- hand about the challenges of carrying out these initiatives in a predominantly Catholic and socially conservative country where stigma about sexuality, lingering gender inequality and traditional gender roles continue to impact access to family planning services and sexual health education, resulting in preventable STDS, unwanted pregnancies and continued prejudice against LGBT communities. The organization focuses on providing low cost health care services to women and youth from underprivileged and marginalized populations, while at the same time increasing awareness about sexual and reproductive health through education and outreach. Their mission is to offer free and low cost family planning services to women, with 50% of services provided to adults under age 25. It currently runs an integrated sexual education outreach program and provides reproductive health services in semi- urban and rural clinics in 11 provinces. They also run a Youth Health Promoter Program which mentors youth leaders to be health promoters and educators, raising awareness amongst their peers. Interns assist local staff with the sexual health education programs in clinic, researching, preparing, and discussing health education materials and giving oral presentations to patients in waiting rooms on topics such as family planning, maintain a healthy sexual lifestyle, pap smears and breast exams. Join health teams in answering questions from youth at informational booths at local fairs. This internship includes 30 hours of Spanish classes per month at a local language school. 8 Primary Care and Social Medicine Cordoba, Argentina Like many other countries in Latin America, Argentina has developed a universal healthcare system and invested heavily in primary care over the last decade. Primary care providers are a pivotal component in the overall strategy of Argentina to provide health for all. As part of this approach, Argentine medical schools encourage students to pursue a career in primary healthcare, whereas only a fifth of US medical students choose this specialty. Join this program and gain insight into Argentina s primary care network and how the goals outlined in the 1978 Alma- Ata Declaration are being carried out. Become an intern at the State Ministry of Health, the entity running federal and state public health outreach and education programs. Join mobile clinics run by the ministry of health providing free services such as blood pressure screening, vaccines, mammograms and nutrition counseling. In addition, work with the Social Services Department within various secondary hospitals in the city, which run education and outreach programs, such as monthly diabetes education groups, a free diabetes medication program, sexual health education, and healthy eating campaigns. Become immersed in Argentinian culture and language through conversational and medical Spanish classes while living with local students or families in Cordoba. Interns may also organize weekend trips to destinations such as Villa Carlos Paz lake region, La Cumbrecita and other hill towns for hiking and outdoor activities. Within Cordoba city, enjoy the vibrant cultural scene including museums, theaters and cafes. Spanish classes take place at a nearby language school. 9 What does the program fee include? 10 Program Logistics Accommodations Depending on the program, students may stay in apartments, dormitories, guesthouses, or with homestay families. All housing is thoroughly vetted and meets CFHI s rigorous safety standards. Rooms are either single or shared. Beds are not shared. Transportation CFHI s partners carefully vet all transport providers and require that drivers meet all local laws and regulations, holding all necessary licenses and insurance. In addition, transport providers are thoroughly reference- checked. Students and local partners are required to wear seatbelts at all times while in vehicles. Travel at night is strictly prohibited on CFHI programs. If use of public transport for program- related travel is not considered safe or reasonable, CFHI provides transport to interns. Student safety: CFHI strategies for minimizing on- site risk Local partners: CFHI staff regularly visits each site to oversee safety and quality as well as develop relationships with our local partners. The longevity, open communication and close relationships with our on- site partners is an institutional asset when coordinating efforts to institute safety policies or responding to on- site emergencies. CFHI San Francisco is in regular contact with all of our international partners throughout the year. While CFHI monitors the political, health and other situations occurring in each country, our local partners serve as the most valuable tool and act as our eyes and ears when monitoring local situations. CFHI offers regular webinars and other professional development focused on health and safety for its global partners. Pre- departure information for students: CFHI sends participants pre- departure orientation materials with information regarding on- site safety issues and guidelines, cultural norms/customs/dress, recommended vaccinations, arrival pick- up procedures, as well as contact information for on- site partners. Provide important student information to partners: CFHI also provides our global partners with important information to ensure that any health needs, dietary restrictions or food/other allergies can be accommodates. Two important reminders are sent to students prior to departure: 1. Participants are also responsible for staying safe by ensuring they follow safety guidelines provided during orientation and the choices they make while abroad 2. Consult their physician and create an action plan for any specific mental or health issue while abroad. IVC Emergency Medical and Evacuation Insurance: All CFHI participants are enrolled in IVC Emergency Medical and Evacuation Insurance Coverage one month prior to departure. For those participants in programs longer than 8 weeks, CFHI enrolls them in the same plan but for longer than 90 days so that they are covered for the duration of their program: 10
11 Safety net for student: Students have a great support system through the program. This safety net is comprised of On- site Local Coordinator/Medical Director/other support staff on call 24/7 On- site Medical Directors available to assist and coordinate care for student in case of medical emergency Counselor/mental health specialist available as- needed Emergency protocols updated and regularly reviewed with local team 24/7 CFHI U.S. Emergency Line Care/comfort/guidance of on- site local team Student orientation upon arrival: Students are thoroughly briefed upon arrival during orientation about safety guidelines they must follow throughout the program. Cell phone: During the orientation meeting, students receive a local cell phone pre- loaded with minutes upon arrival with emergency and other useful numbers programmed in. On- site emergency procedures: All our partners work collaboratively with CFHI to ensure that all medical and non- medical emergencies are responded to in a timely and structured manner. In case of emergencies, CFHI San Francisco and our partners on- site work as a team to ensure that the best care is offered to the participant and that all parties coordinate not only the provision of services or care, but also to maintain close communication with participant s university/parents/insurance companies. Key emergency contact information is publically posted on- site including phone numbers, place of contact, etc. Safety Policies: We work and coordinate efforts with our partners to provide a safe environment for learning and to minimize on- site risks to participants by implementing specific safety policies at all our program sites.
