4 4 LPRES Digital Health Lancashire Person Record Exchange Service A Digital Transformation Programme Background An agreement was gained from the Lancashire & South Cumbria Digital Health Board in July 2014 to support a bid to the IDC Tech Fund Two for matched funding for a Shared Record and Wellbeing platform. This interoperable enabling technology is fondly known as Lancashire Person Record Exchange Service (LPRES). We started publishing in March Aims By collaborating and sharing information about patients and service users across care boundaries, our aim was to provide care givers with access to the right information, at the point of care delivery, leading to: Improved continuity of care Safer services for our patients Reduction in unnecessary diagnostic tests Less paperwork and more efficient services Greater access to data for patients, their carers, and providers of health and social care services
5 5 High-level Programme Principles Collaboration as a founding principle Do once and share Don t duplicate Do things at scale wherever possible Maximise value of legacy investment Ensure the public know what we are doing (no surprises) Engage the public Engage IG specialists / leads Adopt a single unified, electronic approach to IG via investment in and development of the Information Sharing Gateway - Quote taken from the ISG website: The Information Sharing Gateway (ISG) has been developed by a sub-group of organisations in the Lancashire & Cumbria IG Group in order to improve and modernise the administration and risk assessment of information sharing in the public sector. It has been designed by IG specialists, for IG specialists, to support their IG reporting on data flows and information sharing (principally to IG Toolkit). The development was funded by the LPRES programme in Lancashire, to support the use of the LPRES healthcare information exchange platform. It is a generic tool and is not tied to use with LPRES, although it is developed with the needs of sharing via interoperable systems in mind. It is a next generation Sharing Framework that will adequately support electronic information sharing across care boundaries in a way that current, paper-based systems cannot. The purpose of this system is to provide assurance that the information being shared, managed and processed will be done so in such a way that is Data Protection Act compliant. It centralises and shares key resources in a way that is accessible and transparent.

10 10 Benefits to front line staff Benefits to front line care givers in health & social care: Effective Information Governance (IG) is supported: Between different organisations Between care providers and patients Shared information used to inform future strategic health priorities for our patients Better and faster/real time clinical decisions based on comprehensive and more timely information Improved continuity of care for patients across organisational boundaries Improved outcomes, such as: Reduction in appointments Increased safety and reduced risk in relation to vulnerable adults and children Reduced length of stay due to accurate, up to date information to aid clinical decision making Improved patient outcomes due to improved self-management Reduced number of diagnostic tests carried out Reduced number of missed appointments across organisations (Reduced DNAs) due to increased patient engagement and commitment to the wellbeing Quote of the week, Renal Consultant, LTH to CCIO:..Just to say Loving LPRES helped me see acute in-reach referral on EDU today and indicate no further renal follow up required and might be suitable for Out Patient Management pending Physio/Occupational Therapy. A patient who lived in Blackburn and was visiting friend locally and became unwell.used it frequently in Out Patient clinic too as we have several patients from around the region eg. Blackpool discharge letters very helpful given the Cardio Vascular input our renal patients need
11 11 Benefits for patients Benefits for patients: Improved patient experience due to: Less repetition of health/medical history each time treatment is accessed in different organisations. Up to date information is available via the interoperability solution - alongside the necessary data sharing agreements once consent is given. Care is co-ordinated between providers giving patients greater reassurances, confidence and trust in the care givers treating them Greater access to health information, data and knowledge about condition management, helping to maintain health and wellbeing and not just providing treatment for illnesses when they occur Improvements in the patient experience due to reduction in unnecessary admissions to hospitals/care providers The provision of treatment can be delivered in the most appropriate care settings as information can be accessed where and when it is required.
