3 BACKGROUND The Sutton Clinical Commissioning Group s structure has been split in to three geographical localities Sutton/Cheam, Carshalton and Wallington. Each locality has a dedicated GP Lead as follows: Dr. Jeffrey Croucher Sutton/Cheam Dr. Ashraful Mirza Carshalton Dr. Farhan Rabbani Wallington To support the three GP Leads, Sutton CCG has appointed a Locality Manager to work across all three localities and a Locality Assistant as follows: Carolyn Reynolds Locality Manager Jillian Slade-Thornett Locality Assistant. The role of the Locality Leads and Locality support staff is to ensure collaborative working and engagement with all practices within the Sutton CCG so that the commissioning priorities and QIPP programme can be delivered, whilst recognising the differences of individual practices and localities within the CCG. A payment of 1 per patient has been allocated to each practice within the Sutton CCG for engagement as follows: Practice Leads in commissioning (or a practice representative) to regularly attend and participate in monthly locality meetings and relay information and action points to the Practice s clinical and managerial team. Practice Manager or practice representative at monthly practice manager forums and sharing of information with the team Practice Nurse attendance at monthly forum and sharing of information Supply monthly referral data to the CCG as requested Timely response to audit requests, e.g. high cost patient audit enquiries Practice attendance and participation at quarterly plenary These payments will be made in two tranches (April to November will be paid this month) the second payment (December to March) will only be made to those practices that are able to evidence engagement as above. The process for agreeing the payments to the practices will be developed for the remainder of the year, and this will aim to incorporate outcome measures. ALL LOCALITIES Monthly meetings of the three localities have taken place as follows: Carshalton Sutton and Cheam Wallington The GP Mental Health Lead has presented to the Sutton/Cheam and Carshalton localities asking for feedback on the services provided by the South West London Mental Health Trust as part of their bid for Foundation Trust status,.and the Lead GP Page 3 of 5
4 for End of Life Care has presented on the requirements of the Local Enhanced Service which includes utilisation of the Coordinate My Care initiative. The same presentations will be made to the Wallington Locality in December. Locality Lead Nurses have now been selected for all three localities. A Lead Practice Manager has been appointed for Carshalton, and the process of selecting and approving two further Practice Managers for Sutton/Cheam and Wallington will be finalised in December. A Locality Newsletter will be produced on a bi monthly basis for distribution to all practices and stakeholders as a means of information sharing. SUTTON AND CHEAM LOCALITY Sutton and Cheam Locality have been meeting on a monthly basis with an excellent attendance rate including GPs, practice nurses and managers. Standing items on the Agenda are practice out-patient referral rates, A & E attendance and admission reports which are produced and presented numerically and graphically by the Head of Performance for the SCCG and the PCT Pharmacist updates the group on the current prescribing position by practice. Updates are also given regularly from the QIPP Leads on Urgent Care, Planned Care, Mental Health and Acute Contracts. The Nurse Lead on Long Term Conditions presented data on emergency admissions which shows the numbers of patients being admitted compared with the previous two years. Practices are required to investigate any increase in numbers and address why this might be happening. Practices are required to use the CPM tool to identify those patients with LTCs and monitor their progress. This will be a standing item on the monthly meeting agenda. The Locality Manager attended her first meeting for Sutton/Cheam on 14 th November and gave an up date on the NHS 111 mobilisation and GP Out of Hours procurement. CARSHALTON LOCALITY Practices are attending the locality meetings with at least 2 practice members at each meeting including GPs, Practice Managers and Practice Nurses. The group appear to be engaging and working well together. Referrals are graphically presented and broken down to all individual referring clinicians and discussed with Sian high and low referrers are discussed and reasons/advice exchanged. Prescribing behaviour for each practice is also discussed. The PCT pharmacist advises the locality on Medicines Management. Two QIPP leads are invited to each meeting to go through their areas and obtain feedback from the GPs. The locality is used as a platform for collaborative working such as the QP areas and the practices have worked well together on this. The Locality Manager attended her first meeting for Carshalton on 27 th November and gave an up date on the NHS 111 mobilisation and GP Out of Hours procurement. She also gave out LTC data on behalf of the Nurse Lead on Long Term Conditions as above, and this will be a standing item on the monthly agendas. WALLINGTON LOCALITY 7 of the 9 practices are engaging well within the Wallington Locality. The GP Lead and Locality Manager will meet with the two practices who are not engaging after the letter to practices re payments for engagement has been issued. GP performance is reviewed at each meeting using graphical reports produced by the Head of Performance. Good peer to peer support and sharing of good clinical practice. All practices have identified a key area for audits to support reduction of referrals. Two QIPP leads attended to discuss the QIPP agenda/plans for Planned Care. PCT pharmacist is a standing item on the agenda. Mental health and EOLC will be Page 4 of 5
